    Have you ever wondered what it is that makes prism glasses so difficult to work with? Or maybe why your eyes just feel so strained all the time if you aren't wearing them? Your brain is doing a lot of heavy lifting if you don't have them and need them, or if you have them and they aren't working properly.
    A lot of factors go into properly fitting and prescribing prism, but there's also a cosmetic component to keep in mind. It get's math heavy really quickly, so let's just have some fun with the basics. Today you get to learn a little more about that big dirty P word in optical, and why some places won't even make your glasses if you have it! You're an outlier, but we like outliers around here ;)

    Iv had a on going fight with my family over the prisms in my eyeglasses . My sister thinks she’s a doctor an tells me there is no such thing as prisms in both of my eyeglass lenses . This has gone on for years . As a child I needed glasses but my mother was against anything like glasses for a child . When I turned 18 I had a job driving a tractor with a shredder on the back to cut the weeds along side of the lease roads . I went to work an promptly drove the tractor in a hole an broke a Tie rod on the front of the tractor. Thanking I would be fired the first day of my new job I’ve told my boss that I didn’t see the hole . He told me to go an get my eyes examined an if I needed glasses he would pay for them . I needed them an he did . So a few years later I was getting new glasses an I told my sister that I had been getting prisms in both lens to correct double vision something I’d been getting since my first pair . Since then she tells me there is no such thing as eyeglass prisms . The crow she ate after watching your video was worth the wait of many years with a big laugh . I told her Medical school was still waiting for her . Lol thanks for the Insightful video.

    My prism lens was a lifesaver for both tennis and pickleball. It was so much easier seeing one ball than 1.5.

    This is very good! As a kid and as an adult, I was always hitting behind the ball whenever I swung a bat. At 27, an optometrist discovered that I had exophoria. I'd been wearing glasses for myopia and astigmatism since I was 11, and never had been tested for binocular problems. I had prisms for years, and it seemed to help. I'm 70 now and had cataract surgery last year. No more myopia or astigmatism. BUT I wear +2.75 readers, and constantly hit the wrong letters while texting. Never a break! Actually it's not that big a deal.

    I am being referred for ADHD, and after some research I found that I should have a tracking test as the symptoms mimic those of ADHD, just to rule that out. And low and behold, I have a tracking disorder and have been prescribed prism glasses today! I was so overwhelmed I nearly cried when I could suddenly see correctly earlier with the optometrist glasses on! I didn’t even realise I had a problem until then … 34 years down the line! 3 weeks to get my new glasses and I cannot wait!

      It’s so interesting that you say that because I have been feeling like I have been having ADHD symptoms that just keep getting worse over time, along with a lot of anxiety. I found an optometrist that was highly recommended and he prescribed a yoked prism for me. It’s crazy to find out it has been helping people with ADHD symptoms. I really hope it helps me! I feel so lucky to have been guided to go to him. I’m 27 so it has been many years for me too. I wasn’t involved in any auto accidents but I did fall and hit my head while I’ve skating and I wonder if my vision was affected because of that incident.

      You should really do therapy for such issues, not prisms… therapy can actually change your brain and eye nerves/muscles long term, glasses are for symptoms only and some get used to the prism and have to keep going up. Its more ideal for vertical misalignments that cant be changed, tracking and convergence is a therapy diagnosis.

      Hey, that's great. I didn't discover that I had exophoria until I was 27. As a little kid in school I could never color or write between the lines. That was in the 1950s, and they didn't know about ADHD then. We were just lazy or stupid.

    I saw a few videos on RUclips that claim prism glasses are just crutches for the eyes. That’s concerned me, and they even went as far to claim that it would make BVD worse and eventually end up needing surgery to correct the prism. This comes from people who own vision therapy clinics, and I don’t know if I trust their advice. I got prescribed neurolenses, and I am excited about them. I’m waiting on them to arrive.

      Little tougher to say, as it's very situation dependent. Vision therapy can do a LOT, but there are many issues that go deeper, and many VT ODs will prescribe prism as well. Actually a good friend of mine locally built a vision therapy practice. But in terms of relying on prism increasing the issues... I wouldn't go that far, but oculomusclature isn't my specialty 🙃. I've seen clients for a decade that have maintained the same prism power for that entire decade FWIW.

      Well, prisms help some and do make others worse. Neruolens is known to be a very inaccurate way to prescribe prisms, just fyi…

      @@HerculesMines almost every neuro optometrist who specializes in binocular dysfunction says it is inaccurate and the machine isnt so much the problem as the optometrists who have the machines, they have such little of any training in neuro optometry, they are really not qualified to check..

      Mil had cataract surgery, vision is 20/20 but she needs readers. She has had only vision in one eye from age of 13. But she is having depth perspection now,dizziness, blurry. Will prism lens help her?

    A week ago I had a 3mm stroke in the pons area, giving me immediate double vision. I've got prism lenses (the kind that go over my existing glasses) on the way. I'm hoping these alleviate my vision problems. Thanks for the video.

      Press on fresnel prism :) it's not quite as crisp as the real deal, but definitely the best way to test! 😁

    Went to the optometrist to get my eyes examined today for the first time in 5 years. Im a 25yr old dude. I have no complaints about vision discomfort or double vision anything like that. At the exam we found out that my OS sphere went from -1.5 to +0.5 and my OD sphere stayed from -1.5 to -1.5. The cylinder numbers also changed from -0.75 to -1 in my OD and -0.25 to -2 in my OS. I was prescribed 1 BD of prism in my OD after we tested the lenses that I said were the most clear at the end together but the text wasn't joining up in both eyes. We did a maddox rod to determine how off it was and when the prism lens was added the text joined up in my eyes. It'll be interesting to see how this new prescription feels, I guess my brain had adjusted to the glasses I currently have and being without glasses to the point where I perceive no double vision or any symptoms associated with it. Do you think the prism is needed because one of my eyes has become farsighted? Also, they said my eyes are healthy. I don't know why the prescription changed so much, I haven't had any trauma or injuries. My prescription had stayed quite steady before this test.

      The prism is entirely seperate, and good on them for catching it! Most totally overlook the value of comfortable binocular vision. It will take some time initially of course (it all comes down to our brains at the end afterall)

    With a 4.0 prism, various fitters such as America's Best were reluntant to fill the frame that has a wrap. I was turned down by Kaiser also, but last year when I presented an Oakley Crosslink frame with a moderate wrap to them, they filled it which became my most accurately filled prescription, giving me distance, medium, and close up reading accurately, while at the same time, adding the prism which without, required I drive with one eye closed - not good. Now I purchased a Crosslink Sweep, which has even a greater wrap. But I pretty sure Kaiser can fill that one too. Thanks for the video and the musing on prism, it is appreciated.

      4.0 Diopters prism isn’t too too bad depending on the rest of the rx, but I could see where it would be tough in certain wrap frames :). Good luck with the higher wrap frame those go round!!! Glad you enjoyed the video 😎

      ​@@ThatGlassesGuy03 I was recently prescribed prism lenses ( 4 diopters) and I also have myopia (3.5 diopters in each eye).
      Optometrist used an index of 1.67 and anti glare coat but now I'm experiencing 'Chromatic aberration', seeing yellow and blue glow everywhere.
      My binocular diplopia is caused my ocular myasthenia (so..weak ocular muscles).
      I've used prism prescription before. (2.5 for myopia and 2 of prism) and it double in one year😢.
      I returned the lenses and they weren't able to explain why is it happening. The anti glare coat doesn't produce that chromatic aberration in lower prescription lenses.
      I'm despered. They say lower indexes/material will make the lenses too thick. It's already 7mm

      Your America's Best makes prism prescriptions? I've always been told they don't even have the ability to make those.

      @@PsychoMantisss They could do prisms, but what they are hesitant on doing is filling a prism prescription in a "wrap" type frame. I am currently using The Lens Factory mail order because they are less than half the cost of fitting a lens to my customer provided frames when compared to Kaiser, my medical provider. And they will try and fill any frame, short of an extreme wrap. I like to buy vintage Oakley's and vintage Bausch and Lomb's that have a semi wrap. The latest that the Lens Factory filled successfully were Oakley Fate frames. Although advertised as women's frames, they look okay on men.

    @That Glasses Guy Hi, I am just wondering what you mean that convergence insufficiency is a "whole different ball game?" I was just prescribed prism glasses (not neurolens, for reading only) for convergence insufficiency and I am pretty sure this helps but probably not enough. I especially feel symptoms returning when tired and sleepy and trying to read. I am wondering if the prescription may be inaccurate and I may benefit from having the prescription checked or having the prism strength increased? Or is it possible that my brain is still adapting and I should just wait a few weeks?

      Hey Anton, consult an optometrist that specializes in vision therapy and ocullar muscles for this area (An ophthalmologist may check this as well, but they like surgery a bit much for me here). My knowledge of this subject is just enough to know when it’s an issue and how to make lenses for it :).
      The key take away here, though, is that when the problem is worse when tired, you can’t correct for that without a separate pair again, as if you add the prism for when the muscles are weakest, you create doubling when they’re strongest. It’s a battle of what’s comfortable and workable in many of these cases until the underlying problems are handled.

    I live in Japan so please excuse my bad English.
    I’ve been having headaches and eye strains lately, and my eye doctor says I have exophoria (Or lazy eye? My non dominant eye would shift outward when tired). The doctor says I can consider having a prism glass made.
    I’d like to ask that, If do make a prism glass to help me with working on a computer or reading, is it discouraged to wear that prism glass all the time? Should I only wear it when needed? And if I do wear the prism glass all the time, will the exophoria become worse over time?
    Thank you for your time in advance. Just like to have some general guidance or direction.

      Your English is just fine! As for your questions let's get into it! So as you know with exploring the eyes do work together well most of the time. Prism glasses will always be compensating for the shift (I actually have a 25D exophoria myself!) And as such you would cause the same problem youre trying to avoid by wearing the prism glasses when not needed. Most often in these cases the regular glasses with a good optical center is enough to help the eyes stay focused as the brain works a little harder to ensure you're looking through the sharpest spot, of course that doesn't work if you have no or a very small prescription aside from the prism.
      Yes, it will get worse over time. Eventually when it becomes a real problem and more common than not, there is a surgical procedure to help with this called strabismus surgery. I wouldn't worry about that just yet 😁
      I hope this offers the advice you're looking for!

    While prism glasses haven't helped me gain any depth perception (to be fair the PD that Zenni's app gave me was off by 3 so it was blurry and I sent them back after two days after getting a proper reading), they did instantly stop the migraines. A new pair is being sent out. I only used Zenni as a temporary option as where I live only one or two places has the ability to even make them, and one is a one hour eyeglass place that I'm not overly fond of due to their staff being incredibly rude and the glasses being overpriced/subpar. But they did have an amazing warranty at least. The other place does neurolens. I didn't want to pay for an expensive pair of glasses that may or may not entirely fix the problem so I was advised to buy online to try them out and if they work then to buy a better pair with a finalized prescription, if not then we would try a third surgery, which I don't want to do as the last surgery barely lasted 2 years and we have to travel to the other side of the state to have it done/even see the doctor in the first place.

    Thank you for this, I haven't watched it yet, but I commented on your one of videos around 3-4 months ago saying I just wanted to throw my astigmatism lenses off my face the second I put them on, well I've had my glasses remade twice now(made 3 times total) and it still wasn't right, HOWEVER, the second time they made them, they accidentally put a prism in one of the lens which caused a lot of headaches and eye aches behind my eye BUT my eyes looked more symmetrical than I have EVER seen them. Usually my eyes look like I have sort of a lazy eye or one eye is bigger than the other, just not symmetrical, with the prism lenses my eyes looked GORGEOUS. I'm starting to wonder if the problem isn't astigmatism, but BVD instead(like I've been mentioning to the doctor since my very first visit SMH). Does the way I explain my eyes looking normally, and my reaction to the prism lens make you think I might be onto something with maybe need BVD correction rather than astigmatism? The 3rd pair of astigmatism lenses still don't feel right.

      Yup, the prism can be used for both anatomical or visual concerns 😎 wicked fun stuff to play with but general we save the anatomical assymetry prism for cosmetic uses only (e.g. aligning a prosthetic eye a little better). But in general yes most people have slight BVD, where it becomes an issue is when the symptoms are a problem because MOST cases just regular glasses resolve the issues, and in the ones it doesn’t very slight prism is all that’s needed, BUT it has to be in the correct direction 😎. IMO based on your struggles here, I would fine an OD that regularly works with ocular muscles and BVD or find an ophthalmologist/oculoplastics specialist that can properly measure for an vergence or prismatic errors 😎
      This from a guy who has a very close friend that had a 2D eso missed for 31 years and had no depth perception her entire life. By the time we met and I figured it out through my own learning process of becoming an optician and expanding my skills personally (beyond normal optician training) the prism was just something she didn’t like. I.e. she now likes her world flat and depth is not a parameter her brain likes 😂😂😂

      I highly suspect that I would be the same as your friend in that I probably wouldn't prefer the prism lenses either just because my brain is so used to the way i perceive things, truth is I enjoy letting my eyes go double vision and even closing my eyes for a few when I'm tired, and I feel like a restraint on that might be bothersome

    My son is 19 years old, none of his eye doctors have ever addressed has left eye turning in, sometimes it's turned in as far as anyone could turn their eye in, while the right eye appears normally. This eye turning in thing happened after a cataract surgery he had about 8 years ago. He now has what he calls double vision and doesn't even use the turned in left eye... His prescription is -14 and -15, for the first time an optometrist suggested a prism... My son absolutely loved the prism at the doctor office, it makes the double vision much better he said. Could you make a video about the types of frames that are best looking, when having such a high prescription and prism? My son wants round glasses, but we've recently got a pair without prism and they are just so sick and Mr Magoo looking.. I wonder if you can help my son figure out the types of frames that would look best with this kind of strong prescription.

    Hey. Love this video. I've recently been diagnosed with horizontal diplopia in my right eye and the prism correction is very high, 12 diopters in my right eye alone. I'm awaiting a consultation with a specialist to understand what is causing my problem, but I'm concerned with how thick my prism glasses will be. Are there alternative lens materials etc that would reduce the thickness?

      For that kind of power, no. The lenses will be fairly thick even in a very well selected frame. Don't chase the thinnest material because the optics will be trash if you do. Thick lenses don't mean glasses can't still be stylish though 😉😎

      solid idea, I'll make note of this, I've been running out of technical video ideas lately. LOL. A quick summation press on fresnel prisms trade off a lot in the way of optics and should really only be used temporarily as a test of concept or before/after surgeries when needed on a short term basis. The ability to clean alone is severely compromised and not good, but the optics are also much more blurry and compromised.

      I used the press on prisms to find the correct size for about a month. There is a lot of reflection, they get dirty so fast, and vision is a bit blurry with them. Having prisms in the lenses is just like regular lenses. I expected it to cost a lot more than they did. Had cataract surgery and having to use the clings is driving me bonkers. A few more weeks to go.

    I needed them 3 years ago but the optician didnt believe what I was telling him based on his "experience" & they messed up my prescription so I lived with it for 3 more years but now its so bad Ive had to sort it out but my appointment isnt for another month yet & I had my first check in december,the headaches & mental exhaustion from constantly having to focus has wrecked me I dont have the mental energy for anything.
    UK sucks

    My eyes are uneven, so prisms make a huge difference for me since my eye muscles strain so much trying to focus together. The prisms relieve that.

    Thank you for this video! I just got my prism glasses for the first time to reduce double vision due to a significant difference in prescription between my eyes. I was told it'll take about a week to adjust to them, but I'm noticing significant blurring in the middle of my lens (in one eye). I end up moving my head up and down trying to read text in front of me. It's only been a day, so I don't know if I should be patient about it. I'm just not sure if it is normal to struggle so much when you start. My previous prescription was very similar except for the prism added.

      This sounds on the face more like what’s known as slab off for a progressive lens. In which case, yes there will be a distinct blur zone across the lens. It takes some time to get use to and can be rather annoying but overall vision and especially near zone comfort should be dramatically improved! 🥂

      @ThatGlassesGuy03 thank you so much. This helped me explain to the doc what I was experiencing. They helped me adjust the frame and nose pads, so that the blurry line sits above my eyes, instead of directly in my line of sight. I don't notice it as much anymore and no headaches now :)

    Question! I recently got prism glasses because I was feeling unstable and had trouble walking. Went to update my prescription and found out my eyes are slightly misaligned. Now I have .5 prism for my left lense. My brother basically wrote me an essay saying it could misalign my eyes more and not to use them. It has made my walking better though. Is he right about prism glasses possibly making my eye become even more misaligned?

      Short version, no. Today we know there are far more people walking around with this problem and it’s becoming pretty common place to see between a half to one full diopter of prism to correct some smaller muscle imbalances. I’ve seen people wear low and high prism for decades with little to no change to the prism required. Just like very few have symmetrically positioned eyes, not all are perfectly aligned for best acuity and comfort of vision either.

    Wow I didn't know it went up so high! I have prisms due to nerve damage that gave me double vision and I started with one prism of 4 out in my right lense, then I was switched to both lenses at 2 out, is this common with double vision? I had a optometrist tell me it was better to have some in both lenses

      Absolutely. Unless it’s a specific visual field issue that must be corrected in one eye, it’s MUCH better to split the prisms for a better cosmetic affect and better frame balance. Great question!!!

    I 've just been reffered to an opthalmologist from the neurologist because he did not find any other problem than something with an isolated 4 nerve palsy that is responsible for my double vision, he told me that is likely that I would get an prism glasses treatment do you know anything about this? Do I need to wear them together , are they dangerous for my eyes? I saw many people complaining about the sides effects, the refferal I got in Leeds but I believe is a better chance for me to get a better opthalmologist in London. If you kind to give me your opinion, would be amazing.. thank you!

      There is no side effect of prism, and they are far from dangerous. Certainly less dangerous than driving without proper binocular vision ;) A second ophthalmologist’s opinion never hurts, but most people actually can benefit from smaller amounts of prism for best binocular vision. This is really only being understood and implemented very recently in terms of the optical industry.

    Car accident concussion after 4 years of closing one eye found out double vision did surgery to move eye muscles and place muscles in new place. Then physical eye therapy, after all this have migraines everyday, now dotox ....question would prism glass help with the migraines and do you wear them all the time? I do have computer glasses that i wear when working, what about driving i can't see with low beams have to use high beams to see.

      I would speak more with your ophthalmologist/optometrist as there's really a lot going on here. In re: to prism glasses for your migraines I can give a pretty firm no to that helping based on your information given. For the lights, again I would defer to someone looking at your eyes partially because of the other issues. 🙂

    I have had Type 1 Duane Syndrome since birth, and I’ve been recommended to get prism glasses. I’m not entirely sure if I should actually consider getting a pair. 😬

    I was wondering if you / your doctors are as reluctant to perform or reccomend surgeries for binocular dysfunction issues as I am getting from the eye medicine community at large. My soon-to-be first vision therapist has a glaring lazy eye from a botched surgery, even. For people who have double vision extreme enough (or fluctuant enough) that prism lenses are too hard to manufacture or unreliable, what other options are there besides an ill-advised surgical route?

      It’s all pretty murky waters. We don’t do any of the medical side of things in my shop and just work magic on the back end with the RX’s that come through, so I get to deal with these issues in a lot of ways doctors often don’t think about. Little bit different perspective :)

    Can lenses have prisms for distance but not the add-on? I do need them for reading and computer. Before having trouble w/double vision, I prefered bifocals based on my high prescription. Also have an astigmatism. Had cataract surgery so have a lot of changes. Refraction is Tuesday. I am patiently waiting for new glasses so I can ditch the cling-on prisms.

      Generally no. There are ways to sometimes create that as a net effect, but it’s not something that can naturally be produced if that makes sense. When specifically placed prism is needed the stick one are the only current route.

      Time for more power! Moving a plus lens away from the eyes increases plus power. You're making it closer to a +2 as well as increasing the effective magnification.

    I got prescribed 1.5 diopters for horizontal heterophoria I think it's called in english. The ophthalmologist told me that we're not going to go over 2.5 diopters for each eye. I've read a lot of bad things about prisms so I'm very scared, but also excited because when the lady put low prism infront of my eyes my vision got sharper and I forgot I had double vision for a while.

    Hi, I was recently prescribed prisms for vertical heterophoria. I have also been doing Vision Therapy for almost two years and physically my eyes are moving so much better, but I still have reading comprehension problems. This is stumping my docs, so the thought prism glasses for only when I am reading or on the computer might help (which is basically all day). I still seem to be having reading comprehension issues along with a pressure/fatiguing feeling on the inside corner of my eyes. Should you be able to feel the glasses like this , or should they just feel like a normal pair of glasses?
    I also am dealing with moving words, like if I stop and just stare at texts, they seem to move back and forth a bit. My docs don't seem to know what to do about that either.

    thanks for this video! I am 50 and have suffered from diplopia for the past several years. I have been to my optometrist over and over trying to get the prism power correct in my glasses. I have 2 pairs- 1 for over contacts (progressives) and a pair for without contacts, for which the prescription is very strong (especially my right eye. Is it normal to keep going back to get the prism amount correct? It really is exhausting. Also, my diplopia is especially bad while driving. Love your tinted lenses! I notice bright light affects my diplopia more. Does the tint help with yours? May try them on my next time getting prism correct! Thanks again!

      Myself, I don't need prism lenses just yet 🙂. I do like various light tints for a handful of uses but always default back to a softer green/yellow if its for all day long. Where it can help is in poor materials that are more prone to aberrations, which prism makes more obvious.

      Now for the prism increasing, this really depends on the cause, and any corrective surgery really requires the problem to be stable. I would recommend looking into vision therapy with a local optometrist that specializes in it to see if there is a chance of improving or at the least reducing the problem of it constantly increasing 🙂. It's definitely not unusual to see prism that keeps changing though, especially in the case of a Phoria that has largely been fused and is just starting to become a problem. I'll face that one in the future myself 😅

      @@ThatGlassesGuy03 Thanks for the reply. I just returned from my optometrist. I explained to him Im still having diplopia with the prism he prescribed. He basically used the strongest test prism to where I was having results, then attached a stick-on prism to my right lens. The results driving home were remarkable. Im being sent to specialist. I get the impression the amount of prism Id need in my lenses is not feasible. Im hoping the specialist will recommend surgery to correct my problem.

      Here we ar 2 years later and after going to a specialist and having the max amount of prism in my right lens for a while, yesterday I had strabismus surgery on both eyes, I'm recuperating well and looking forward to seeing how the surgery went after my eyes settle down a bit. I was given a fresnel lens to negate the prism in my right lens until I figure out the post-surgery condition/prescription of my eyes. .I'm not crazy about the lines on the sticky lens, but can't do much about it. My eyes are sore and runny, but doing well. Hopin one surgery will fix my problem! Take care!

    Well am screwed. I have thyroid eye disease and I just got double vision last week. I only wear glasses for reading but for everything else I don’t need glasses. So the eye doctor had me purchase glasses with clear non prescription lenses and he put a temporary prism sticker over the right lens.
    Well when I put them in today I was able to see to drive again and watch tv. For the last week I was wearing a pirate patch 👍.
    Is the prism supposed to be clear because mine is cloudy when I look through it? Also it sucks I have to carry 2 pair of glasses with me all day long now. One for all day to not see double vision and the other just to read.

      The stick on prisms are fuzzy, the proper made glasses are much better 🙂. Not much to say about the two pairs except for advice on cases made to carry two glasses easily. I usually have at least 3 pairs handy 😅

    hi there hope you can read my comment and give an honest opinion
    ii dont have eye grades or astigmatism however my doctor found out i jave 4th cranial nerve palsy, at first sight my eyes look fine but once i look down my right eye will not go down as the left eye.
    I have a prism with just. 5 power. I am hesistant to wear it as i might get use to it and in the long run i will be needing more n more prism.
    Wearing it makes me more confidence as my coordination and balance improve a lot.
    Just that i was concern in the long run it will hurt me tha do good.

    I am a nurse w 5 plus hyperopia with an add. Currently wearing shamir progressive. However, I have been reading about aspheric lenses and double aspheric lenses. Can u elaborate the differences in eye magnification between either lens?

      Aspheric on its own has no affect on cosmetic magnification. The distance of the lens away from the eye, curvature, and thickness at the center will determine that. Reducing lens size & decentration (thus center thickness) and selecting a flatter curve (suboptimal for optics) in a frame that sits close to the eyes will have the largest impact on perceived eye size :)... good topic for another video! 😁😁

    I started developing a minor exotropia after getting LASIK done. I’ve heard from a few doctors that getting prism glasses really is just like a bandaid. If you just adjust the lens to fit your weak eye muscles then it’ll just get worse. I’m currently doing eye exercises, I’m really trying to avoid getting the prism glasses since my issue is intermittent and not permanent so far.

      In early and light cases that can absolutely be the case. That’s where vision therapy is key 😎. Intermittent especially you don’t want to try to correct in glasses :)

      Fortunately mine has stayed pretty consistent and I can still hold solid fusion in most cases (exophoria). Only really a problem for me when extremely tired. 🎉

    I have accomodative insufficiency and alternating exotropia. I think I have about 1 diopter of prisms in my glasses. The prisms cause reflections of lights in the lenses and it was really noticeable the first few months especially at night but I've just gotten used to those being there.

      1 diopter of prism shouldn’t be enough to create serious internal reflections alone. Unless you were already very high minus. More likely something else is causing it such as a blue blocking ar, high polished edges, etc.

      It's not uncommon at all for new prism to feel strong or cause strain initially. The big thing to be concerned about is over all vision and making sure you have a single image vs 2 images. This discomfort typically will improve pretty rapidly, but the big thing is, don't switch back to other glasses and wear these continuously :)

    hello, i have a huge problem , but i will make it short. Can prims glasses cause damage at a younger age? i went when i was 12 years old to a eye ''docter'' he said i need prism, and every 3 months or so, i needed my eyes adjusted, like i got a lot of prism in a small time like 2 to 3 years, then the hospital said i needed to quit the glasses. so my question is, can it cause long term damage?
    i hope you repley....

      Very unlikely. :) The short version is it only shifts the image and can cause some temporary weirdness in getting use to other glasses or no glasses, but nothing long term to speak of

      While technically this tint would filter upwards of 90% of blue light.... purely a fashion tint. My blue light lenses are so clear you would never know it... thats what I wear in most of my other videos 😎

    Thank you for this video! I’m on my second day of wearing prisms for convergence insufficiency (lowest strength), but I’m still seeing double when I relax my eyes. The doctor says to give it a few days and my brain will “adapt.” I’m trying to keep the faith, but can’t see how there will be a magic moment when these images fuse :( Does it takes time or should they pretty much work right away? Thank you!

      With convergence insuffiency it's a whole other ball game. Neurolens? 😶. At any rate, yes it takes time. I actually prefer a bifocal with a customized inset to produce the required prism for adaptation before moving into progressives.... but most people don't like hearing multiple stages and lens costs 😝. In the longer term you will find a dedicated set for intermediate and near with the prism will be much more comfortable than trying to do it all in one full progressive. As with all they're great for most things, but in your cade specifically that second pair is crucial, as the prism can be controlled more readily for those two zones than the contoured prism nuerolens offers.

I’m just confused- my optician says the glasses should resolve the double vision right away, the tech says give them time, and the Internet says everything in between :)Am I just banging my head against the wall trying to make these work? Or should I just stay in the struggle and hope for the magic “click”? So confused!

      You're welcome! So with single vision reading only, more often than not it works or it doesn't. Since it's a muscular issue and not a brain issue, the fusion is either there, or it isn't. With progressives and lenses like neurolens it's a different ballgame as there is a lot of brain learning going on in addition to the muscular limitations and the new image fusion. Sometimes... where the brain was fusing the weak convergence, adding the prism to the lens for comfort of wear can, for a short period, induce doubling as the brain has been fusing the image that was double all along, and it needs to learn. This leads to a lot of ambiguity online because each case is different :)

I was just prescribed prism glasses (not neurolens, for reading only) for convergence insufficiency and I am pretty sure this helps but probably not enough. I especially feel symptoms returning when tired and sleepy and trying to read. I am wondering if the prescription may be inaccurate and I may benefit from having the prescription checked or having the prism strength increased? Or is it possible that my brain is still adapting and I should just wait a few weeks?

    I developed vertical Strabismus/4th nerve palsy as a result of a Traumatic brain injury. How would a mirrored len or polarized effect the prism correction. I have a 1.5 prism diopters.

      In this case they won’t have an effect “except” increased internal reflections to the lens design may be more pronounced. Keeping the frame smaller and lens carefully selected will help some. And of course this also depends on what the rest of the prescription looks like to some degree.

    I've had prisms in my rx for most of my life. My rx is RE-9.75 -3.50× 47 / 9 bo 4bu LE-10.25
    -4.25 x 135 / 9 bo 4bd. Lenses always very thick on outside but high index helps to reduce thickness. Does that mean i have 18 prisms. Also my lenses seem to need increasing very frequently and the cost is enormous.

      Wow! That's definitely up there in prism power I've worked with! Actually you have close to 24 diopters of prism power combined once all the math is done 🤯. On top of a very strong myopic prescription as well. The regularly increasing is more concerning but im sure your doctor would alert you if there was something medically to be concerned about :) high index does help some, but a smaller lens with good centration is the best course of action, and one of my favorites for one this extreme is a little guy called the Kala LilO. 33mm perfect circles set at roughly a 63 frame PD from memory.. pretty close to most average faces for great centration on a wide range :)

      My vision was getting worse often and getting expensive found a tumor on my optic nerve had radiation to stop the growth and strambismus surgery to fix my eyes . try getting an MRI to find the cause

    I have a question about my prescription. I have a TBI and was subscribed a 3.5 right ul and 3.0 right up prism with both lenses +2.25 in both lenses. I was told it is unusual to have both prisms up right. Usually one is up and one is down? I feel worse wearing tge glasses.. how long does itvtaje to get used to them?

      Typically that's correct. This is an application of what's known as yoked prism and is used to move the image as a whole. Ideally to a part of the retina not affected by the stroke to give you a better image. That kind of lens does take some time to get use to, as quite literally your whole world is shifted. I would say a solid two to three weeks to feel truly comfortable if wearing full time i.e. wake up, put on specs, take off only to go to sleep. Don't let an optician that doesn't understand the application trick you, it is unusual yes, but not everyone out there is walking around post stroke with affected visual fields. 🙂 trust your prescribing doctor on this one, it's life changing when used properly.

      First off, thanks for replying... I was prescribed these by a neuro ophthalmologist. His notes say it could be from a CVA TBI etc. I have severe 24/7 headaches, neck and face pain, nausea, weakness, dizzy, super light sensitivity and sound. Can't even look at some patterns like window blinds. Where I had the glasses made said they had not seen where both prisms were up and to the right. I actually feel worse when I have them on. Been wearing them a week.

      @@kevinsomers8923 definitely trust your specialist on this one. If it doesn't improve soon I would let them know you're still having trouble adjusting, they may tweak the prism some more. The idea is to focus the light most where the retina is still working well. Sorry to hear you're going through such a tough time all around :(

My eyes are very painful, red, dry feeling with severe headache still and dizzy. I spoke to the ophthalmologist office and told them I am worse. They just said wear them another month. I can't find anywhere that says it should take two months to get these to work and no where that says I should be getting worse. I had the glasses checked and the prescription is correct, so it is my thought the prescription is just wrong. Have you ever seen anyone take that long for it to work and did they get worse while getting there? Thanks

    Can someone go from prism to non prism lenses? I just switched from regular lenses to prisms and I can't find any information if you can go from prism back to non prism lenses. Has anyone gone from prisms back? My optometrist said something to the effect that it's prism going forward. Anyone know?

      So, you absolutely can in terms of being able to wear it yes. In terms of just not needing it any longer? No. Without extensive and arduous vision therapy or surgery to fix the muscles around the eye it will be beneficial. Now whether those benefits are worth it or noticeable to you is the real question. Generally binocular vision with best comfort and depth perception will need some prism. (Almost everyone can use small amounts of prism, but that’s time consuming and hard and you’re just not going to get that at every exam). I actually adjust my lenses to add a quarter diopter for enhanced depth perception in all my driving pairs 😏.

    Hi!As far as i know 1 diopter of prism(0.5 degree) shifts your world with 1 cm at a distance of 1 meter!How do you calculate the shift of the optical center?
    1 DP=1 mm?And if you wear lets say 4 prism( 2 each eye Bu Bd) is it possible to decrease the power in time or is it gonna only go up!Thanks!

      👋 correct on the dioptric shift and power of prism. There are a few different points we use to reference the correct power, and in higher prism we do adjust the major reference point of the lens to align more properly with the visual axis accounting for shift. As for the optical center itself I'll have to dig into my books to find that formula, it's not something we use much :)

      I wanted to address the prism over time in its own comment. So, typically left alone to its own systems that will stay the same or gradually increase. An optometrist that specializes in visual therapy can better explain how to keep it stable or possibly reduce it depending on the situation and if prism is used only to compensate for muscular issues. If the problem is within the visual system and the image needs to be shifted to be seen due to a visual field defect, thats a whole other matter :)

      @@ThatGlassesGuy03 i konw it's a kind of jedi thing in optic and a few people really understand the concept and how it realy works, i was curios , in the case of a muscular imbalance and a normal Central Nervous System , the prism becomes a crutch,and after some time you need more, or can actually improve the imbalance and wear less!

Yes in the case of muscular issues, like any other muscular issues, it can be trained for and reduced or operated on and corrected (strabismus surgery) as sometimes it's more of a muscle length than strength/control problem. We actually have an optometrist in my area that specializes in visual therapy and helping these people, but it's usually in combination with prism glasses as well. I wouldn't say prism becomes a crutch either, as most people I've worked with that don't do any sort of visual therapy tend to hang around the same dioptric power for many years :) certainly nothing like advancing myopia that can just as much as a diopter in a year! 🤯

      Yes and no. The concept and the change to how light passes through the same, but they're applied differently and in a specific space in the lenses instead of ground into the entire lens in that case :)

    I have a pretty low prescription but a 3.0 prism in each eye. I was getting chromatic aberration when I first starting wearing them, but my brain seems to have adjusted to not seeing it as much as before. Is that normal? I've worn glasses most my life, and this is the first time I've dealt with CA (yellow blue to be specific). Also, I still notice curvature towards the bottom part of my vision when wearing them, but it isn't bothersome enough to actually do anything about, as I can now walk fine (had some trouble at the beginning 😂) and I seem to mostly just forget. My depth perception has gotten better, my up close and distance vision is FAR better, and my double vision is gone.
    The only other thing that trips me out is red and blue colors on my phone and TV. They seem to come across in an almost 3D fashion. Oranges as well sometimes. Is that a normal thing I should just get used to? I have polycarbonate lenses with anti-reflective coating.

      You nailed it on your last sentence. Polycarbonate will have strong chromatic aberration at less gaze angle than any other lens. With 3 diopters prism… yup, you’re almost always technically looking through the lens off central gaze angle. CR39 or even most high index would be better. Avoid any of the blue blocking coatings as well. You want as clear of a lens as possible here, particularly because well, you know. Can’t I ring the bell. Once you’ve seen it. It’s always there. ☠️😂

      OMG I'm experiencing exactly the same.
      I have 3.5 diopters for myopia and 4 for prism.
      Lenses have a 1.67 index (to thin them) and a anti glare glow.
      I took them back and I'm not sure if it's the antiglare coat doing this. They say they can do it without the coat but will be a 1.5 index and thicker.
      I dont wannna walk with 2 bricks on my nose. Despair.

      @@redleeks6253 you don’t want the lenses without anti reflective coating I promise you. But 1.67 does have awful chromatic aberration (it’s almost the same as poly) cr39 (1.5 index) or 1.6 (hivex) are what’s needed to reduce the effect.

      Index was 1.5. (shamir lenses).
      I don't understand anything of this. I'm just reading the pockes where the lenses came because i ask them back.
      The shop was confused saying nobody ever complained about this chromatic aberration but they only rarely someone pops up with a prism prescription.
      1.6 will save it? And keep the anti-glare coating?
      Because i don't experience CA with my old lenses ( 1.5 / no coating)

      @@redleeks6253 if they offer it 1.6 would be great here. It’s the increased flatness and thickness of the new lenses working against you, as well as the image shift of the stronger prism. Without trying to get too technical, higher prism means chromatic aberration becomes more obvious sooner, and the thicker lenses have an increase in internal reflections.

      @@ThatGlassesGuy03 As an optician. We live in an area with a lot of people over 65 and we see a lot of prism. In general, I give them whatever they're used to and I strive to be super accurate with the measurements. Some of my co-workers feel more comfortable urging people to go to separate SV and NV glasses.

      The doctor will tell you. Temporarily is usually only for trials or while awaiting the final prism glasses due to the time they can take to produce when it’s a large amount.

      You’ll need a proper binocular exam with muscle measurements. It’s not something to toy around with.

    I really think my prism glasses were a bad idea and made my double vision MUCH worse. I shouldve just dealt with the slight double vision I had before, when my eyes were trying to focus at maybe TV length of 5' away. With the glasses, my eyes got used to looking at the image on the glasses lens only 1" away. Now things look double if they're only a foot away. The glasses were a terrible idea. The optometrist screwed me over with terrible advice. But it's too late now. The damage was already done. The glasses made my eyes cross in more to focus on the glasses, so I basically stare at my own nose now. I never shouldve gotten glasses. They made me worse. I only had a slight double vision before. Now it's really bad. The glasses made me focus on the glasses, so now I'm dependent on the glasses. I was better before the glasses. I shouldve just lived with my eyes how they were. I used to be able to see fine within a room, cuz my eyes were trying to focus at TV distance. Now I get double vision everywhere, even within a room, cuz my eyes are trying to focus at GLASSES distance. That was a terrible decision. I should've done my own research instead of trusting the optometrist. These prism glasses definitely made my eyes much worse. They really screwed up my life. I'm really upset that I didn't do my own research. 😢

      That's not how any of this works. At all. You need to address these concerns with your optometrist. Odds are the prism just needs adjusted slightly. Often times what's needed in glasses is different from what's tested in the room, throw in potential calculation errors for the lenses and measurment issues too and it gets hairy fast. The docs know what they're doing more often than not 😎

    I was in a bad motorcycle accident and broke my orbital bone and one eye sits higher and close up everything is fine, 30 or so feet I see double or things move when I know they're not, I was prescribed prism glasses. I've NEVER had glasses b4. Next week the glasses are supposed to be in.. how long will they take to work?

      It will be noticeable instantly in that case. Prism for muscle issues or eye location issues is a beautiful thing. Your brain will be happy to not be working as hard to fuse the image. The question will be if you induce double when reading or not, and how long it takes to be able to fuse that, if at all.

      @@ThatGlassesGuy03 I hope it works quickly, I've ALWAYS had 20/20 or better and now with everything in my right eye bouncing sucks.. I have a tbi so everything is slow going.. but if my vision comes back perfectly that's one thing I can cross off my list. 🤞

      This is definitely a long road to recovery, I'm sorry to hear about that all around 😔 especially a when it's a rider. I haven't ridden myself in a few years, still have all my gear... it never goes away 🤣. The "bouncing " will be the more interesting residual to see if it resolves or not. This could just be the brain trying to get the eyes to fuse at far, or something else. Either way, that might take longer to go away.

    I wouldn’t recommend prisms to anyone,
    Over the last 4 years I’ve gone from 1, to 3, to ,4.5 and now to 6 in each so 12 diopters. It will only keep getting worse as it weakens your eye muscles from working less and less.
    I am now looking at surgery as the measurements of my eye turning in are now bad enough for them to touch.
    I also have weird eye movements which I can feel every time I move my eyes which got worse from wearing prism glasses. My eyes do not work together at all.
    Please use prisms as a last resort and always try different solutions like eye exercises and vision therapy first.

      I’m all for visual therapy, but that’s not how this works. I’ve had customers hold the same level of prism for my entire career (spanning over a decade).

      I immediately noticed less headaches, neck and shoulder tension, and eye fatigue. I FINALLY had no issue switching between looking at my phone/book, husband and the TV. It was marvelous. And I can drive without closing one eye.

      It's unusual for such sudden onset, but yes! 5D of prism in each eye base out ( BO = thickest part outwards) on top of that near -6 power to begin with definitely builds some thickness at the outer edges!!

      Ask about a trial with fresnel prism :) it's applied to your current lenses so you can experience it in the real world at WAY less cost than custom ground lenses. The clarity isn't as good, but the comfort of vision if needed will be MUCH better.

    Prisms are not good because it progresses so fast. Someone started with 6 D it went up to 16d in 6 months. Over minus lenses thearphy actually work for small deviations.

    In my world prisom is a dirty word.
    Long story short, my rx is L -8.00 R -7.50
    Prism 4. AND I LOOK HIDEOUS. I spent almost $900 and although the vision is good and it has corrected my double vision,... I won't wear them because I look like a freak, with the distortion against my face. I am quite devastated. Why do I have to give up looking good to see good

      Ahh the peripheral prismatic shift! Smaller lenses more central to the eyes, closer to the face, and if all else fails digital lenticulation to the rescue to reduce the prism in the periphery 🙌.

      Of course. That’s a fairly common use, assuming it’s always there. If it’s a phoria that comes and goes, that’s another story. Essentially if the prism is there and the eye isn’t doing what makes you see double, you’ll then be seeing double from the prism. Vision therapy is the best solution on this case :)
