We had one of these which cracked and wouldn't hold water so my mother filled it with sand and, during the day, stood it on the coke stove in the kitchen. at bedtime it was wrapped in a sheet and placed in the bed. It actually stayed warm longer than with water.

Ah, I know the answer to that one… when using this, you place it along the body to transfer the warmth, so the time is on its side. Having the opening on the side means you can place this part facing up so it doesn’t leak. A top opening would always have the potential to leak, when the warmer was on its side.
