    Dear Dr Lim
    This is actually a painful topic for some women. Thank you for your video. It actually is more than just skin deep. When a woman has a brown upper lip, it actually cuts at her heart. Your job isn't just surface, it helps with self-esteem and self-worth. Thank you for this video on pigmentation. Please, keep us up to date on advances in pigmentation treatment. In South Africa, there isn't a picosure laser, so treatment is limited. Your videos on pigmentation mean more to women than you realize.

      Thank you... yes, I do really understand this. I realise the trauma of this condition, as I have patients flying in from around the country seeking help. Pico is not really needed, its a advanced procedure, but nano can be effective. Dr Davin Lim. Brisbane. Australia.

    Melasma can be difficult to diagnose and tricky to treat. Pigmentation such as freckles, sun damage etc.. can be treated by non- specialist, but melasma should be medically managed as this condition can flare up if treated incorrectly.     here in the states, at least in Chicago, it's impossible to find a doctor who knows how to use this for melasma. it's only available for tattoo removal. I've called half a dozen places looking for pico sure treatment and all I've heard is "it's only used for tattoos" ...
    and the before and afters are not always that great, unless you have very pale skin.

    Thank you! I had NO idea IPL was not recommended!! I had one session several years ago, and my melasma must be the surface type because it did lighten it up enough so that I wasn't quite as bothered by it, but now I'm motivated to look into the other machines and peels in the video. I also used Tri-Luma years ago and it was so-so. Last year I started using Missha Time Revolution Night Repair and after a month I was surprised to see that it also helped.

    Thank you! I have a question. I have a brown spot on my knee which I am not sure it is Melasma(but it's the same as the spots that you explained) or not. Also, There are some small brown spot that recently appeared on my right hand and some on my face. Also, I don't have any child and I always use sunscreen. Is there any treatment about brown spot?     Dear doctor Davin I had a chemical peel for skin,unevenness about 6 months ago and was priscriped tretniod about 3 months ago my skin started to feel tight dry and,it won't,go away I stop using evething. also I started getting broken capillaries on my cheeks. I feel like it ruined my skin iam 22 years old and Hispanic and don't know what laser or treatment could help me with the facial tightness dryness and broken capillaries I wish I could go see u but I live in the us

    Hi Dr. Lim, hope you can answer some of my questions about melasma treatment. I'm currently doing picosure+piQo4 laser and just finished my 3rd round (4 weeks apart) but I still see the melasma! Im debating if I should continue to waste my money for the 4rd treatment I have scheduled. For the kind of money i have spent $800 per session) doesnt justify for the result i have. It got lighter, but by no means gone. How many sessions are usually recommended to do? Thanks!

    I've seen a dermatologist about hyperpigmenation caused by sun damage. They have recommended the elos SRA laser or topical tranexamic acid. Should people who have darker skin (type 4/ type 5 ) avoid these types of lasers?

    Hi Dr, I simply love your videos. I have a few questions.
    1. Would microneedling help with melasma especially if used together with HQ, Tretinoin and vitamin C?
    2. What is the best treatment for dermal melasma as I understand that it is the type which is very difficult to treat?
    3.I have heard that use of lasers are not advisable in those ethnic pigmented skin, eg. Latins and Indians as they might cause PIH? Is this true?
    4.Can one crush tranexamic acid tablets to use on the face (mixed with water like a paste and applied to skin though understandably oral TXA works best?
    5. what would you do for resistant and rebound melasma?
    Keep up the good work Dr. Your channel is my holy grail for getting the best advice for skincare.

      WOAH!!!!!!! Can't comment on this one, TCA CROSS should be done by specialists, that is why I educate on doing dilute and SAFE TCA PEELS, too much or accidental spillage can lead to disfiguring third degree skin graft burns.     Dr. Lim your explanation of the different type of laser is very helpful. I myself have been battling with skin pigmentation for the past 8 years. I had few dark spots on my face after giving birth to my daughter. So I went to see a Dermatologist. They prescribe Triluma. I must have an adverse affect bc now my spots are much bigger and darker. So now I finally decided to go with Laser. But your vdo says to stay away from IPL for malasma. So is Pico the best thing to use?

    Thank you so much for making this. I live in Florida and the sun is unavoidable. I wear sunscreen daily, use retin-a, vitamin c, and have used hydroquinone in the past. Nothing seems to work for me. I can't afford to get laser, unfortunately. Is triluma a prescription product?

      kimberlee533 I'm not Doctor Lim, but make sure you know the amount of UVB protection your sunscreen has. UVB protection protects the skin for tanning. Good sunscreens from Asia rate the UVB protection with "+" and sunscreens from Europe use a number system. If you are using products that are photosensitive like retin A and hydroquinone, but your sunscreen's UVB protection is low there is a chance it could have been undoing your efforts. Remember retin A and hydroquinone make the skin easier to tan, so you need a really good UVB protection with your sunscreen and consistency.

      Thanks for the information. I am currently using the Elta MD broad spectrum spa 40. It claims of me for normal, combination, and post-procedure skin. I believe it is made in America so there are no + or numbers. It just says UVA/UVB protection. The other one I use is Cerave AM which claims to be developed by dermatologists.

      kimberlee533 no problem. Search for sunscreens where the PA factor is indicated and try to aim for the highest protection. Good luck

    Hi Dr. Lim, I have a dark brown spot on one side of my forehead (where I don't have bangs hanging over) and have been treated with a q-switched laser once. I don't feel like it got any better....maybe even worse. I looked pretty bruised afterwards and after one week the scabs came off an revealed pink skin that went right back to brown. I am living in Germany where it is winter currently. I have been protecting the area with SPF 50 by Eucerin. I have only used cosmetic treatments such as Caudalie yet with no result and am currently starting the Eucerin pigment line. Do you have any ideas what route I could take? The rest of my face is pigment free.....the area first started showing after half a day in the bright sun without sunscreen (no sun-burn involved). Thank you so much!

    Here in Afghanistan, dermatologists have a very limited resource so the only way they treat melasma here is with combination creams of Tretinoin + Hydroquinone and a mild steroid. Once a certain time limit is reached, the patient tapers down the steroid use and continues with Tretinoin + Hydroquinone. What are your thoughts on this, Dr?

      ....one should try to get off the HQ and steroid but use vitamin A - Tretinoin indefinitely. DL

    Love your videos! I am skincare junky and for several years have started focusing on anti-aging. I am Asian and of course struggle with hyper pigmentation. What would you recommend for under eye circle and bag? And how long should I use hydroquinone for? In what order should I apply it in my skincare? A good skincare regiment would be great and what products REALLY work. Thank you for the great videos! 😊

    Thanks Dr Lim - I was considering IPL, but now it's definitely off the list. Sunscreen daily SPF 30 (layered sunscreen, then tinted sunscreen, then SPF foundation powder) has certainly reduced pigmentation for me. Also, I stopped applying perfume to my neck!

    I am new to your channel. I live in Upland, CA and have monthly Tribeam laser treatments for several years for my Melasma. I apply 4% hydroquinone on a weekly basis and sunscreen every single day. I use to apply the HQ cream every day. So far the melasma is under control but its a daily battle especially during the summer months. Do you suggest HQ cream twice a week or TriLuma for maintenance purposes ?

    I especially appreciate the sunscreen info! I have melasma and am curious, how often would I have to do these treatments the keep the melasma under control after initial treatment plan? I hope that makes sense....

    Just curious - why is Triluma only a short-term treatment? Could it be used once a month or even once every few months on an ongoing basis? What is the ingredient in it that makes us unable to use it regularly?

    Dr i use uveil SPF 60. is this a good sunscreen? i have melasma an forehead temples and cheeks. my melasma is not in worst condition .tell me it can get worse if i dont treat. i use sunscreen regularly. i went to doctors twice they gave me creams like nano white and safriderm and tablets pico sth (sth that was extracted from plants). my melasma was faded a little but not gone completely. Should i visit dr again?

    Hello Dr, thank you SO much for this video. !! I cannot find anyone in the state I live in that offers the picosure laser. What are your thoughts on the accolade laser? Will this help clear my melasma? I have fair to medium skin tone. Dark blonde, green eyes. Sometimes burn in the sun. This is the best laser option I can find near me. I believe it is q-switch?       QLS in normal 1064 can do the trick... its not the laser that counts, its the specialist behind it... lasers are merely instruments, correct settings together with the combination of creams and SPF is the key to getting great results. I can get results with a cheap 30 k laser or a 250 k picosure, but the settings are the most important factor. (different for each laser). Dr Davin Lim. Brisbane Dermatologist. Australia.

    Now I'm a bit confused because I have what looks almost exactly this except that it's only part of my forehead and a bit on the side of my lip. However, it only appears if I've been in the sun (holidays) as my part of 🇨🇦 is rain, rain all year! Is melasma present all the time in the patient...winter and summer?

      Depends on the UV exposure.. this condition can be difficult to diagnosis as atypical variants occur. Sounds like melasma... but a close look will give a better idea. Dr Davin Lim

    Thank you so much for this video. I've had melasma for 13 years and tried IPl, which made it initially dissapear, then it came back. I'll look around for dermatologists who have QSL or Pico laser to try. Can the laser be done without hydroquinone? That ingredient irritated my skin.

      Yes, but you should use a tyrosinase inhibitor... your dermatologist will guide you. Or do vvvv low strength TCA peels, and add Lytera..

    Thank you Dr Lim, l always enjoy your informative videos. I have moderate melasma. I live in Sydney but am in Taiwan (where my family is from) at the moment. I was recommended using a Nd-Yag laser on my skin here. Is this similar to any of the lasers you mentioned in this video? Is it the same as a Q-switched Yag? Do you recommend me trying this laser over several sessions?

    Quick question, if I may? You mentioned about where melasma usually presents, but is it restricted to these areas? I ask, as I have incredibly similar looking pigmentation from behind one ear and down the back of my neck. People assume it's a birth mark, but I was not born with it, it developed in my school years. Just never really known what it is! Could it possibly be melasma?

      Sorry, I don't, I have quite a few patients flying from Newcastle to see me.... problem is a lot of Specialist dermatologist still treat ezcema, rashes, moles , skin cancer etc... and do no solely focus on laser dermatology. If its closer, Laser Dermatology (Dr Phil Bekhor) uses the same tech and methods I do, as we co authored the melasma paper on the dermatology college web portal . Davin Lim. Brisbane. Australia.

    I wish I lived in Australia so that I could come to your clinic, you inspire confidence. I have melasma only on my forehead and i have now had 4 picosure treatments in a clinic in Toronto. The melasma did not get darker and not lighter but bigger. I used 4% hydroquinone before treatments and in between. After four treatments with worrying outcome I am thinking of quitting. Not sure if there is a way to asses if its them or me? I would be thankful for any advice.

      Hi there, I am also from Toronto and would love to know where you had your treatments done. I suffer from Melasma on my forehead which is getting worse. I was always scared to do laser but now there are different Picosure lasers available. I heard that it takes time for the dark patches to get lighter because it has to do with your skin's natural shedding cycle (30 days and longer as you get older). Did your melasma ever get better?

      @@larasunshine7 hi my melasma only got better with professional deep peels at a dermatologist office. I tried all kinds of lasers to no avail, just when I thought my melasma got better it went dark again. The peel i tried you leave on for about 8 to 10hrs and then you peel for a week. But sadly it gets worse every summer anyways. If you end up finding something that works long term pls let me know ;)

    Hi Dr.Lim, I do laser esthetics here in Toronto Canada and watch all your videos! I use the Lutronic Spectra (q-switch) on clients for melasma as well. I was wondering what spot size and what energy are you using? Also do you prefer 5hz or 10hz? Is it possible to combine glycolic peels with laser? Thank you so much. Keep up the great videos!

      Each laser has its own fluence that is NOT standardised. ... go by the clinical effect. Hz depends on how fast you can move. Don't forget the studies on VEGF-1 receptors and T. acid modulation. All the best, Dr Davin Lim. BRISBANE SKIN specialist.

    Hi Dr, i have had small patches of melasma at corners of my eyes but after getting 1 prp and 2 sessions of low frequency fractional laser with 1 month gap between each session.,now its all over my cheeks... What should i do bcz my dermatologist is saying it will go after 2,3 more sessions but i think it will spread more... And dermatologist also said to control melasma i need to get laser session after every 2 months..what is ur opinion on this is it not damaging for skin if doing so often... TIA

    Hi doc lim, For my melasma I have started to take sessions with Thalgo I beauty treatment .they said it’s is not laser machine but some I beauty treatment and wit it my melasma will reduce to a great extent almost 95 percent and I have to use all Thalgo products etc.please suggest should I go ahead and take further other 7 sessions,or will it too aggravate my condition.

    I absolutely love you videos! Ok so I just went for a consult today on my fine lines, moderate rosacea, and moderate melasma. I'm wanting to do the CO2 fractional laser for fine lines but since I have moderate redness from rosacea, they wanted to do 4 sessions of IPL first to get rid of the redness...BUT you said the IPL makes melasma worse. What protocol would you follow to improve my 3 skin problems??? AND do you have any suggestions for doctors in the USA??? preferably Arizona?? I wish I lived near you!!! Thanks!!!

    +Dr. Lim I enjoyed video. I am sunscreen shopping and kept seeing in the comments on Amazon advice to buy a sunscreen with Mexoryl. L'real Ombrelle was the only one I found. I did see a La Roche moisturizer that contained it. I'm interested in your thoughts.

    Dr Lim, what's the difference between PIH and melasma in terms of treatment? Isn't PIH easier to treat? My derm equates PIH to melasma and says neither should be treated with laser because of hyperactive melanocytes, but I don't understand how there are hyperactive melanocytes in a 2 year old PIH from a temporary inflammation. If PIH was constant hyperactive melanocytes epidermal PIH wouldn't fade. It makes no sense.

      Let your dermatologist guide you, I drop my HQ, replace with vitamin A, C add lytera, lots of combinations that do not involve continual use of HQ.

    hello Dr, I m from India in my early 40s. I hv Melasma on my cheeks, forhead, n nose for the last 6 yrs. I was thinking to take laser therapy like IPL but now stopped thinking for it. I took orally Antioxidants tablets, n used SPF 30 sunscreen till now. And used a cream of Hydroquinon for 1 n half months. To my wonder this cream has removed all my Melasma for once. but after I stopped using this cream....my Melasma came back again n its worse than before....
    My Dr said no to laser therapy till I reach my age of 50 coz of side effects. Rytnow I m 41 n a mother of 16 yrs old child. Plz plz plz help me n suggest me best possible way of treating this Melasma. Its a humble request....
