  • How did this happen? Ketamine, previously only known as a club drug, is now known as a new method for treating severe depression. Not only that, but it is seen as potentially being the most significant breakthrough in treating depression in decades. Dr. Steve Levine, Founder of Actify Neurotherapies, explains how this transformation has happened, how ketamine works in the brain, its impact, and the future of this treatment. He also shares a couple case studies that illustrate why people are asking for this treatment and why Actify Neurotherapies has now grown to 9 locations in 8 states.
Check out our shorter Spaces segment with Dr. Levine here: • Ketamine Treatment for...
    ___
In this deep dive, you will learn …
    Introduction and background (0:18)
    - Created Actify Neurotherapies in 2011
    - Had a traditional training in psychology
    - Started as a psychiatrist and grew frustrated with his prescription options
His discovery of ketamine as a treatment option (2:43)
    - Saw a patient who had tried everything to treat depression and found that her mood was enhanced only with medicines that contained dextromethorphan
    - Searched for research-based evidence that it helps depression
    - At the same time, research around Ketamine was developing and demonstrating its efficacy
    - He consulted with anesthesiologists to see if it was dangerous and then started a program at a hospital, which then expanded to multiple locations
    How does ketamine work (8:46)
    - It goes a lot of places and does a lot of things which is different than the throwing a dart approach psychiatrists take in prescribing other drugs
    - Broad spray pattern means it is more likely to hit the right pathway
    - Binds to a receptor, produces BDNF, heals the brain and allows it to learn new things in a more efficient way
    - Great for cognitive behavioral therapy
    - Has anti-inflammatory and immune-modulating properties
    - Has activity at classical receptor targets of serotonin, norepinephrine, dopamine
What can a patient expect (13:52)
    - Full consultation before determining this is the right approach
    - 40- minute IV, followed by 40-45 minutes of recovery, then a meeting with the doctor
    - Three treatments in the first week, two in the second, once per week for the next few weeks, then waiting two weeks between treatments, then three, then hopefully a month
    - Some feeling patients feel: intoxication, floaty sensation, mind being disconnected from body, enhancement of perception, heightened senses
    The impacts of treatment (18:56)
    - The language people use to describe their experiences
    - How their depression has changed
    - Disconnection from the trauma, which is the goal
    - Does not work for everyone, but many have a profound experience that changes them
    - Brain is more receptive to new learnings
    Case studies (25:36)
    - Successful radio DJ from Denver
    - Family whose son was treated
The future of this treatment (30:20)
    - Current perception vs potential future perception
    - Forcing innovation and a movement back to neurosciences
    - Additional implications for ketamine
    Exam room and process (36:29)
    - Chronological explanation of the process

8 treatments in (6 initial and 2 maintenance) and to say my life is right side up again is an understatement. 34 years as a city firefighter and paramedic has slowly been filling me with grief. Like boiling a frog I didn’t see what a toll this was having on my family and me. I was sad, angry then guilty about my outbursts and this turned into seeing the only way forward was an end. Suicide. Ketamine Treatment Center of Denver (I’m not getting paid) saved my life! Everyday I am feeling better and thinking more clearly. The continued research of this actually “healing” the damage with the glutamate is just a tiny glimpse into this. Is it a miracle? For me loving to be back to work helping people and retiring soon, yep that is a miracle.

Welcome Karen I have been doing treatments since June 2019 and I just want to say the miracles have come and still coming. I was suicide or ketamine I am one who antidepressants did not work and I started on Prozac when in it was the answer. They help some but for me they didnt PTSD and Major Depression Disorder. And recovering from suicide. So it gets better it does. I still have my days but no suicidal ideations with a plan since i started I am on maintenance now. It is great to have the horrible pain of this depression lifted after every treatment i still am getting better.

Eileen Doherty I’m pressed for time I can write more later but I’ll tell you for now and short that as far as depression goes I had a really bad fatigue and it is almost nonexistent. I’m on my sixth treatment of six I just have to schedule my last one when I’m ready... my last “experience” was two days ago..as of right now in this very moment I was just painting my house which let’s put it this way... I started this project a year ago and haven’t felt the motivation to finish until the past couple of weeks...I have to cut this short but my only concern is the longevity of this new state of mind and it is very subtle but anything feels 1 million times better than being completely and utterly miserable

    This is brilliant and exciting!
I have been suffering with depression for over 10 years and 2 years with suicide ideation. We definitely need to turn to new ways of utilizing medicine since we haven't had any big breakthroughs in depression medication in over 10 years. Western science has been turning to plants outside our country to heal people, and also going into the past to see how we can utilize medicine differently. Modern medicine is seeking answers for people who are no longer responding to depression medication. I see a path of hope for our nation.

@ss2878 That doesn't make sense. Pharmaceuticeticals companies want to make money. The SSRI and SNRI bubble is stagnanted now.
Anyone entering the market with a new treatment for depression will be like when Prozac came out back in the 80s. It made Lilly a fortune and had five years all to its own and then they came out with a SNRI that made them even more money. This is how they keep things going. Once a generic version is out that is really the endo of the big cash. You can get Prozac now at prices less than aspirin. So, there is no insentive for the major pharmaceutical to bang on it. There are only tiny minority who brlieve that the brand name if more effective and given the fact Prozac is only effective partially even when it is, your analysis doesn't make sense.
The formulation for Sketamin will soon lose its exclusivity. If it were really effective, it would be made accessible. I think that it isn't as effective as it is made out to be. You can tell from the differences between the initial reports and later reports.
Unless you know exactly the pharmokinatic of a CNS drug the outcome is unpredictable, even if you have calm surrounding and so on.
These are really powerful drogs. This is why you have aneathetist present for Ketamin's primarly use. This is going to be that same with Psychedelics. They will either have no effect or suddenly have and extreme effect.

I can't get it, because in England it's only available to rich people, at £200 per infusion.
Fortunately I can treat myself with ephenidine, which does the same job.
The reason why ketamine is available as treatment, is because of a person who reported to their psychiatrist, that PCP helped their depression. That psychiatrist decided to try using ketamine to treat depression, as it works the same as PCP.
That was a brave psychiatrist, as most doctors don't listen to their patients, and believe that illegal drugs don't help people, and shouldn't be used.

I was surprised because I expected to feel tired and want to go home and to bed. Instead, I went to lunch with a friend! I felt better than I had in years! It lasted for few days. Later, I told my dr that was the best nap that I’ve ever had and asked what they gave me and he said propofol! I realized how Michael Jackson got hooked. I've never done ketamine in a therapudic setting though ive tried it a couple times, but ive used memantine for this purpose and its been life changing, i feel so happy and optimistic about life even after its warn off, im more productive, patient, and im happy to be around people im close with. I dont have brain fog anymore and my restless leg syndrome is basically gone now. its been amazing, im so glad doctors are opening up to this type of treatment because this type of therapy forces you to tackle your problems head on, you find out why you have a certain issue, and you learn how to fix it, its really emotional and eye opening. I cant wait till this becomes more mainstream as i think this would really help a lot of people. I've actually tried dextromethorphan too and i find it really does help when used responsibly though memantine does a little better for me. Just dont go dosing to high with dextromethorphan because it can fuck you up long term.

  • @marywilliams5262
    @marywilliams5262 2 года назад

It feels like a magic bullet to me. My depression was more grief related and I have never sought help before. Talk therapy frustrates me because intellectually...I know what I am feeling does not make sense. I am loved and I have a wonderful life but after taking care of my father during his cancer and then caring for my mother after his death wore me down. I suffered a birth injury with my fourth child and am grateful he is alive but it has been difficult. Fighting with the school to provide him a free and appropriate education that involves technology was like pulling teeth just twenty years ago so I think I had a lot of "weight" that I was pushing to the back. I am a strong person and believe in picking up my pack and marching but by this point there were so many bricks in my pack that I was not able to pick it up and march any longer. The first dose did not do much and the second dose was increased a bit the next month. I am two weeks out of my second treatment and I feel like a new person. I feel the way I should have felt all along and I recommend Ketamine for anyone and everyone because a lot of people do not realize they are carrying around depression and can feel differently. How would a person know if they have always felt a little heavy? It builds so slowly like the frog who will allow the water to boil him to death because it is a gradual increase in temperature. I don't mind the feeling and believe the deeper you go the better. However, it isn't something I would look forward to doing or seek out now that I am starting to feel better. People abuse substances of all kinds as a way to self-medicate and the Ketamine removes things you did not know were there but are so glad they are gone. I would put this in funeral homes, for example, because the medical community needs to move away from the Puritanical bias that things that feel good are bad etc. I recommend it. I would not want it to be something I used on my own or at home. I would try this first if I was treating depression! The medications took months and I wish I had that time back.

Look into DXM. Do a bit of reading first, though.
Johns Hopkins is the team behind esketamine, I think. They've also tried a few dxm dosing regimens.
I can vouche on an initial dose of 90 mg. I will not give a frequency, but multiple daily doses might be necessary. It depends on how you metabolize it. 90 mg had visual disturbances for me, so I worked my way up slowly with smaller doses spread out.
You need to be very careful with your doses! There's no official dosing regimen.
And remember, this is a psychoactive substance.
Highly elevated levels of glutamate can mimic schizophrenia, so it's important you stay within low doses.
There's no need to take big doses. This isn't lsd, which helps people conquer fears. This is dxm increasing glutamate levels and boosting serotonin slowly but immediately! So it does work rapidly, but you need to find a stable dose. That comes with time after working your way up slowly.
I have found this helpful more than any stupid Big Pharma med.
Just be responsible. There's visual disturbances and other issues you might stumble across.
They go away with tolerance, which is why you need to slowly work your way up. It's like any other med we use.
Show it respect and you'll find great relief!
I buy from RoboCough. Their tablets didn't work for me, so I only use their syrup. Seems I don't metabolize their tablets properly or something. It actually felt like a different drug.
There's so much to talk about. There's something called neuro-genesis, which apparently, these dissociative anesthetics cause.
That means there might be permanent effects after prolonged use, I think. I don't get paid to know this, but I've done actual reading regarding the science behind neurochemistry, if that's what it's called.
Wish you all the best. And please be safe and responsible.

So, I've been taking it at home orally and, yes, I have noticed I have felt better, and my husband has noticed too. It has not been dramatic, but as you have probably found throughout the 30+ years, any improvement is wonderful (and "dramatic" is relative when you get deeply depressed and begin to feel better). When you take it at home, you are required to have a "sitter" who stays nearby and checks on you throughout. (In my case, because I have high blood pressure, my husband takes my BP every 30 minutes.) I've only told two friends so far what I've been doing because I wanted to see first what it was like, but I am going to be open about it now in case someone else decides, after his or her own thorough research, it might be worth a try. SSRIs have never done anything for me At all. Since my early 20s. Not even close. I have gained a lot of weight on different antidepressants. Then gave up. I was diagnosed with bipolar in my early thirties. And took lithium which I did not tolerate. And the. Later lamictal. Which I also hated. 😅. I’ve had many years of therapy. Which is helpful to some degree, but The only thing that keeps me decently functional is STRONG and regimented, involved lifestyle routines. Sleep and light and lots of exercise. Also pretty dedicated meditation. Daily. It’s actually quite time consuming. And when I get sick like with a virus, or have to be less active for some other reason, it all falls apart. Quickly. Considering trying ketamine with all this. To get better and more sustainable results. All this mental illness, but with bright spots inbetween. A damaged and imperfect but supportive marriage. A lovely 8 year old son. A business brand I grew with a partner and then sold successfully. Degrees.different careers. 🤣. I’m in my 40s and am looking forward to some consistency maybe. One day.

Every time I did a therapeutic dose of K in a comfortable setting, when severely depressed: I was able to hear cracks in my brain, I believe these to be the damaged pathways being re-constructed. Afterwards I would feel really re-birthed because my identity vanished totally, I was able to re-invent myself and not judge myself any longer. Also I felt as if I was thrown around the room, yet knew I was laying completely still - but remained very calm, learning to enjoy the movements as a rollercoaster. However I must say: a few sessions after, something struck a nerve: the material-inner alchemical reality of ketamine, made me doubt on how efficient it is - yet dangerous, as if almost a cheat and some entity (my consciousness) advised me to look into other ways of dealing with guilt, suffering and stress - now I am opening up to meditation after years of judgement. However: I still have issues with institutionalized matters where theory becomes an excuse to induce suffering, like eg. animal sacrifice-culture. Sacrifice things worth sacrificing, immaterial things standing in the way of progression, broader, non-relativistic understanding & symbiotic societal enhancenment, maybe. At the moment I am finishing my Bachelor in the Visual Arts and actual enabled myself to theoretically weigh up against authority figures standing in the way of my personal goals - which are for the benefit of as many people as possible, even those standing in my path.

I hope I can at least get a beta blocker for the panic attacks🤷🏻‍♀️🤦🏻‍♀️. Ketamine is very expensive and not regulated . Good Luck Man✌🏽❤️

Juliette you should. Rent for studio apartment with kitchen , bathroom, usually a balcony etc is just $125-$175. Food is inexpensive. Taxis very cheap if you need one ( $1 usually around town. ). Blood work at a lab is cheap too. .26 mg from 1 mg. I should be done in 3-5 weeks. Then the real healing begins.

Do you have any strong academic material that evidencially break down systemic conformation bias in the ketamine studies?
Also is there any contemporary academic journals I can read to understand more of your points?
I don't have material on this point, because I just thought about it and will research right after this comment, but I'd like to understand the human impact in medical vs abuse application if ketamine vs opioids. From a high level understanding the US has an opioids crisis. It would be interesting to understand the differences and simiairties on the societal impact.
Thanks for commenting :D time to read.
