  • Educational video describing conditions and treatment of patellar dislocation.
The patella is a protective bone located in front of the knee joint. The patella is attached to the quadriceps muscle through the quadriceps tendon. The patella facilitates the function of the quadriceps to straighten the knee. The patella lies within the patellofemoral groove. This groove allows the patella to slide vertically with normal movement of the knee joint.
Dislocation of the patella occurs from the patella sliding laterally out of its position on the knee. It may occur from a blow to the knee, acute trauma or severe twisting of the knee.
In majority of the cases dislocation is associated with pain, swelling and a lump on the lateral side or outer side of the knee.
The patella will usually relocate by itself back into the normal position in the groove as the patient straightens the knee.
Sunrise view demonstrates lateral dislocation of the patella. X-rays should be carefully evaluated for the presences of osteochondral fragments displaced within the knee joint.
    Mechanism of patellar dislocation:
    •Medial patellofemoral ligament failure.
•Lateral dislocation of the patella may also cause direct impaction injury to both the medial patella and lateral femoral condyle.
    Causes of patellar dislocation
•Weak VMO muscle (vastus medialis oblique).
    •Large Q angle ( normal male 14- female 17)
    •Excessive foot pronation
    •Patella alta
    •Genu valgum (knock knee)
    Treatment
•First episode is treated nonoperatively unless there is a loose fragment inside the joint which should be removed surgically
•Use knee immobilizer or a brace for 2-3 weeks to allow medial structures and ligament to heal.
    •Quadriceps strengthening exercises.
    Complications
•50% of the patients treated conservatively will have recurring episodes of instability and some patients will require surgery.
•Common complication of patellar dislocation is chondromalacia.

Just came here to say keep on exercising don't give up, it prevents your knee from getting dislocated.
I stopped exercising for a while thinking my knee is fine and back to being normal but today it dislocated again, it hurts so bad.
Don't stop exercising no matter how strong you feel you are.🙂💐

I dislocated my left knee twice, first when dancing at age 17, and again at age 21 whilst wearing a knee guard. I had ligament transplant surgery that same year. I am now 63, and no more dislocations. I have learned to adapt automatically to some of the permanent limitations from the surgery, mainly the lack of " suspension " in the joint. I still dance in community theatre. And although I have not been able to do some simple activities due to the lack of spring in my joint, I am ever grateful that I have been dislocation free for over 40 years now 🙏

@@susfrank2x372 I am so sorry to hear. I had to see an Orthopaedic Specialist/Surgeon. All in all I have had 3 surgeries to that knee. A second one about 8 months after my main procedure when I underwent a ligament transplant. By body began to reject the attachment screw. By then the ligament had ( thankfully ) knitted successfully. My third op was when I was 41. Done by a different Specialist, who removed debridement ( floating scar tissue and bits of loose cartilage) that broken loose over the years.
Depending on your immediate recovery from your dislocation, it may be a good idea to see a Specialist. He/she will be in a better position to assess you and advise you better with regards to your sports activities. I had a reoccurring dislocation in my early 20s, from dancing. I was a leed dancer in theatre at the time, and I was referred to an Orthopedic Specialist.
You are young, and hopefully with good intervention, you may still be able to enjoy your sports. I wish you well 💐

@@katarina7822 I am sorry to hear you experienced the same injury through dance. And thank you for your kind words regarding my recovery. After my second dislocation, I saw an Orthopedic Surgeon. He performed my surgery. I did experience a few other complications along the way that I never mentioned in my first post. My body did begin to reject the screw that was holding the transplanted ligament in place about a year after the surgery. Though fortunately everything had fully knitted by then. However it did involve another surgical procedure to remove the screw and more physiotherapy in order for my knee to regain the ability to bend. And in 1998 I had a third surgical procedure to remove scar tissue that had built up in the joint throughout the years and would sometimes become trapped behind the patella. So my case isn't necessarily what most people will experience. All in all, I can say that having the ligament transplant surgery was successful in that I have suffered no further dislocations in over 40 years. However in my case, I did loose the " suspension" in my knee. It is perfectly flexible, however because I no longer have the " spring element" in that joint, I learnt to adapt around certain movement of turn and twist etc. It is second nature for me. I still dance which I love. However, there are some routines I could never do again after surgery. I am sorry this is so long. I am not sure if you dance competitively or as a hobby sport. I danced in theatre. Depending on how you feel, you may wish to consider seeing an Orthopedic Surgeon, who may be able to advise you from your personal experience of how the dislocation occurred in your dancing and if what you are doing to assist in your recovery exercises etc. will be enough for you to continue dancing without surgery. I do wish you a full and successful recovery and many more years of dancing 💖

I have a condition where my tendon is weaker than normal people have, so as a result I was born with a natural dislocated patella and I grew up with it (both of my knees btw). There was no way of holding it in place without surgery, but I couldn't get my surgery until I was 13. And as a consequence of growing up with both dislocated knees, my thigh muscles are weaker than usual people. The worst part is when I'm not paying attention a little bit when walking, I can just fall down without reason because my muscle is weak. And if it's in the public, you know how embarrassed I am, what's more if somebody asked me if I was okay and helped me up, and I just say no it's really nothing like literally nothing.
Thx for reading this, just want to let everyone know that there are people out there that seems like it's a common situation, but the root of it is not necessarily so.

I'm still thankful to my practicum prof for letting me to undergo in my ojt. Sadly, I had to wear a knee support because I don't know when it will dislocate again. My parents don't want me to have surgery because they're afraid of complications. I thank God that I finished my ojt and for everything.

Got surgery in 2017 but dislocated it again recently. Doctors always tell me there’s nothing permanent or really nothing they can do. Hi Lytle,
Listen if you kept your knee in complete rest from a year, things would be much better. But since circumstances got the best of us, please don't lose hope. Body has it's own healing system. Please consider keeping it in rest now for about 1 or 1 and a half years, if you want that knee back. Doctor will tell you a lot of things, but the reality is doctors are doctor by reading books not by any practical means. Keep it rested for atleast 2 years or something (Since it's 10 years). Also don't do any heavy physical hardship for atleast 4 to 5 years.
Rest is the best treatment for ligament problems and bone problems. Doctor really can't do anything. Once you get bone problems, you lose atleast 4 or 5 years of your life. But don't worry, it is curable. Rest is the only solution for these injuries. As I have mentioned before also, "Doctors are doctor by reading books not by any practical means"

In 2018 when I was 10 years old, I was trying to get up a concrete plateform. I hit my knee aginst the platform edge very hard. In my right knee, above the patella, I got hit. Since then, it has happened 6 times that while running, my patella displaces slightly, I lose balance and fall down. It is very painful. I can hear its sound while falling. I think I damaged my medial retinaculum or vastus medialis.

i've had 3 knee cap dislocations when i was younger. most pain i;ve ever felt and still makes me squirm thinking about it. I was told that i had abnormally stiff tendons and muscles around the knee joint (thanks Mom side of the family!) as well as shallow grooves on the socket where the kneecap sits. So, some people get dislocations because things are too loose but for me it was because it was so tight, like a rubber band. orthopedic performed sugery to have the knee cap sit more naturally. made all the difference in the world

Very helpful video! I also talk to my knee and tell it that it needs to straighten up its act! LOL I just realized that this is what has been going on with one of my knees for years! I had posterior tibial tendonitis for 17 years in my right foot & ankle due to an injury. That took 7 years to get a good diagnosis, and another 10 to find a surgeon who would do something about it (I also moved several times during that time period, which didn't help with continuity of care). It got to the point where, every time my ankle gave out, my knee twisted. I told this to my podiatric surgeon, and that's when he decided to operate on the foot & ankle. That was nearly 10 years ago. For at least the past 4 years, my right knee has had an unnatural swelling, and more pain than the left knee (I have osteoarthritis in both knees). Despite wearing braces on my knees, the right knee keeps locking up on me, and I can actually feel the knee cap slide out of position and back in again, which is VERY painful. When I told my current orthopedist about this, he did a BACK MRI. Yes, he thought it was my back, so when that MRI came back ok, he dropped it! So I dropped him (even if he was playing a hunch, dropping the issue is not an option that I have).
I need a knee specialist. A couple of months ago, it dawned on me to look up if knees can become dislocated, and voila, all of the medical research I'm reading sounds familiar. I spoke with my PCP about it, and I am trying to find a practice that has a knee specialist in my area. I live in such a rural area, it's not easy to find specialists like that. I used to have a knee specialist (over 10 years ago) but he retired. My mother and sister were medical professionals, so I'm not totally ignorant when it comes to understanding my medical situation. I have tried every conservative measure that's been asked of me, but I have other medical conditions that make things a little harder for me. I've been wearing two braces at the same time and it doesn't stabilize my knees anymore (a soft spandex & elastic slip on sleeve, with a metal, nylon, and Velcro brace over top of that). I can't even get my housework done anymore, I can't get on my knees, they both buckle or lock, and I actually have had a back problem for about 32 years now that keeps getting worse). It's depressing. I understand that we age, but I just want to be able to clean my home so I don't look like a hoarder and a slob, because it's isolating when you can't get things done and you have no help.

I dont usually comment or share but this topic is one i am fairly familiar with. I have had my patella dislocate so many times i cant remember the exact number. Here are some of the instances when. They have occured starting with the first at 16 i was playing basketball in high school went up next thing i remember im on the ground patella to the side it was out for eight hours till doctors relocated. Again at 18 playing basketball. At work tripped on metal bar fell off trailer, riding bike,in car, going down stairs, getting out of bed, and numerous times while sleeping. Sometimes 2 to 3 times a night. I have needed the help of emergency room 5 times rest of the time it goes back by itself or i put it back in. There is usually a little pain but now i just put it back in and i can continue doing what i was doing. I am very gratefull that i haven't torn anything as of yet. If this happens to you dont freeze up try to sit up make sure leg is straight try to grab your toes(or put a belt or rope under foot) and pull slightly upwards as your leg comes to your head. It shoud slide back in. After dont make any sudden movements it might pop out again. Well i hope this kinda helps. Oh las dislocation 2days ago. I am now 47.

My kneecap first dislocated 13 years ago. Ever since then, it has dislocated about 1-3 times per month. It is one of the most painful things I have ever experienced and it happens quite often. I just recently went to the doctor for it and am due for a surgery. Hopefully this can fix it and I can play sports without being terrified of my knee dislocating.

I injured my left knee once when I was a kid when me and my brother had a fight (you know how kids are haha) And I injured it again when I was in high school, 8th grade. I was walking on the stage that time and it was wet because of the rain. So I was walking on the stairs (I was extra careful that time but I think it wasn't my luck haha) and since it was slippery I fell and last time I remember I was at the last step of the stair holding my knee and crying. But the shocking part of me was when I looked down at my knee and saw it was dislocated. I was really shocked that time and I just cried like so much. I was also scared like I had a feeling I'll never walk again because of what happened to my knee. But yeah, ever since that happened, there are things that I am now more cautious or careful like if I ran and if I want to I just couldn't run faster because I'm scared that my knee will fell out of place again and since then I've come to realize that my left knee is weak and when I say weak like if you put a force on it (well not that strong) its just hurts easily and the top part like if you hold it and like move it left and right its just easily dislocate and that what really scares me. And all my life I really regret not exercising regularly or eating fruits or drinking milks just to strengthen bones hahaha. But yeah, right know my knee is still weak and I really can't exercise because I've been busy in the house and studies but if I had a time I really want to exercise and strengthen my body especially my legs. I really don't want to happen it again twice is enough hahaha.

im 22 and this injury is ruining my life. second patellar dislocation and i will be out of sports and training for 2 more months at least. stretched every ligament possible, doctor said he's almost 100% sure it will happen again and then i will have to undergo a surgery with a 6 month recovery time. i have to give up playing sports with lateral movement (every fun sport) and long distance running is out of the question now because of the arthritis. i had just recovered from a broken toe and immediately dislocated my kneecap, so i've been sitting on my ass for 4 months now. my squat and deadlift have definitely plummeted. physical therapist says i have the left knee of a 75 year old but my right knee is fantastic.
ended up getting so depressed that i got sent to a psyche ward while in a leg immobilizer for 6 days. im losing muscle mass and turning into a sad pillsbury dough boy. do not fuck around with your knees.

My physeo put it back into place but there was an osteochondral medial fascet fracture, and the chipped bone had to be removed via keyhole surgery. After 6 months I got back to sports but just 2 months in, I dislocated it again while playing football. Is there anything other than weights i can do to recover properly. It doesn't need contact, a lot of times the patella dislocates by a sudden twist of the body eg if u twist ur right leg to the right and put pressure on it it will dislocate.

Leg and knee have gotten a lot better. I researched on my own and I’m almost 100% sure that this is what happened to my leg. Even states that can be due to a collision. I was knocked all around in the car all the while I was slamming down on the brakes. Hurt sooooo bad but after 4 weeks I can finally walk without limping although I still feel popping in my knee and some pain.

( hurt like crazy ... can't feel from my knee to my foot ... thought i broke my leg). I have had it dislocate from running fast to wrestling with friends ... The latest happened this past weekend now at the age of 34 while i started playing hockey again after taking 5 years off.... the pain is still the same but now i know that i need to get my shin pad off asap and it will reduce the swelling .... one of the worst injuries i think to get because its so unpredictable. Wondering if i should look in to surgery...

+Jenna Taylor Hey Jenna, I fell your pain. I'm only 16 and have dislocated my knee twice (the same one). This knocked me out of sports for 2 seasons each time. I feared that it would happen again after the first time and it did but recently, I've started various exercises in order to strengthen the muscles and ligaments that are responsible to keep in on track and in place. I also now use a brace that aids in tracking that I wear during sports to decrease the risk even more. I'm not perfect and never will be but the best way not to live in fear is to spend a lot of time doing exercises as well as maybe get a patella tracking brace to use during activities that you would feel comfortable having it on. But make sure you do not wear it when it is unnecessary or else you will weaken the muscles and make the brace do most of the work when it shouldn't. I use my brace for sports only to help track it when I don't always have full control of it. If you have any questions or concerns, feel free to message me because I know EXACTLY how you feel and I wish you the best of luck.

i’m a very active kid and recently i have found out i inherited my moms knee problems, basically, the bones that connect the bones below my kneecap are more shallow then they should be, it popped out the first time in my left knee in august 2023, while i was army crawling on my friends floor to move something, the next time it happened was during a softball game, i was playing catcher, a runner was trying to steal home, i spun on my right leg to fast and it popped out, i didn’t react but it hurt like crap, i finished the inning and didn’t tell anyone i was hurt and just powered through the game. third time was softball practice, i was practicing a double play when i was rotating from catching the ball to throwing the ball to first, i pushed back on my right leg while it was too low to the ground and my knee dislocated. fourth time was at school, i was playing gaga ball and my friend was throwing the ball up back up from the hill and it was kinda a bad throw so i went and jumped for it, again i went to low on my right leg and still pushed off of it, making it dislocate, that was the first time anyone at school saw it, and a lot of people play gaga ball, multiple people saw it actually move out of socket and back in. finally the most recent time was the day before my track meet, i was playing four square at our extra recess, my friend who had missed the ball and went to go get it, threw it a little high so i jumped for it, nothing was to low got the ground, i didn’t push off to hard, but the kid in the square next to me was kind of overweight and he bumped into me while i was in air, he just happened to bump into in the exact spot where it’ll pop out, so since he hit it perfectly, it popped out, and it probably wouldn’t had since i had my brace on, but it had slid down at the exact moment when he bumped into be, the second i was going to land i was going to fix it, but of course, i couldn’t. if anyone was wondering i did fine at my track meet
