I think little bit updations needed to what u have said..... less than equal to 4 its highly unlikely and only greater than 4 or we can say if score is 5 onwards then it is higly likely. And the 1.5 criteria has 3 bullet points ( like u mentioned as four bullet points under the heading TIPS) instead of 4 bullet points.
You are right Shivam. They usually mention 3 points as they mention Surgery or Immobilization in a single point. So if either of these is present then it has 1.5 points each.
