It is unclear what exactly it is about, but there are several versions:
•It is literally about a man nicknamed Cotton Eyed Joe who stole the main character’s gf and left. Cotton-eyed Joe could be a black man with very light blue eyes, an alcoholic or a man with a cataract.
•”Cotton-eyed” could be due to alcoholism, since it was broadly used at the time to refer to drunk people. The main character is an alcoholic and thus unable to get any pussy.
•Syphilis.
The STDs theory is considered to be the least likely one though and the one about stealing girlfriends the most likely one. One of the earliest published versions literally refers to Joe using a variation of the N word, so it is highly unlikely that he is a metaphor for a disease.

Ah this takes me back to when I was an apprentice DJ for my grandfather before he passed. 4-H crew loved this song so much. They had this one officer (Part of the mounted patrol for the fair, and the 4-H dance was at the end of the night when they were off-duty) who was just all-in with the song.
