    The end of this video gets me because I don't know what to do anymore. I wish dr. Rudnicka could see me herself. I've been to more than 10 doctors, 05 being 'specialists' who sent me back home telling me 'there's not patterns, there's nothing wrong with your hair'. No one will close a diagnosis so won't let me try a DHT blocker. I've been losing hair globally in my entire scalp, especially sides and even on the back, however this has been happening for years (progressively) and not in clumps, so it's not alopecia areata or telogen effluvium. I believe genetic hairloss is the only possibility left. I feel neglected and like I've wasted too much time already, losing ground.

    i watched this video again. can u please do a video on the difference between oral spironolactone versus oral finasteride in adrogenetic alopecia in women? and if spironolactone affects every part of body and if finasteride affects on;y hair follicles or breast tissue as well?

    Thank you got an informative video. I didn’t realise that type 3 isoform of 5-alpha reductase was present in significant levels in hair follicles (always thought that it’s preferred substrate would be androstenedione instead of testosterone. I also thought both finasteride and dutasteride were quite potent inhibitors of type 3 anyway, so in my mind it’s always been finasteride against type 2, and dutasteride against type 1 and 2 as far as AGA goes. So I’ve learned something new.

    Hey doc, i would love a video talking about how these medications affects the hair cycle, because when you are suffering hair miniaturization the sync of how this happens on each hair is different, so when using these types of dht blockers these miniaturized hairs are putten into the anagen phase for longer time. The question is i've seen people saying that their hair starts synchronizing again and they start experiencing a "seasonal shedding" that makes their hair looks thinner before getting thicker again, especially close at one year and two year mark after starting these medications. Would love to know what you think of this topic :). Thank you!

      This may be the case with another medication, not finasteride

    Dobry wieczór, dzisiaj natrafiłem na kanał Pani profesor i muszę tak ja przedmówcy serdecznie podziękować za cenne materiały jakie Pani tworzy. Ze względu na to że problem AGA dot. mnie osobiście i mojej narzeczonej (narzeczona w trakcie diagnozy) to interesuję się tematem dosyć intensywnie jak na amatora, ale nadal mam jeszcze zagwozdki o które chciałbym zapytać. Obecnie przyjmuję finasteryd w dawce 1,25mg dziennie z w miarę dobrymi efektami, miałem epizody w których próbowałem stosować na skórę głowy minoxidil, ale z mało zauważalnym efektem. Zainteresowałem się fragmentem w którym wspomniała Pani o doustnym minoxidilu który wydaje mi się wygodniejszą formą do regularnego stosowania. Czy minoxidil doustnie stosuje się także u kobiet? I czy w polskich aptekach minoxidil doustny jest dostępny, ew. pod jaką nazwą? Pozdrawiam serdecznie

    Excellent post, thank you. Can you suggest appropriate dosage for using finasteride in women? My doctor has prescribed 1 x 5mg tablet per WEEK (not day).

      That's correct. He's having you take it just once a week instead of daily. I mean you can cut your pill into 5 pieces and take it 5 times a week.

      It is frequently used by women with female pattern hair loss as a DHT blocker. @@mirasingh7428

      Thank you. As per available data there are no time limits. Dutasteride 3/week was discussed because of the longer half-life but there are no studies about efficacy of this type of dosing

      The chances of side effects are extremely low. Why don’t you just try it everyday and see how that goes?

    Maam.. what’s your opinion on bicalutamide for acne and hair loss? I am 41 and female. I was on finasteride 5 mg since 6 years and was doing fine. But last few months I was noticing that my scalp had again started showing. So my doctor switched me to bicalutamide 25 mg daily. But ma’am after 2 months of taking it, I feel my acne is only getting worse. For hair I know that it will take time. What should i do?

      No, this is not a usual treatment regimen, because both drugs have a similar mechanism of action

    Szanowna Pani Profesor. Po pierwsze chce przekazać Pani wyrazy uznania, podziwiam ludzi takich jak Pani-którzy cały czas rozwijają naukę i siebie, są zawsze głodni wiedzy! Mam takie pytanie, czy poleca Pani Profesor jakiego dermatologa w Krakowie-najlepiej takiego szczególnie zainteresowanego chorobami włosów i skory głowy-dokładnie mam AGA. Bardzo mi zależy żeby kierował się EBM. Może zna Pani takiego… będę bardzo wdzięczy za odpowiedz. Pozdrawiam i dziękuje
    Ps. To prawda że mezoterapia dutasterydem i osoczem ma swoje oparcie w EBM (w AGA)?

      Dziękuję bardzo. Dutasteryd doogniskowy ma pozytywne opinie wielu ekspertów, ale jest w minimalnym stopniu EBM. Przede wszystkim nie jest zarejestrowany jako lek, więc nie przeszedł niezbednych dla leków kontroli. Dlatego nie ma go w tym wideo. W Krakowie jest wielu doskonałych dermatologów i jestem pewna że wszyscy lub prawie wszyscy zajmują się AGA.

    hi doc, is the result of hormone DHEAS a definitive result for androgenic alopeacia, my result is normal but I still lose hair from the crown area a lot. P.S. I'm female.

      I am not sure that I understand the question. If the question is whether hair loss is reversible, the issue is complex, but it may be reversible

    I am a patient of OCD and taking psychotic medicine last 6 years. My main concern is my hair fall started when I was 14. Now I am 30years old. Totally bald. Probably Norwood 7. But the problem is I have losing hair from lower and side portion of the head. Maybe retrograde alopecia. And also losing beard.almost 70% of my beard density has gone and losing my beard line. I saw many dermatologist but nothing works. I just want to know is it treatable?

      You definitely need a diagnosis. There are at least few conditions which cause this type of hair loss

    Could you please discuss what types of hair loss are associated with both head and pubic hair loss? I’ve been shedding for a year ( 25 yr old girl) and now my pubic hairs are falling :(

      Thank you. This is complex, especially with no details. In a young person the most probable case may be alopecia areata, whereas in a 40+ woman it may be for example lichen planopilaris (Grham Little type). Some skin diseases may be associated with diffuse hair loss....

    Hey doc , i have alopecia and i used minoxidil and finasteride for one year 8month and the finasteride now is Cut off from the market switched to dutasteride injections every 10 days i going to clinic for injection for 6 sessions
    -May question now my plan its good or no ,because i stop the finasteride and make duta sessions and how many time to back my hier its good ?

    Pani Profesor proszę o poradę - przyjmuję finasteryd 1mg doustnie a poziom DHT nie dość, że się nie zmniejszył po kilku miesiącach, to jest większy (ok 1500). Dlaczego, co robić?!

      I suggest to analyse with the endocrinologist the reason for increased testosteron and whether it requires endocrinologic treatment

    I saw a video that states the since Dutasteride is over 500 Dalton it doesn't cross into the blood from the scalp if applied topically. But Finasteride is less than 500 Dalton and does cross. Even if both drugs causes and is systemically absorbed, it is still way less (> 10X) absorption than the oral forms correct?

      Partly yes, but any skin changes (scraches, folliculitis etc) on the scalp, even minor, may alter the penetration

    What is your experience regarding
    Intralesional dutasteride in AGA in the form of nesotherapy
    And topical spironolactone in AGA ( I have previous published study with good and significant results :
    Ammar AM, Elshahid AR, Abdel-Dayem HA, Mohamed AA, Elsaie ML. Dermoscopic evaluation of the efficacy of combination of topical spironolactone 5% and minoxidil 5% solutions in the treatment of androgenetic alopecia: A cross sectional-comparative study.       Thank you, it is always such a pleasure to have your feedback! Intralesional dutasteride has good opinions and some literature data , but it is not approved as a drug with all consequences, so I prefer to wait for a more validated product. Regarding spironolactone - you are the master, so I can only repeat after you "oth spironolactone as a 5% topical solution and minoxidil as a 5% topical solution might be used safely in a twice-daily dosage to treat AGA". 💕
