    Great info. Additional consideration for stroke: CTA head/neck to follow up initial head CT in the absence of a bleed. This is to diagnose emergent large vessel occlusion (ELVO) for possible mechanical thrombectomy to be done by neurointerventionalist (if available) via Interventional Radiology. Can be done in combination with rTPA/TNK.

    For stroke, what do you think about the VAN assessment for Large Vessel Occlusion? Helpful for EMS in rural setting?

    Man i am struggling e the content, i dont seem to memorize it no matter how i try, i memorized the cardiac drugs eazy peazy but this algorisms dont seem to be much consequal relation to me.. and we dont give fibrinolytic in ER so why the heck would i do fibronolytic checklist in the ER ..

    Thank you for your help! I’m studying for ACLS now that I am an outpatient recovery nurse. It’s so nice to have adjunct learning tools to the ACLS book for those us who have been out of school for some time.

    Don’t give BB immediately after diagnosing stemi, studies showing benefit are from before the reperfusion era. Modern studies show essentially no benefit and increased risk. They need to be started within 24hrs but not in the acute management.

    Question... How do you give TPA ?
    IV push or infusion

      Infusion. It’s weight based. After you calculate how much you’re going to give over one hour you waste what you will not use from the bottle, give the bolus through the infusion, and then administer the drip over an hour

      @@nicolem4673 i saw some videos giving 10 percent of the tpa dose bolus and then the rest infusion. what do you advise?

      @@alaahussain9677 yes so 10 % of the total treatment dose is the bolus. then the rest of the infusion over 1 hour.

      Well, it really depends. If it is during a code for PE, we give it push, but can be an infusion. We do infusions for other reasons such as stroke and ACS.

    just found you, am prepping for ACLS. I work in a Critical Access hospital that has a small ICU that does not have critical patients a lot. So this is a great site for knowledge.
