Loose Wheel On The 2006 Chevrolet Uplander LT

Mom brought to my attention that when driving it felt like one of the wheels was going to fall off and was making a grinding noise and vibration. Upon Investigation I found one of the front wheels was loose. The could turn all 6 lugnuts by hand. That's how loose they were. Did I forget to torque that wheel down when i changed the brakes? Did someone try to take the wheels off and got scared off by my neighbor's dogs? Very interesting.
