Good intentions help processes evolve and adapt. In many corporate environments, companies hesitate to define a clear end goal, as accountability is often seen as a threat to future opportunities, which reflects a cultural issue.

"Definition of done" is a common missing component in instructions.
"I set high standards because I know you can meat them."

Thinking of an outcome may undermine the process. Always thinking about the fruit and forgetting that the root is what needs to be nurtured.
The secret sauce here is that this happens at every level of command. You have to be a follower and a leader.
