Then you forget about blood tests, and go straight to imaging. To diagnose DVT per se, you can use Venography, CT or MR of the vein. To diagnose PE, you can use imaging modalities such as CT scan, lung scan (V/Q scan) or Pulmonary Angiography. Note, if venous ultrasound is positive, you go ahead and treat for PE directly. Interpretate as follows; score above 4 means that PE is THAT likely, you'll still need to do CTA even if D-dimers would've been negative. --> you need to prove PE or ultimately exclude, since you probably need to treat it. score below 4 means that you are allowed to exclude PE by laboratory means and U/S. --> you need to exclude PE before abandoning treatment.
Sure.. actually if D-dimers are elevated 1. Helical CT (3 possible results) *High resolution- look into other ddx *Indeterminant-pulm. Angio or v/q scan *Confirming PE-treat
