This channel is great. Thanks for all you do. I know you can't give medical advice here. I have what I think is a rare tendinopathy, it's bilateral calcific insertional triceps tendinopathy. The calcifications are visible on x-ray and MRI confirms fluid in the tendons. The problem goes back several years, I had taken a long layoff from the gym to resolve the left side. When I finally got back into it I only had minor pain. I built up slowly only doing pushups. Eventually starting triceps extensions with only 3 lbs. After many months I graduated up to 10 lbs and out of nowhere was getting pain on my right side, near full extension of the elbow, accompanied by tenderness on the lateral side of the olecranon. I stopped doing the extensions but continued with pushups because there was no real pain. That was 10 months ago. In an attempt to finally resolve this, I laid off pressing exercises altogether for 4 weeks, then did some very light pressing I had no pain but the elbow was irritated a few hours later. So, I seem to be worse off for taking the layoff. I'm scheduled for a PRP shot as I've had 2 opinions that think that's a reasonable option. Neither opinion thinks the calcifications are the issue or should be dealt with directly. So my question is if PRP has any research for insertional tendinopathies? Also, should the calcifications be dealt with could they be the source of the pain?
Pretty much all the PRP research on tendons is insertional or origin. There are no (that I can think of) studies looking specifically at triceps tendinopathy, but anecdotally, yes it works. Good luck!

Thanks for your comment and sharing your experience. There's a lot of research (will make a vide on this very soon) on the different types of PRP, specifically looking at platelet counts. It'd be interesting to learn how many platelets you had in your PRP injection (can probably get a rough estimate by how much blood they drew) and correlate that with outcomes. @@JeffreyPengMD I am 54, very fit. I ran marathons, mud runs, and other competitive type races. I' have done weight training and calisthenics my entire life. I eat a healthy diet. I tried PRP because of the current information available, as I stated my results were not as positive. Thank you for the response.
Hi, I was just reading that you said they obtained about 5ml of your blood. I'm pretty sure that amount is no where near the required. 50cc is the goal! You need a platelet count of at least 10 billion or more.
I have been dealing with constant pain in my lower back on my left side as well as muscle inflammation over the past 20 years, but it got worst this past year. MRIs showed a couple of herniated discs and an annular tear. PT and acupuncture did not seem to help relieve the pain at all. I even had an MIS discectomy, which did not help and may have been made the pain slightly worse. I have started doing softwave therapy, but not sure how much that is helping me so far, but I heard very good things about PRP, so I am still hopeful and optimistic. Local anesthetics such as lidocaine has been shown to damage platelets. However, ropivacaine, is much less damaging to platelets and a small amount can be used to make these procedures much more tolerable!

@@Cybernetic263 I noticed improvements about the 6 week mark. Ask your doctor what and when to start strengthening exercises. That part is super important. No squats or lunges. Buy a foam exercise roll, sit on the ground with the roll under the pocket of your back knee, isolating your thighs & knee, and just extend straightening your knee. No weights, just plain foot lower leg extensions. Eventually ankle weights or a large bag of ice in a pillow case if you don’t have weights, just for the mild resistance. Some people see results in 4 weeks, some 12. It just depends on the extent of knee damage. One knee I had a second injection after seeing no results after 3 months. That second one worked and has not given me problems since my 2019 procedures.

I just watched this again and you are very convincing. I would love to try this therapy but have the following concerns. First of all my Uncle had the treatment on his hip and it did not work at all, I am not sure if he followed the instructions perfectly so am not sure as why it did not work. I also visited a regenexx office in Larkspur and they took xrays and told me it would not work. I assume because of lack of cartilage. Does this make sense to you and how do you diagnose the possibility of it working? Thank You.
Great information. 4 years ago I had 3 doctors who told me I needed surgery for the rotatory cuff. I found and alternative Bio energetic Dr. I had various injections (7 sessions) PRP injections for my shoulder and I'm 90 % cured. Now I have a meniscus tear in my right knee. I have app with another Dr that also uses PRP. (the previous had retired) I hope I will have the same results as my shoulder. I know at least 5 people that had knee arthroscopy surgery and was not successful. They are in pain and still going to physiotherapy. Thanks Dr Peng I just subscribe to your excelent videos.

Thanks for the great videos. We re considering PRP for an injured knee. Question. I didn’t hear any mention of caffeine but read you should not have any 1-5 days before or after procedure. What are your thoughts ?
Excellent video, and appreciate the detail. I continue to get the "no data" response from doctors I'm seeing (and doctor friends) re: PRP, and it seems by your analysis there is a quite a bit of data. I'm considering it for a pesky, ongoing (+8 months) either brachioradialis strain or bicep tendonitis (two docs, two assessments). It continues to be irritated by select weight-lifting and other bicep pulling motions.
Hello Mr Peng ,thanks for the informative video.I got a PRP treatment on my knee for osteoarthritis and a week after it swallowed and got very painful no major incident I just used a strong pain relief cream with camphor ,chondrotine and collagen is that could be the reason of the sudden swallow and strong pain ?I m 10 days from the shot now the swallowing is slowly less so is the pain but still very painful can t walk properly ...will this affect the platelets ? have a good day thank you .
Thanks for your question, these are all "in general" and of course needs to be individualized. There are less studies for ligaments and muscles. We lump them into the tendon category of most of the time needing just 1.

Hi.It seems the prp has helped my knee.I don't have much pain, but the knee /arthritic is so swollen and it is hindering my walking a great deal. What now Dr.Peng? Please advice.
This video is very helpful but the part about preparing for your prp begs a new question. If we rid ourselves of Tylenol, Motrin and eat tons of fruits and vegetables, would that help osteoarthritis in all our joints overall? Or are some joints just stubborn?
Lifestyle has a HUGE impact on osteoarthritis flare ups. But once you have arthritis, modifying your lifestyle will not regrow cartilage and get you a new knee. PRP can help treat the internal environment of the knee to protect the cartilage that is remaining.

This is the first video I've come across that really gets to the questions I have. Thank you for cutting the BS and just getting to the point. One question I do still have though, I have two completely torn ligaments in my left ankle (ATFL and CFL) so my ankle is pretty unstable. I'm running fine on it but I easily roll my ankle on trail runs even with a brace. This happened in Feb 2019. Does the 3.5 year gap matter or would I still be a candidate for this treatment? Thanks!
Unfortunately PRP has not been studied for ligament tears. We do it a lot for UCL problems in the elbow, but results have been mixed. Using PRP for ankle ligament issues would still be considered highly experimental, regardless of when the injury occurred. Best to discuss with your health care provider. Good luck!

I just can't afford to get it enough. It's $1000/six-hour session. Soon I hope to be able to, as I could before: about once every three months. It's currently been a year.
+1 vote for this comment "Could you please make a comparison between stem cell and PRP for back pain?"
Thank you so much for such a complete comprehensive technique and PRP injections. It answers to all my questions.
Thanks for taking the time to comment, glad it was helpful!
If person has early stage of chondramalacia patellaIs if she takes 3 injection shot so is it one time treatment? or it has to be repeated every year ?@@JeffreyPengMD
Had PRP for knee OA. All pain is gone. Hope it lasts.
So far did it work good ?
How long it took you to have good results?
Yes it has great benefits
I had injections in rotator cuff tear .Extremely painfull and lasted for a good 48 hrs.Should of had a driver .Couldn’t move my arm for a few days as well .Praying it works.
Good luck!
Well... how's it working for you?
Thank you for your explanation
Wish I had seen this video before my prp injection, it was SO painful no local was used. I have a 50% tear in my patellar tendon so hopefully it works, had it 2 days ago.
Local anesthetics such as lidocaine has been shown to damage platelets. However, ropivacaine, is much less damaging to platelets and a small amount can be used to make these procedures much more tolerable!
How is your injury now? How long did it take to see any improvement?
@@JeffreyPengMD My PRP ortho sports medical doctor was going to use lidocaine for me and I specially requested ropivaciane after reading Dr Peng's view on local anesthetics and he said he actually preferred ropivacaine too.. Thanks Dr Peng.
This is new to you but my doctor has been doing it for 10 years
10 years is very new in medicine!! For whatever reason (of which there are many), it takes incredible effort to get new things approved in health care. Thanks for your comment.
Great information..thanks....I have taken two prp sessions combined with dextrose injections.for my rotator cuff shoulder partial tear..in india...going to have third and last session after two days....i can still feel the pain but hopeful to get better after few more wks ...
Good luck!
Has it helped
Hi dr Peng how long we do rest after PRP and should we have to get bed rest for two weeks and what's the reason behind it?
Thank you Dr. Peng! 👍😊
You're welcome 😊
I am a 53 year old with severe OA in my subcalcaneal and knee joint. I had 2 PRP in the subcalcaneal joint, 1 PRP on the left knee, and 2 PRP on my right. I have been pain free for 5 years now. Just wish I could find a clinic to inject PRP into my hip. My current regenerative clinic does not have ultrasound or fluoro guidance imaging.
How long it took to see results? I’m 2 week of my first Prp I only some improvements
@@Stewmade64 im on my 3rd week of my first Prp injection. I will do that exercise thanks brother 🙏
@@Cybernetic263 wait until you are cleared for exercises from your doctor. I think I was cleared after 6 weeks, but ask your doctor, especially if you don’t have a follow-up appointment scheduled.
My orthopedic specialist advised me that PRP may lend up having bacterial infection and that will worsen the matter as one will need operations to remove the bacteria infested knee. Moreover he said if this happens it will worsen the knee condition. He advised not to do PRP but have knee replacement. Please advise doctor
The related therapy rekindles the hope of a pathbreaking technology to a lasting relief from a crippling health condition plagueing a very large no. of populafion. All glory to Dr. Zeng! Is this facility available in India?
PRP is most definitely available in India
Thank you so much ...very useful information
You're welcome! Glad you found it useful
Dear Dr Peng. Would you rather suggest activated or non-activated PRP for osteoarthritis? What is the expected consequence if activated, resp. non-activated PRP is used? Thank you so much for your videos.
Thank You.
Very informative thanks
Glad it was helpful!
How much is PRP for OA,n where is your clinic n how much per knee.
What’s should be the minimum interval between two PRP injections for knee osteoarthritis?
Thank you Dr, 🇮🇩
Great video. Thanks a lot
Glad you liked it!
Does PRP have effect on meniscus tear and degeneration? How many shots is recommended by what timeline?
Great information. 4 years ago I had 3 doctors who told me I needed surgery for the rotatory cuff. I found and alternative Bio energetic Dr. I had various injections (7 sessions) PRP injections for my shoulder and I'm 90 % cured. Now I have a meniscus tear in my right knee. I have app with another Dr that also uses PRP. (the previous had retired)
I hope I will have the same results as my shoulder. I know at least 5 people that had knee arthroscopy surgery and was not successful. They are in pain and still going to physiotherapy. Thanks Dr Peng I just subscribe to your excelent videos.
Thanks for sharing your experiences, good luck!
Hi,
I have the same problem as yours and did prp injection for meniscus tear in knee. Could you please share your experience and result in knee prp? Thanks
To be fair, there is not much data for PRP specifically for what you're describing, possibly distal biceps tendinopathy
This is something that needs to be discussed with your healthcare provider. Good luck!
Thank you...

What are the results for PRP-injections in the Flexor Hallucis Longus tendon in the foot?
Did one for foot pain. Omg almost puked the injection was so painful!
Dr. Peng i have a question ❓ can I physical therapist give PrP therapy?
Thank you. You said one might need 3 injections into a joint and 1 into tendons to feel results. What about injections into ligaments and/or muscles?
Thanks for your question, these are all "in general" and of course needs to be individualized. There are less studies for ligaments and muscles. We lump them into the tendon category of most of the time needing just 1.
Thanks for the content , after Prp injection in my knee , it becomes swollen , with pain 12 hours after injection which lasts for three days , also i felt sometimes locking in my knees don’t know why !! May be because of the volume !!
Anyway after the fourth day my knee movement returns back to normal without locking .
Any explanation for these symptoms !!
Is it indication that the injection in right place ??
Can it help for meniscus tear ?
Theoretically it has huge amount of growth factors , can this induce cartilage or chondrocyte to regenerate the cartilage ??
Thanks
Excellent video. Detailed information is given. Thank you. My wife is taking supplements like nutrajoint ( Rose hip extract, boswell sarrata and vitami C) for knee arthitis, immunosuppresants (Azaron Azathiooprine 25mg) for vasculitis , diabetic medicines (vildagliptin ) and diamicron and urotone ( Bethanochol chloride) for urinary bladder weakness plus cacicum , vitamin d3 and multi vitamins. Which of these she should stop before prp? Please reply.
Could you please make a comparison between stem cell and PRP for back pain? If you have already done that, could you please share the link?
AGREED. My thoughts exactly.
Painless my A$$ !!!! lol i’ve had epidural injections without anesthesia. I’ve had Cortizone injections in my shoulders, wrist and knees this PRP injection to the shoulder has been by far the most painful injection I’ve ever endure.
If you had a patient with both knees with arthritis/ started off as a meniscus injury (over use, jogging) would you encourage that patient, or discourage that patient from having both knees done at the same time? New subscriber, looking into this procedure. Thank you
Do you also perform alar ligament injections through the mouth for CCI patients or is it too complicated for you doctor?
I was diagnosed with osteoarthritis caused by hip dysplasia. The pain has rapidly increased day by day. I really need to buy some time before I have my THR since my wife is 6 months pregnant. Can PRP injection delay the day I need to have THR?
Hello Doctor.. Four days ago, I underwent plasma prp in my shoulder because I suffer from inflammation, calcification, and an occasional tear in the tendon. Now the pain has improved greatly, but I cannot move my arm. Should I try to move it and do a light exercise for it to regain its movement, or should I wait another period? Please answer. Thank you.
Hello. I see you comment this 11 months ago.i'm intrested how are you now. Is your sholder free from pain and can you do everything with it?
Hi.It seems the prp has helped my knee.I don't have much pain, but the knee /arthritic is so swollen and it is hindering my walking a great deal. What now Dr.Peng? Please advice.
Can PRP injections work for neck injuries?
Thank you for this information Dr. Peng. Do you have any guidance on single spin vs. Double Spin PRP methods?
do u do platelet lysate at ur office? Its the better than PRP i heard
Does PRP help with protrusion in lumbar disc and cervical stenosis?
This video is very helpful but the part about preparing for your prp begs a new question. If we rid ourselves of Tylenol, Motrin and eat tons of fruits and vegetables, would that help osteoarthritis in all our joints overall? Or are some joints just stubborn?
Lifestyle has a HUGE impact on osteoarthritis flare ups. But once you have arthritis, modifying your lifestyle will not regrow cartilage and get you a new knee. PRP can help treat the internal environment of the knee to protect the cartilage that is remaining.
Do ligaments in the cervical neck generally respond well to PRP injections?
Is it a good idea to eat beef liver before PRP injection for knee?
my ortho suggested prp for my shoulder, I have bursitis and 2 small tears. tried cortisone injection but did nothing other than ruin my diabetes control for a while
Sorry to hear about the pain. PRP for shoulder pain is definitely an option if cortisone and physical therapy / home exercise program doesn't help!

Yup
This Thursday is 4 weeks since my prp to shoulder and knees. Can I ice or heat? Also can I start hiking inclines?
Would love to know where this is done in Florida? Thanks for this info
I just had this therapy in Jacksonville, FL by the wonderful Dr. John Anderson, a foot and ankle orthopedic doctor. Because UF does a large number of these treatments, the cost was only $500 vs $1500 at other facilities.
I had PRP for hip arthritis and small labrum tear. Doctor said my platelets were very high. Procedure worked but it has only lasted for 4 months and my pain is returning. Im not overweight, eat well, exercise. How long does PRP last? Seems like I paid a lot of money for short term comfort.
This is something that needs to be discussed with your health care provider, good luck!
Wow I wish I knew all the things I should’ve done before getting PRP. My doctor did not let me know of any of that. I had a Tylenol the day before for period cramps. 😪
In theory, Tylenol should be totally fine! You'd want to avoid NSAIDs.
Dr. Thanks so much for all the info . My question is do they add any synthetic things to the blood before they inject it or it’s all my blood ?
TIA

Hi , Thankyou for your video , very helpful . I,ve had a few prp injections for my knee now over the past 2 years and all were a nice yellow colour but my last one was Rose red in colour , my doctor said it is ok but Now i,m a bit curious , is the Red PRP as good as the Yellow ?
Hi. My PRP was also a bit pinkish and the doctor said it was okay. How are you now?
Thank you for such an informative video.

Thanks a lot Dr Peng for this excellent information. I want to know that for stage 3 knee osteoarthritis Hyaluronic acid injection is better or PRP?
I’m afraid of all the invasive procedures on my knees. Kindly reply.

PRP seems to be better than HA for osteoarthritis
Good luck!
@@JeffreyPengMD thanks for the quick reply. I'll book a consult locally
@@JeffreyPengMD I'm having a consultation with a specialist regarding PRP or other injections for relief in my elbow where I had a UCL tear a year ago. After the initial tear, 3 months of physical therapy helped me regain use of my arm, and then a few months after that it felt pretty good. But its been degrading a bit the last few months. Are there alternatives to PRP for the UCL for pain relief and recovery?
Can you tell me if this would helpful of Ischial Bursitis? I have it and nothing else has helped including steroid shots.
Do you need to stop anti-inflammatory supplements, such as fish, oil and turmeric before and after the procedure?
Not typically. NSAIDs need to be stopped because their mechanism of action is to inhibit platelets.
After listening to your video Iam going to focus on healthy eating and drinking water plus cardio vascular exercises before I fix an appointment for stem cell therapy and prp. Being said that how long should we do this before having the prp/ stem cell therapy to get ideal or best results ??
There are no studies answering your question "how long to make changes before starting a biologic treatment". With that said, what is certain is that healthy lifestyle changes are things you'll want to continue for the rest of your life!
great👌👌👌
My resting heart rate shot up ever since prp ,not happy .
Thanks for this and other video. Dr Penguin, do you know why insurance is still no covering the cost of PRP injection if there are data showing benefit ? I have knee pain after ACL and menisus operations 25 and 5 years ago. So am scheduling to do PRP injection shortly. Thanks again.
Dr Peng are there any current trial studies where I can participate w/o the great expense of these procedures ?
Wonderful video sir..
This is very helpful for me
I have one doubt sir.. may i take alcohol after prp within 2hours
Alcohol should not affect PRP, though please discuss your specific situation with your health care provider. Thanks for your comment.
Can I have PRP in my spine and left hip?
Does PRP help with healing broken bones?
celebrex doesn't affect thromboxane a2, resulting in sparing platelets.
