I have been practicing Medicine for 26 years and I still review the basics. This was awesome.
Thank you for being one of the seemly rare doctors that does this! It's a mark of excellence.
Do you happen to know the best initial vs gold standard test? Is the initial 24 hour urinary free cortisol and the definitive is the dexamethasone suppression test?
@@rachelgreenbaum6148 That’s for cushing’s syndrome not adrenal insufficiency
Your comment changed my day ! I worked as a doctor in another field for 3 years and now I’ll be going to work in the emergency section. My self confidence got so low that I had to repeat stuff again from med school , I thought something was wrong with me and I felt ashamed . Now I see this comment and I realised that this is how it works .
Agreed, I’m an endocrinologist and I love videos from OSMOSIS. Every time I have a weird case in my practice I recur to osmosis to review the basics again.
I have Addison's disease! I'm 19 years old and was diagnosed in 2016. Nice to see more people being educated on this disease!
My cortisol level is too low in the test, what to do?
You need to see a specialist- an endocrinologist as soon as possible and have them do thorough testing.

@@torahibiki adrenal insufficiency with Db1 is difficult to control. I need at least Double the insulin due to my cortisone tabs.
Good mnemonic for etiologies:
ASTHMA
A: auto-immune
S: Suddenly stopping Steroids
T: TBC
H: Hemorraghia/HIV
M: Metastasis
A: adrenalectomy
I think these are ranked based on prevalence too, but not too sure about that.
Thank you very much

Wow. This was actually VERY informative as I’m about to get tested for Addison’s disease next week. It’s crazy because I was in the ER with acidosis at one point and a few months later I had low blood sugar issues 😳 now I’m dealing with low blood pressure, low potassium and orthostatic intolerance.
Hope our video was helpful, Kimmie! 🙏🏼
Same case with my husband, hospitalized several times due to seizures, low BP n low glucose level... don't know what to do
wait, in the video they say addisons causes high potassium, how do you have low?
@kshetrimayumm I hope you are doing well. How r u now. Did u get tested for ACTH,serum cortisol, and CBC?

Once again, Osmosis produces an amazing video, they just keep getting better. You guys are truly the best medical RUclips channel out there.

I was so upset when i started seeing your videos popping up right as I was finishing graduate school, but now they're becoming increasingly more useful while studying for boards! Thanks so much for all you do.

Your videos are literally saving my poor tired med student brain thank you so much!
I'm in nursing school and these videos are so helpful! Thanks so much for sharing.
Thanks! What school do you attend? We'd love to know more about how the videos relate to what you learn in the classroom!
Osmosis I go St. Petersburg college in Pinellas Park, Florida. Since I’m in nursing school most of our lectures over these diseases focus on patient management, interventions, and treatments. Mostly a lot of the “what can you not afford to miss”. I like how you guys give the patho and description of each disease. I do really love the videos and think you guys do a great job. Thanks for sharing!
Your videos have been so helpful for me while in Nursing school. Weird side note, I could listen to your voice/videos all day! Thank you for these!
Wow, thank you, Kimberly! 😊
Great video, was diagnosed in 2019 just before covid (thankfully), with adrenal insufficiency. for 2 years they blamed weed(CVS) cyclic vomiting syndrome. my sodium hit levels of 110 risking brain damage. Lowest i got was 104 and i felt close to death. Felt like my innards were rotting. All it took was one blood test to find missing enzymes in my blood. I've had a few near death experiences from bleeding out, overdoses and adrenal insufficiency. but this was the worst, so much suffering for me and my family. Finally healthy at 5'9" 188 lbs and on hydrocortisone and fludrocortisone. Thanks very much for this scientific and well made video on Addison's
Thank you for the great illustration. I had been introduced to an patient today in hosital ward. He has symptoms like weakness, black pigmentations in hand and oral cavity, orthostatic hypotention, low bp etc. now it seems fully clear to me
We're glad that this helped! 🙏🏼
Addison’s since i was 17, that almost killed me (1996). Here are some tips that after 25 years can help. I am also a Type 1 diabetic since 2008. I am 43 now…
1. Salt. Don’t be afraid of it. Pickles, kimchi, sauerkraut, chips and salsa i know i know you all love it. It’s salty AF! Your body doesn’t process salt like normal people. Enjoy it and when you feel faint or light headed you are low on sodium.
2. Don’t over due water. I know you hear how much water you need but it’s bullshit. Drink when you’re thirsty, our ancestors for thousands of years didn’t sit around drink 20 gallons of water a day. The more water you drink the more you flush salt out of your system. Deadly, be careful.
3. Stress, don’t be afraid to take extra hydrocortisone when you’re sick or stressed. No need to overdue it but an extra pill here and there is a life saver.
4. Sleep, you have a rare disease that cause fatigue like a mother f’er. Get your sleep preferably 8 hours and when you need it trust me your body will tell you. Do not take your hydrocortisone too late in the day or it will keep you awake.
5. Eat clean! Do not put garbage in your body, it will affect your Addison’s. I find a paleo diet works amazing for me but I’m also a type 1 diabetic. Trial and error, do you. Stay away from junk food.
There is lots more but this is the important stuff. Good luck.
I totally agree.
Did the steroids cause your type 1?
@@venomx4093 they are not sure. But i was on waaaayyyy more than i am now so my face used to be crazy puffy. Type 1 runs in my family. Obviously not one has Addison’s. I’ve NEVER met another human in person who had Addison’s
@@K.Stroh_ My new dr thinks I have addison's, but he is digging DEEPER into it now. It has been a long journey of misdiagnosing and prescribing the wrong treatments from different doctors. His suspicion came after seeing my vitiligo and hyperpigmented skin along with my symptoms that started in 2015. He doesn't think addison's was the culprit. My labs had shown hypothyroidism in 2015, 2016, 2017 and nothing was ever mentioned about it. I was prescribed statins back then which cause kidney failure. I've heen studying up on hypothyroidism and addison's and now see why my new doctor is digging deep. He had asked if I was diabetic and if it ran in the family. I am not "yet" but since my pancreas is inflammed and my pancreatic enzymes are messed up, I suspect it is coming. I just went gluten free and started taking enzymes, hoping to cut this off before it gets too far gone.
@@K.Stroh_ I know steroids is the main treatment for addison's and thyroidism and long term use has been shown to trigger type 1 dyabetes.
What does it mean when cortisol levels are dangerously low but adrongens (particularly DHEA) are optimal or even very high? I’ve seen this in numerous test results of pre-menopausal women, but the relationship between low cortisol and high DHEA has yet to be explained to me beyond it likely not being Addison’s. Good video, thanks.
Thanks for the personal touch and credit to the script writer at the end!
Glad you liked it! We're trying it out.
Thank you!!! Newly diagnosed and you all (all involved) explained everything well!

Always watch RUclips first to understand, then read
these videos can give you enough mental picture of the topic and you can recall easily and swiftly during the tests as compared lectures with rich text slides. Great job.
plus you can watch them over and over again as opposed to revisions from big filthy books which is tedious dull and boring
Awesome, so awesome. Although I finished nursing high school, I continued my education in technology, finishing my Phd soon. I sometimes miss medicine and all I can say is that your channel makes everything covered excellent. If I as a chemical engineer enjoy these videos, then those who are in medical schools will praise them. Will you be doing hypothyroidism soon?ASAP science and Osmosis are my favorites. Subscribed.

Amazing lecture. So precise and clear. Well done!
As always...your videos are always fantastic!! I am amazed by your videos! Thank you so much for helping us especially the undergraduates:)
great video. Aldoesterone acts of the collecting duct rather than the DCT.
Nice catch! Thank you… this is why I always read comments before watching videos. Saves me from confusion
7:04 - Does dificiency of aldosterone really causes hyponatremia? in our university we are taught, that aldosterone doesn't influence a natremia, because water always accompanies sodium, so if sodium leaves the organism, water follows it, so the absolute amount of sodium will be decreased, but natremia does not change
Yes. Those with Primary Adrenal Insufficiency/Addison's do not produce enough Aldosterone and therefore need a mineralcorticoid (Fludrocortisone) steroid to maintain their Sodium levels/ blood pressure. Those with Secondary AI, usually produce Aldosterone and therefore usually have normal blood pressure/don't need mineralcorticoid steroid. Both PAI & SAI need glucorticoid to replace Cortisol and both can drop their blood pressure during an Adrenal Crisis.
Watched this in class and presented 10 minutes after..got an A😆🤸🏽♀️😘

i know I'm kinda off topic but do anyone know a good place to stream new movies online ?
@Cullen Gus Try flixzone. Just google for it =)
@Cullen Gus i use Flixzone. Just google for it :)
Great video! I have secondary adrenal insufficiency...from adrenal surgery from Cushings...this was very informative. Do you have a video just on Adrenal Insufficency from Cushings related patients? Also (possible questions for a writers) I thought it was only called Adrenal Crisis...as the common term in the medical world. If anyone has AI or addisons and have been in crisis they will know how scary it is because not many medical professionals know because of its rarity. So just wondering if that is used more and might get their attention better?
It can be called Addisonian Crisis, Adrenal Crisis or Acute Adrenal Insufficiency. If you use any of the terms you'll generally be understood.
Very impressed. Where were you when I was studying in school? This was very thorough and all in one source.

Awesome video. Knew someone who just before her wedding (stress) went into adrenal crisis… very scary
You are a life savers .. thank you
Happy to help 😊
Amazing video!!!! I love it and you are extremely helpful. However, Fatty acids can't be used to make glucose. It can only undergo beta oxidation to produce acetyl Coa only glycerol can be fed into forming dihydroxy acetone phosphate which is then converted to glyceraldehyde-3-phosphate which then can be used to make glucose. Therefore fatty acids can't be used in gluconeogenesis.
odd-chain fatty acids can be oxidized to yield propionyl-CoA, a precursor for succinyl-CoA, which can be converted to pyruvate and enter into gluconeogenesis.
Such an amazing summa summarum of all the tens of sources I collected info. Thank you so much! 😊

Hi there, medical student here. You covered this topic brilliantly. Bravo. Keep up the awesome work!
The liver uses amino acids, lactate and glycerol derived from triglycerides as raw materials for gluconeogenesis, it uses the energy from beta oxidation of fatty acids but cannot use these directly as a carbon source
