    You folks may not believe it but a lot of the issues people have with swallowing in westernized countries (aside of having a stroke or something) is the diet you have. I’ve had swallowing problems since my early teens, after trying everything I went carnivore and Keto diet and all the inflammation in my body started to subside, I fast more and no sugar no bread and no vegetable oil (seed oils) except olive oil. After about 1 month things started getting better and my body feels better. Another thing is exercise. Get cardio and weight lifting in, it will improve general health. Last, don’t be negative. Consider all your blessings and thank God for everything. You will be happier.

    I had a stroke that caused me life long issues with swallowing. Something so easy and natural to most people, is something I have to actually think about and concentrate on. I had have basically physical therapy to "teach" myself how to swallow, by having a scope put down my nose and watching on a monitor, in real time, what was happening as I ate pudding. Not exactly fun to do multiple times a day. And it took almost 6 months to get good enough at swallowing that my doctors were confident to remove my PEG tube (for non doctors, that's a feeding tube placed directly through the outside of my body into my stomach, everything I ate and drank went in my stomach through this tube). Never take for granted such seemingly easy things you do everyday without a second thought.
    BUT, I have a question not related to stroke, why does it sometimes feel like after swallowing something such as a large pill feel like it isn't swallowed and feels stuck? I highly doubt pills get stuck in my throat, but curious why it can feel like that.

    From what you describe I have esophageal dysphagia. It started about 36 years ago and was only when I tried to swallow large pills like prenatal vitamins. The last year or so it has been happening more often and with food - not liquids at all.

    Im like the opposite of normal swallowing issues, I can swallow pills no problem, even decent sized pills, but food is an issue for me. I feel like I have to chew the food into oblivion, even if its already mushy food, and then swallow little bits at a time. The food basically has to be the same size and a pill, its crazy. it took me 45min to eat 3 scrambled eggs the other day.

    Since I had throat cancer surgery and radiation therapy I have not been able to eat. I survive with a feeding tube

    My food sometimes “sits” in the lower esophagus and comes up to the throat hours later. Sometimes hot food hurts in one spot. I was just diagnosed with Barrett’s esophagus. I just want you to know I really appreciate your videos. I just found you today!

    Thanks to your video, at least now I know I'm dealing with some kind of problem in the esophagus.
    Recently started having problems getting meat/bread down, but only after about 3-or-4 swallows.
    Drinking between swallows fixes it, but with too many swallows, liquid takes much longer to help.
    During the onset, I once I made the mistake of swallowing 5-or-6 times, thinking the liquid to help.
    But I guess once you've swallowed too many solids, the liquid cant make its way to the bottleneck.
    And in this case, drinking more and more liquid just prompts the gag reflex.. Lesson learned!

    Hiatal hernia can occasionally cause swallowing difficulties. This would be included under "structural".

    I had swallowing problem for the past 4 years. Went to gastroenterologist and ent also did barium swallow which was normal. Then I went to the best ENT doctor of my country and it shows that I had pharynx tissue increased along with tonsil and other types of infection kind of thing.

      @@bolaskinade ohh I went to Thailand for further treatment. They said I’ve oropharyngeal dysphagia. And gave me therapy, exercises and medicines. Now I’m much better than before

      i cant swallow properly usually at the begining of my meal i eat fine but then when im half way through its becoming a nightmare and i need to use water to finish up😢

    Yes, enjoyed watching this very informative, educational and so useful video. And I can 90% confirm In what category my current issue is. Transferring food and kind of spasm doesn’t allow some certain and type of solid food to be swallowed comfortably and I myself within 5 months that this issue dramatically started in my throat can confidently say swallowing that plant-based foods eating fruits like apple and vegetables like carrot corn lettuce , any salad leaves .. are very and kind of risky for me and I avoid them . All animal base food like meat chicken that well cooked I. Stew and soft rice with olive oil , dark chocolate any chocolate consistency and soft cakes as well are good especially chocolates and dates syrup honey are wonderful and dates syrup was what I survived with for that First 42 days that I lost 5 kg . I survived with dates syrup water and tea for 42 days ! And only with
    diaphragmatic breathing / deep breathing and facial /tongue& swallowing muscle exercises finally I got able to eat chicken stew with rice porridge in second month then after 3 month like a normal person till now but still for fruits like apple I need to make purée or blend but soft leafy leaves mixed with much olive oil in layers of soft bread I can swallow . I never done any test till now my vocal chords were affected a lot too that time now much better almost gone but I like to do barium test and image from my Larynx (the easy test not that laryngoscopy ). I would get glad if you also share your recommendations in this regard. Thank you

    GutDr thank you for a nice video. my father just got the evaluation with the speech pathologist done with result of Oropharyngeal Dysphagia. Besides potential speech therapy, and followup with GI doctor, do you suggest we should also be talking to a throat specialist to see if there are other treatments? are they called the Otolaryngologist?

    Whats crazy is the first time food got stuck in my esophagus, I drank some Kombucha after the food wouldn’t go down and I almost choked. I couldn’t breathe for like 30 seconds and finally by the grace of God I was able to cough and vomit the Kombucha up. It’s happened once more with me trying to follow the food with a drink since it feels stuck and I choked again. Now when it happens I don’t drink anything and it very slowly works it way down or I have to vomit it up.

    Dysphagia due to DYSTONIA..i have it inside my throat tongue esaphogus etc also jaw diversion and cervical DYSTONIA contribute to it..nj tube almost killed me because rhe spasticity made me suffocate..
    My bmi does not go over 14

    I feel that I have this issue, it's only developed in the last 2 years. I feel Iike it's so hard for me to swallow food and liquids. It even causes me discomfort in the back of my head because my muscles are working so hard. I don't smoke or drink. And now it even hurts to talk for a long period of time. I do have nasal drip which I think has caused it but I'm not sure. I think I have more of the issue with esophagitis. Also, I'm a person who doesn't take medications nor will I ever opt into it. I just don't think it's for me. But I am willing to see what a ENT Dr has to say.

      Dude this is me. The back of my head, neck and even tongue feel so tight and pressured when i eat. I get exhausted trying to talk sometimes like it requires alot of effort to speak. I have had voices changes also and post nasal drip as well. This sucks

      @@Hondo34 I think it helps to sleep with Vicks rub on your chest and try a humidifier. I did that before and I changed my diet. I truly believe the cause is anxiety, and malnutrition. Stay hopeful, stay hydrated, and know that it WILL get better. Take baby steps, that’s what I’m doing. It will eventually go away. And make sure to go outside and get some fresh air / sunlight. And I strongly recommend herbal tea with raw honey. I’ll pray for you!

      @@Hondo34 also, about the tightness of the neck, head and tongue.. I think it’s from wearing a mask and from the post nasal drip. Look up some stretches to safely fix that or invest in going to a spa to get a massage once a month for your neck and head. That should help!

    I have been trouble swallowing and my gastroenterologist said it could be just anxiety. I feel food gets stuck right at the back of my tongue, I hope i can get another point of view and get some studies done.

    I'm relieved that i discovered that im not the only person suffering through this problem, but am depressed even more so because i dont know how to fix it.
    Everything started for me a few months back, one night i found myself not being able to breathe properly in my sleep. Almost passed out because of my throat being so messed up. Panicked for about an hour, eventually relaxed enough to sleep but was still anxious and not breathing too good in my sleep.
    A few weeks later I'm noticing that I'm starting to have difficulty swallowing foods, even just my saliva. Researched it and found that these two symptoms are part of this general disorder that affects the breathing and swallowing muscles.
    What i noticed helped me is getting a neck and shoulder massage. That same night i panicked, my brother massaged me enough for my muscles to loosen up to breathe properly.
    I'm trying to loosen up again on my own because im still having these problems but its hard. I'm pissed because ive been dealing with way too much physical and mental suffering this past decade. I want to be happy and free. We all deserve it.

    Hello Gut Dr,
    I've been diagnosed with Achalasia since my 15th. I am still really curious about the cause of it, is there anything more known about it? In the Netherlands the Doctors think it may have something to do with auto immune disease. Could you do a video about Achalasia? It's still very unkown in society and more knowledge and widely informed people would really help for the people who are suffering from Achalasia, like myself.

    I found your channel helpful and subscribed. Would you please make a video for after bariatric surgery? On what happens to the stomach? My brother had this surgery and it’s been a month he is suffering from pain. Is it normal? 😔😢

      Thanks very much. I do plan on making a video(s) that covers surgery including bariatric. It's not uncommon at all to get pain after surgery and that can last for some time but I would recommend that your brother contact his surgeon if he has any concerns

    Hello doctor, i began having trouble swallowing food around 10 years ago. It started happening occasionally but over time progressed to the point where i was unable to swallow pills anymore. I found that eating dryer foods that require a lot more chewing tended to get stuck in my throat. When i eat out i always opt to eat foods that have a lot of sauce or typically don't require as much chewing as i fear a choking episode to happen. When that happens the food usually sits at the bottom of my esophagus for a good 5-10 minutes and i keep having to drink water to try and push it down. It's always a very scary experience and always leaves me trembling. Sometimes i have delays in swallowing, even when it's a normal sized bite for me it sits in my throat for a few seconds before coming down. Some rare days i have no issues swallowing at all and even feel like my chest is looser but most days I'm not so lucky. I've been told to wait for 6 months to see a doctor regarding my issue and i really want to know what's wrong with me and how i can treat it. Many people don't realize how badly this affects quality of life. Always being the last one on the table chewing food is embarrassing, tiresome and stressful. I just want to be normal and enjoy food and swallow pills like everyone else.

      It's happening to me & I've always and I get severe anxiety when it come to eat, I accidentally ate something with spice and been fourth day In scared to eat and when I swallow small gulp of water in hurts.

    Have you heard of dysphagia getting worse...where you're not eating at all, but as you swallow your saliva it is like the brain sends signals to your throat muscles, in which there is an extreme delay in simple swallow for seconds to minutes causing gasping and dry throat???? Please explain...I hope you understand my question. Thank you for your time.

    Gut Dr, I have problem with frequent belching for about years now which is associated with acid reflux and heartburn. After a meal when I take lot of water I feel discomfort and sometimes pain in the chest and difficulty in swallowing. What might be the problem with me?

      Thanks Ate I am curious about the precise reporting on panendoscopy? Was this actually gastroenteritis? Or was this colitis? Gastroenteritis is defined as inflammation of the stomach and intestines but typically as a consequence of an infection (often bacterial or viral). Often this resolves before too long so if you have repeat procedures carried out by your gastroenterologist it would be reasonable to presume that it will resolve. However some forms of colitis could be due to underlying conditions such as inflammatory bowel disease and changes may still be present on repeat procedures.

    Just developed this now, choking a lot, feel like when I move the food to the back of my throat and then either can't swallow, or swallow and it goes down awkwardly, slowly or gets stuck. Literally nearly choked a few times recently. All this is in the last couple month. Don't even Want to eat anymore, just having yogurt, protein shakes and soups. I'm even struggling with swallowing all them. I've had stomach issues with inflamed stomach due to acid, had that since I was 18 and I'm 34 now. I've Had a bad back since working heavy work and off the gym, damaged discs and bad upper posture, I'm wondering if any of them issues or a combination of both have weakened the muscles/oasophycis area. Either way I'm in a bad place through this, always loved my food. Anyone else got similar going on?

      I started feeling like this for the first time about a month ago while eating soup. It freaked me out. It happened again a couple days ago with some food and then it happened the 3rd time this afternoon while eating soup again. I'm thinking of making an appointment with my gp because I'm worried af now.

      @@Seekingtruth-mx3ur aye get checked just to put your mind at rest, mine had stopped on its own so try not to worry about it, it's probably nothing, sometimes the swallowing muscles are a bit weaker, there's exercises you can do to improve you're swallowing . Hope you get sorted

    Gut Dr. Just curious, what complications can arise when a ileostomy patient becomes pregnant? is there a higher risk to, both patient and child due to increased internal bowel pressure? If birthing complications arise, do potential risks from c sections increase? Just curious about your opinion in this scenario. Thanks!

      Hi Nathan great question and the first thing to say that having an ileostomy should not stop one from having a successful pregnancy. Many women with an ileostomy go on to have a successful pregnancy without complications to mother or child.
      The most significant visible changes that will occur during pregnancy are to the stoma itself and to the skin around the stoma. We know that later in pregnancy the stoma may begin to protrude less and in fact the skin around the stoma starts to flatten out. The stoma can in some cases be flat against the skin (flush) or even retracted during the latter stages of pregnancy. Fortunately that shouldn’t alter the functioning of the stoma - it’s more of a cosmetic issue.
      Sometimes there can be complications as pregnancy advances such as skin irritation or lacerations to the stoma. Applying a moisturising cream can be helpful. Nausea and vomiting associated with morning sickness can also be challenging for stoma patients and can sometimes result in significant fluid and electrolyte losses. If this is a concern seeing your local doctor is important and they may need to arrange for a short time in hospital to provide intravenous fluid and electrolyte replacement.
      The other thing that can sometimes happen is that flushing or irrigating the ileostomy pouch is more difficult particularly in the second and third trimesters. Useful tips would include lubricating the catheter and position changes but I’d certainly recommend discussing this in the first instance with a stomal therapist or nurse.
      The most dreaded complication is intestinal obstruction which is fortunately uncommon. If this does occur and there are concerning symptoms e.g. marked abdominal pain, absence of passing flatus presenting to your local doctor quickly is paramount.
      C-sections can sometimes be required and obviously a decision to go ahead with a C-section should be assessed medically on a case by case basis. There are the usual complications that can arise from a C-section e.g. wound infection to be mindful about and of course there are concerns about scar tissue. Having said that C-sections can happen regularly for ileostomy patients without any complications but it's important to get your doctor and stoma therapist involved early.
      After delivery parastomal hernias which are hernias around the ostomy site or prolapsed stomas can occur. Fortunately these are uncommon but important to be mindful about. Again seeing your local doctor quickly should these arise is very sensible.

      Hi Sal have a look at my video on gallstones - you may find that useful. Basically with the loss of the gallbladder to store bile acids one of the issues that can happen is free release of bile acids into the small bowel which can lead to a condition called bile acid diarrhoea. Some people can develop a condition called post-cholecystectomy syndrome. In this condition there can be a continuation of symptoms caused by gallbladder problems or there can be new symptoms developed that correlate with the gallbladder. For example in this condition symptoms can include nausea, vomiting, fatty food intolerance, heartburn, flatulence, indigestion and upper abdominal pain. The exact mechanism behind post-cholecystectomy syndrome is not known but it’s thought that alterations in bile flow is responsible.

    My first time was 1 year ago when I was drinking coffee it stuck in my throat, the coffee didn't go down or up and that's where my problem started and now I've learned some styles how to swallow food or water or coffee but it still sticks in my throat everything I eat, water, bread, soup, everything, sometimes when I have a burning throat, I swallow better, I don't know where the problem is, I did the endoscopy with a tube and it turned out fine, I suffer from chronic diffuse gastritis, maybe this is a reason

    Been feeling pain in lower right side of my stomach and after seeing a doctor and eventually scanning me turned out there is some poo still in my intestines and to be honnest i am struggling to get all out but not constipated at all when i do. It's very annoying and always feel bloated very fast. Anything i can do or take to help this process go smoother?

      Hi Richie sorry to hear about your symptoms which are proving challenging. I’m glad your doctor has done tests mainly imaging studies to rule out mechanical obstruction. It sounds like what you’re describing is faecal retention which itself can be a cause of bloating. I mentioned in my video that not everything is gaseous and indeed faecal retention and impaction is a known cause of bloating. It would logically follow that anything to help with faecal retention may help with bloating. I would touch base with your local doctor to make sure that there’s no further tests that they need to do or anything else they’re concerned about. If the clinical picture all appears consistent with faecal retention you could try in the first instance optimising natural fibre sources such as chia seeds, psyllium husk and oat bran, aiming for at least 30 grams of fibre a day. These sources of fibre are low FODMAPs so shouldn’t contribute towards bloating. Essentially what I would be advocating for is low FODMAPs, high fibre foods that can assist with relieving faecal retention whilst not exacerbating bloating.

    I sometimes cannot swallow fluids and then blackout. This came after my 9th stroke and changed in some ways after my 12th stroke, The last 4 were basal ganglia due to large blood clots and AFib. Why can I swallow food, but not water? Why do water get hard in my oesophagus and cause me to faint?

      This is a difficult answer because there are a wide range of diets advocated for different people in different parts of the world. Additionally diets optimised for weight loss can substantially differe from diets optimised for 'digestion'.       The symptoms described make me wonder if it's all due to gastritis or whether it could actually be related to IBS. Often there is an overlap between gastritis and IBS. The treatment of isolated gastritis is quite different from the treatment of IBS as the latter may involve not only medical therapies (drugs) but could also involve dietary measures and psychological interventions largely to reduce stress and anxiety.

    I have had my pharynx, larynx and esophagus examined and everything was without pathological changes, but I still have a problem swallowing.
    And I will go for an examination of the salivary glands.
    but my mouth is dry when I wake up in the morning so my mouth is very dry and then my saliva is formed although saliva is like foam and then my tongue is white and I scrape it off and that is a coating and it is constantly forming.

      Hi Ashgar thanks for the comment and sorry that your girl is going through this. I can’t give a specific medical answer without a comprehensive medical history, examination and the results of investigations but can provide some general comments. Vomiting in a young child has many possibilities. One of the most important questions to ask is if the vomiting is acute or chronic. If acute we obviously need to consider infectious gastroenteritis but also think about other causes e.g undiagnosed urinary tract infection. Then hydration needs to be considered - your local doctor will be able to consider whether or not supplementary fluids are required and whether this is given orally or intravenously (via a drip). Now if the vomiting symptoms have been present for weeks to months your local doctor will be wondering about whether your child is growing properly and if not this may be “failure to thrive” and they may refer to a specialist for further assessment. That may include blood investigations, imaging and possibly investigations to see inside the gut (endoscopies).

    I did the barium swallow study I did the camera down my throat doctors couldn't find nothing significant I still have my problem of trouble swallowing it gets stuck in a weird clicking Motion(best way I could explain it) in the back of my throat then I have to re swallow so I don't choke.

    My son is having issues swallowing and it's even with liquids too. He in the ER 3-5 times a week and they just put a balloon in his throat to stretch it, but it doesn't last long and he's back in the hospital again.
    His tonsils are bad and he's forever removing the rocks, but the ENT specialist say he doesn't need them out, but I believe this could be causing his issues
    I so scared he will choke to death if something isn't done soon!
    Please, what are your thoughts?

They just keep throwing medications at him which has now cost him hundreds of dollars, but haven't helped, but they don't care because every time you have to go back to see them again they get paid from the Insurance companies.

    Can anyone here tell me what my problem is? When I opened my mouth in front of my mirror past my epiglottis, about 2 or 3 inches below, I could see two flaps moving above my trachea, which made me think that my vocal cords were protruding or sticking out. I don't have any problems aside from mild swallowing issues, but I don't know if that's because of my anxiety or what I went to Saint Mercy Emergency Urgent Care to walk in about. They failed to check it out because the front desk labeled my issue as a sore throat. Even though I explained to her what was happening, she said she didn't see anything, which made me think she doesn't know what she was doing. I have no phone or way to contact a laryngologist until the 19th, and I want to know if I'm going to have to lose my vocal cords or if my life is in danger. I don't have any problem making noises or antying like that, but it's scaring. I just see it being visible and moving anytime I make noise but staying open when I'm silent. I did see an ENT doctor a couple years ago about my visible epiglottis, which he told me was normal because I was born with a high-rising epiglottis. That's why I see it sitting on my tongue, but behind it I see two more flaps move when I make noise. I don't know if I should worry or what because I don't want to lose my vocal cords because it means I won't be able to breathe or eat, which means I'm good as dead.

      I just need some reassurance so that my anxiety calms down and I can eat again. The mild swallowing difficulty I am talking about is that I can swallow food in small bits, but when I do, my throat feels like I just ate acid reflux, which brings back my anxiety and prevents me from eating. I can breathe with mild difficulty because I'm already dealing with preexisting shortness of breath from 24+ years of smoking; otherwise, I feel fine aside from severe anxiety over this.

      I'm 30 years old, 123 to 120 pounds, 55.8 kg, and I've been a smoker for 24+ years, but I'm stopping today. I have no major issues, just seeing two flaps close when I make sounds and stay open when I don't make sound, and it's right below my epiglottis. I can see it in the mirror. When I make sounds, the appearance of it makes my anxiety worse.

    For some reason, every now and then, I will, quite suddenly, experience difficulty swallowing. It happens unexpectedly out of the blue. It doesn't happen very often either, it's just random. Would this still be classified as dysphagia? If so, what could cause a type of dysphagia that comes on suddenly then vanishes again for weeks at a time?

      Yes what you describe sounds like it could well be transient or intermittent dysphagia. Intermittent dysphagia is quite common e.g. someone can experience this when swallowing foods too quickly. It's not considered a problem if it's been very long standing as the likelihood in that case of a serious pathology such as cancer is very low. Nonetheless if it does bother you it would be well worth getting this checked out in the first instance by your local doctor
