Smith was interesting in that he had the Old Timey Religion thing going (which at its least gets people looking because) but also was portraying affliction that just wasn't manageable by the medical standards of the time - this to me was as insightful as Al's stone, in how the science/medicine of the time could (and wanted to) treat things.

It was obvious all throughout season 1 that Al genuinely liked the Reverand Smith and felt sad and angry that he was suffering so.
