Scenario 2 with Chronic cystic acne, pt referred by her general Derm should be coded using consultation codes as she is already established and referred by general derm to Derm oncologist? Didn't get this one
For the case around 1:29:00, weren’t the labs and echo credited at the time of being ordered and should not have been included in MDM for that documentation?
1:01:09 "if these 1st 4 items are answered then you can find that code" was an aha moment for me. Also when you explained that you don't use time and mdm for the same report
Sure, the modifier definitions found in the CPT appendix A : Modifier 59 should be used when coding for a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion (noncontiguous lesions in different anatomic regions of the same organ), or separate injury.

My question is concerning the 13-year-old with the left tibia fracture. The documentation states that the radiographs were ordered, performed and billed in the office already. Would this be considered when determining level of decision making since this has been separately reported elsewhere? Thanks in advance.
@@mohammadhameed6459 a medically appropriate History and OR exam must also be documented. Make sure you read the guidelines for yourself, that way you will know it for yourself.

@@tanushreeagrahari3224 These should be very simple because all you have to do is locate the code in the correct section: eg, New Office, Established Office, Consultation, etc, then find the code with the language that says Low Medical Decision-making.

Time and MDM? For the exam, if time is documented use time. In the real world you would have to follow your employer's requirements.
So in the first example you get a different level of E/m code when you do MDM vs time……I’m confused
