Spawn was severely weakened from a previous issue and well, he was never bulletproof and a bullet in the head is a bullet in the head.
What the video doesn’t mention is that he was in a huge fight with a demon spider that poisoned him earlier that week, Spawn isn’t invincible and a bullet to the head will knock him out and all that aside, this is an earlier story where he’s weaker due to not getting all the crazy upgrades he gets in later stories.
Bro's dealing with Demons and Hell Lords on daily basis, and is one of the most OP comic book heroes, but scrawny Kletus shot him in the head and hanged him.
To give him some credit, this was Spawn at his weakest and most vulnerable state. Hell's soldier isn't like most superheroes that are just permanently strong all the time even when he sleeps. His powers come from a very finite source that he regularly needs to recharge much like a phone battery or a gas tank. At his peak, he can handle demon lords, but when he's running low on juice like he is here... well, Kletus gets a lucky shot.
