Recently suggested cutoff numbers are shown below but they are not formal diagnostic criteria (Gastroenterol Clin North Am 2014;43:257): Stomach: ≥ 30 eosinophils/high power field in 5 high power fields Duodenum: > 52 eosinophils/high power field Ileum: > 56/high power field in the ileum Right colon: > 100/high power field Transverse and descending colon: > 84/high power field Rectosigmoid colon: > 64/high power field I find that in the stomach, even a few eos (say greater than 10/hpf) is decidedly abnormal, though the causes for mild increases can be varied.
I had a couple questions if that's alright: Is it possible to have a low number of eosinophils on a blood test but still have eosinophilic esophagitis / gastroenteritis? And would treatments like corticosteroids affect the results of an endoscopy done to detect the disease?
Peripheral blood eosinophilia is not always present with these tissue eosinophilic disorders and yes, it would be possible for steroid therapy to affect the detection of the disease, but I would think only if therapy had been given for more than a few days or weeks.
Is Eos 5 high for a 9 yr old girl ? My daughter has had asthma for years and is now having a lot of GI problema but her pediatrician doesn’t know what’s causing them . A friend of mine told me that Eos 5 is high for her age but pediatrician didn’t mention anything about it .
Normal ranges depend on sampling enough normal individuals in the area, age group and demographics that one deals with. It also depends on the methodology used to measure. Published ranges are often used as performing a normal range study is very time-consuming. So some normal published ranges for pediatric patients place 4% as the upper limit of normal, or more preferable, an absolute count of less than 500 cell/mcL. But even then, a "normal range" may exclude up to 5% of normal people whose values normally fall outside that range, i.e. they aren't sick. Determining whether your daughter's count is indicative of disease is up to your pediatrician in light of both lab and clinical findings.

Symptoms can vary considerably due to many factors. But the diagnosis is more firm if the tissue biopsy is well beyond the eosinophils that are present in low numbers in some areas of the bowel.

Yes, good question. In general, any significant number of eosinophils in a mid- or proximal esophageal biopsy should be sufficient if other causes are excluded. We do use the threshold of 15/hpf in a mid or upper esophageal biopsy, per the consensus recommendations of Liacouras et al. J Allergy Clin Immunol 2011 Jul; 128:3-20. But at other sites in the GI tract, there are not such consensus guidelines. And the numbers may vary widely based on the antigen, timing of exposure, depth of involvement and other factors.

I'm sorry to hear this; it sounds like you need a good gastroenterologist, or perhaps a rheumatologist, with an interest in these sorts of immune-related diseases to help you manage this. There are newer treatments becoming available, hence the need for attention to the diagnosis. Perhaps you would find the right doctor at an academic institution with a wider array of medical providers.

Is there a certain cut off for eosinophil numbers/hpf for eosinophilic gastritis or eosinophilic enteritis? (as you know for EOE it's 15/hpf)

I have been diagnosed with EGD but I have doubt on my diagnosis as I don’t have any diarrhoea, I was only having upper abdominal cramps

Not necessarily. Eosinophils are attracted into tissues by specific factors but mast cell lesions do not usually secrete those.
