Everything from the mention of large quantities of Sodium Amytal (a drug that had a history of being used as a truth serum, until it was discovered that it could induce "false memories" in patients) to the technically accurate descriptions of lobotomy to what is essentially an exposition of the gate control theory of pain suggests that "Andrew" must have been knowledgeable in medicine, chemistry and psychiatry.
They do explain that he's one of the most trained, intelligent and fantastical patients on the island. He's been there for 2 years at this point, likely hearing about other patients treatments. Combine that with his years as a soldier fighting the Nazis, no doubt hearing of there experiments, and his years as a U.S. marshall dealing with other criminals and a few conspiracy nuts. You can tell because when he says that she is, she barely nods her head, as if she's just doing that to appease Teddy and hide her real identity. But she's got to be real. Then, people ignore the exposition of the Truth right in front of their eyes, again given by the institution itself: The opening scene, Teddy can't find his cigarettes. His 'partner' says to him 'Have one of mine. Government employees will rob you blind.' Watch the scene where the "partner" goes into detail about the nature of the institution - '[details re OSS, HUAC] ...this whole place stinks of government ops.' This scene in the cave, detailing the use of psychotropic drugs and lobotomies, and TRUE horrors committed in NK prisons, Soviet Gulags and CIA facilities. People just brush over it all because at the end, Dr Cawley ties it all up neatly in a bow. Dark dark film. I spent time in a mental hospital in the UK, so I've lived it first hand. They try to make you believe you are schizophrenic and severely ill. They even got me to genuinely believe it myself for a while. When Daniels mentions the character Noyce who 'begged for the electric chair... Anything but a mental hospital', I sympathise with that sentiment enormously

No doubt, I have made mistakes and fallen many times in my life, but then how is that different from any other human? Cave rachel is just Andrew's mind speaking to itself , telling itself the same lies and delusions he tells the doctors every time he relapses. she's just repeating everything he wants to hear, just like "dream" dolores" is still this nice soft spoken woman that wants the best for him. you can see in the movie that he does a double "wake", like a dream within a dream while on one of the beds. they immediately ask him, "where did you go?" as if they knew he was knocked out and talking in his sleep or even woke up and "talked" to cave rachel, and asked him where his delusion took him this time . and once in the lighthouse, he mentions cave rachel to Cawley, who had not heard of that invention yet, and is surprised by the severity of the hallucinations and knows that Andrew is completely delusional and really, for lack of a better phrase mentallly unstable and therefore dangerous in his delusions.

apparently the book doesn't explore the conspiracy angle and reveals that he really is delusional and psychotic. there are other bits and pieces in the movie that aren't quite explained away like that as well (it's highly unlikely this psychiatrist would have been surviving down there like that in the elements- why would she have special survival training? why would the garrison on the island not have searched every nook and not found the cave-- what is she using to make a fire with? is she felling trees for firewood without being caught? hell scurvy would have set in if she didn't have some supply of vitamin c on hand, this whole angle that she is real is highly improbable). however i will say that a story which is ambiguous and could be interpreted either way in a circular train of logic would actually make for a better movie for sure than if if it were clear cut

Nah when I first watched this I underestimated Scorsese. I thought a lot of the dialogue felt unnatural in a few scenes, especially here. the two things nonone ever brings up about this movie is this scene and the scene with the security guard who gives the “God loves violence” speech. im not sure if he is the devil or what but he seems to indicate he is supernatural, nowhere more so then when he is having his back and forth with teddy and claims he knows him, in fact he claims hes known him for centuries. he is also never really noticed by the others yet is always in the background including at the end when teddy is taken to be lobotomized.
I love this story like, the opposite of love. It was really succinct and acutely psychologically scripted and yet magnitudes of terrifying. When I was young and technology not so advanced it would get so cold here in the North i thought I would die before thr summer came. More than once nazi armaments were pulled out of rivers and dug up on motorways- one UXB in famously flooded Bewdley. It seems the nazis could not wait to get drunk and solipsistic in ye olde pub!? I know I am replying to a 2 year old comment but... Sadly not many people (at least those who comment) can see the obviousness of the deception.
Most jump to the conclusion 'oh, so he was insane the whole time', even though every single detail of his 'delusions' were implanted in his mind by the institution, using psychotropic drugs, sleight of hand and suggestion.
Pay closer attention to the origin of all the details of the 'fantasy' (drowning is suggested more than once, the names that are anagrams, nature of the government operations, the piece of paper with 'Law of 4. Who is 67?'). Every single one originates with the doctors and is carefully and expertly suggested to Teddy Daniels.
Then, people ignore the exposition of the Truth right in front of their eyes, again given by the institution itself:
The opening scene, Teddy can't find his cigarettes. His 'partner' says to him 'Have one of mine. Government employees will rob you blind.'
Watch the scene where the "partner" goes into detail about the nature of the institution - '[details re OSS, HUAC] ...this whole place stinks of government ops.'
This scene in the cave, detailing the use of psychotropic drugs and lobotomies, and TRUE horrors committed in NK prisons, Soviet Gulags and CIA facilities.
People just brush over it all because at the end, Dr Cawley ties it all up neatly in a bow.
Dark dark film. I spent time in a mental hospital in the UK, so I've lived it first hand. They try to make you believe you are schizophrenic and severely ill. They even got me to genuinely believe it myself for a while. When Daniels mentions the character Noyce who 'begged for the electric chair... Anything but a mental hospital', I sympathise with that sentiment enormously

No doubt, I have made mistakes and fallen many times in my life, but then how is that different from any other human? It's not my place to tell people what to think anyway, and it definitely isn't worth obsessing and becoming paranoid over :) Thankfully, I've been happy, healthy and enjoying life and trying to be the best person I can be, free from the mental health system, and haven't had to go back for over 2 years. Cave rachel is just Andrew's mind speaking to itself , telling itself the same lies and delusions he tells the doctors every time he relapses. she's just repeating everything he wants to hear, just like "dream" dolores" is still this nice soft spoken woman that wants the best for him.
you can see in the movie that he does a double "wake", like a dream within a dream while on one of the beds. they immediately ask him, "where did you go?" as if they knew he was knocked out and talking in his sleep or even woke up and "talked" to cave rachel, and asked him where his delusion took him this time . and once in the lighthouse, he mentions cave rachel to Cawley, who had not heard of that invention yet, and is surprised by the severity of the hallucinations and knows that Andrew is completely delusional and really, for lack of a better phrase mentallly unstable and therefore dangerous in his delusions.

People always call this a plot hole because the hallucination knew things that Teddy/Andrew shouldn't have, but the reason he knew so much is really simple.The guy's been on the island for a few years, of course he's going to pick up on tiny bits of information from nurses, Doctors, guards, other inmates, etc., over the years. You can tell that this is the case due to his lack of information of what's the lighthouse really is. No information gets out, which is why the ONLY hallucination about it is of his wife warning him not to go there. As a patient, he's watched or heard about patients going to the lighthouse and never coming back, or coming back basically zombified. That's why his hallucinations about what goes on inside the lighthouse are incorrect. He had no idea what went on inside the lighthouse, only what happens after patients are brought there. He thinks that the lobotomies go on in there, when in reality, that's where patients are brought to determine if they need to be lobotomized, before being brought to some operating room to have the procedure done. The place is literally a mental black hole. This is how he even knew about the inner layout of Ward C the time we were to believe was his first time in the. there. He already knew the layout, subconsciously, because he had been locked up in the for viciously beating up George Noyce for calling him "Edward". He also mentioned how Teddy was the most violent prison in the facility, which is how he knew how to navigate Ward C, despite it supposedly being his first time inside.
He never beat up Noyce. That was a lie that Cawley told. Had he really done that, Noyce would've gone into detail until Teddy remembered. Yet, he doesn't mention a word about that.

apparently the book doesn't explore the conspiracy angle and reveals that he really is delusional and psychotic. there are other bits and pieces in the movie that aren't quite explained away like that as well (it's highly unlikely this psychiatrist would have been surviving down there like that in the elements- why would she have special survival training? why would the garrison on the island not have searched every nook and not found the cave-- what is she using to make a fire with? is she felling trees for firewood without being caught? hell scurvy would have set in if she didn't have some supply of vitamin c on hand, this whole angle that she is real is highly improbable). however i will say that a story which is ambiguous and could be interpreted either way in a circular train of logic would actually make for a better movie for sure than if if it were clear cut

Nah when I first watched this I underestimated Scorsese. I thought a lot of the dialogue felt unnatural in a few scenes, especially here. the two things nonone ever brings up about this movie is this scene and the scene with the security guard who gives the “God loves violence” speech. im not sure if he is the devil or what but he seems to indicate he is supernatural, nowhere more so then when he is having his back and forth with teddy and claims he knows him, in fact he claims hes known him for centuries. he is also never really noticed by the others yet is always in the background including at the end when teddy is taken to be lobotomized.
I love this story like, the opposite of love. It was really succinct and acutely psychologically scripted and yet magnitudes of terrifying. When I was young and technology not so advanced it would get so cold here in the North i thought I would die before thr summer came.

What doesn’t make sense? It’s Andrew’s hallucination feeding him the delusions he needs to keep pretending he’s Teddy. His chlorpromazine is wearing off so his hallucinations were getting worse at this point. He has the run of the place for days. And he never saw any of the experiments mentioned in this scene.
