I'm embarrassed to say I was stumped by the riddle when I saw this today. I mean, first, I had seen it before and heard the riddle, and second, the theme of the episode made it obvious. But even worse, for some reason it never occurred to me to consider a female surgeon, even today when there are a lot more of them than there were 50 years ago.
My family do it all the time eating talking and on social media if Your family can't do all 3 just say that and times are way better now then back in the day

Yet, Mike still treated Gloria at times way worse than Archie could ever treat Edith, and Gloria more often than not put up with it because Mike was "educated", and Archie was not. Then, in other instances, Gloria would taunt Mike for not acting like a man, and, in one instance, made fun for allowing his pants to be taken away from him. Both were so hypocritical.

If I remember correctly, the surgeon was his mother. And in general surgeons can't operate on themselves or relatives.
