The facial artery in the middle part of the nasolabial fold lays in the middle of all 3 main facial layers, so going superficially while aspirating would be safe. Cannulas can cause vascular occlusion if injected at a certain force and speed, go extremely slow with larger cannulas
That's so good to know. I wanted to smoothen my nasolabial folds and they told me that injecting in the area of the fold is and ''old way'' and what they do is injecting near the cheek bone to ''lift'' the skin causing the fold. I gotta say, the result is not that amazing and I think they used too much filler on each cheek bone (1ml each). I'd love to hear your opinion on treating nasolabial folds with injecting near the fold as in this video vs near the cheek bone. Doing an educated choice is important for patients and I'd love to know how to go about this next time I want filler.
Great information, thank you doctor. i have a question regarding the depth of injection. in the nasolabial fold you said its supposed to be deep, but in the fanning technique with the canula, you say its superficial, isn't it dangerous to go superficial with the canula?
The cannula won’t penetrate the arteries whereas the needle would damage the arterie and maybe even occlude it . The deep injection in the nasolabial fold is done on the bone to be safe and one should *ALWAYS* aspirate !

Do you make osseous contact when performing the first technique? You said the nerve is superficial so I would assume that making osseous contact would be a good indicator you are out fo range from the facial artery? Also, how much filler would you need?
It is interesting how you said at @1:06 that if the medial and lateral cheeks are flat, one can get nasolabial folds. How is that so? I have got cheeks but I still have nasolabial folds. I don't see the possibility of filling my cheeks cos it will not lift them up effectively to minimise the folds.

Great video. Doctor I had my nasolabial folds done a few days ago. The injector used a needle and went shallow. It was very painful . And the. I drove home I notice a white patch above the injection point and my face had turned number. I went home had a head ache fell asleep woke up at 1am with one blood shot RED eye! I rang her immediately at 1am! She told me to come and get it dissolved right away. Nowt face hurts , the hire patch is still there and I have a very hard painful lump at the injection site .

Mine improved without fillers.
If you are going horizontally with a cannula it is safe to go superficially because the nerve lies on the hypodermis (middle of the main 3 layers of the face) or it would be below the smas.

I suggest for accordion lines to use a syringe, go into the dermis with a thick filler to fill those individually (fanning technique +aspiration). Although cannulas are safe, they are better for deeper lines.

That depends on skin type. For thin-medium skinned persons, use a medium thickness HA FILLER. For medium-thick skinned persons, use an HA that’s in between medium and very thick. Make sure to massage all lumps afterwards!

Dan is not a doctor everyone he is a nurse practitioner and I would go to any qualified Nurse or nurse practitioner for fillers rather a doctor. the most experienced, gentle injectors are nurses, so much applause to Dan. Think of it this way ....most nurse injectors do it because it’s their passion doctors do it as a sideline to their actual medical practice. for most nurses that is all they do....inject, and some of the most gifted injectors are nurses.

It’s tricky. A little filler to the cheeks, piriform fossa & around the nasolabial folds is the best and most natural approach, in my opinion.

Folds are actually complex to treat well. I actually do both. I inject deep into the piriform fossa, then fan superficially with a cannula.
