I heard a theory once that the cart seems like a portable version of the cage that he left behind. He is and always will be writing towards himself. He is including his fans at this point but this is his therapy. Every song is for his personal health. We are just a byproduct of similar situations.
I think the shopping cart is representative of the "cage" from before/ the baggage he carries now. He has a lot of trauma and things that he continues to carry with him and I think the balloons are tied to it as a representation of masking. He literally uses balloons to hide his face at one point in a mv (I forgot which one off the top of my head). But with the bars surrounding where he's talking about balloons it makes sense. Additionally they could be a metaphor for avoidance coping as in if they are not tied down they will float/fly away. I think at the end he continues pushing the shopping cart as a symbol of he's still carrying his baggage with him. In the end I think he sits in it for one of three reasons: 1) He feels he's still struggling to overcome his trauma/baggage and it's become a place to rest because it's what he knows. 2) Part of healing emotional wounds is being able to sit with them and explore them which is part of what he probably does in therapy. 3) It's a representation that he still feels trapped in the cage despite trying to move away from it. There’s a whole lot of Easter eggs in this, but the cage also represents him leaving the cage of his own mind that he feels that he’s been trapped in the balloons represent his burdens because they’re black. The shopping cart is white because he wants to fill it with new joys, he’s pushing it across the grass field because it’s hard to get there. It’s not a hard one. This is like one of the easiest things to wrap your mind around but it is also a great and brilliant song because it all fits.

The Cart is a place of safety. Hes used to sleeping in the Cage from Perception.
In addition to the symbols that others pointed out, I believe that the people dressed in white as well as the unique hoodie he's wearing (zipper on the back) are meant to suggest a mental health facility, seeing as he talks about having a breakdown and seeing a therapist. Given how those in white are organized in the next video as well, I suspect it's a commentary on the nature of that industry, at least in part.
