for biopsy codes example in answers options have two primary code acc to guide lines only one primary codes is reported additional type of biopsy is coded with add on codes so answer will be 11106 ,11103*2 --no modifier 59 required for add on code

    • @AMCIMedicalCoding

Hi Praveen: Good Forensic effort. The answer is Correct 11106 ( Incisional biopsy), 11102 (Tangential biopsy first lesion), 11103 (Tangential biopsy additional lesion), it is appropriate to add modifier 59 for separate lesions(see CPT appendix A for a full rationale). Remember: Add on codes are exempt from modifier 51 not 59.
