    So, I had a patient once. I was watching real time while orders kept being deleted and new orders kept being placed. I was about ready to put some calls out because I was so confused as to what was going on. Finally, I turned around and saw the cardiologist. He told me to stop deleting his orders. I told him it wasn't me. Well, lo and behold, the nephrologist was sitting at another computer around the corner and they'd been going back and forth deleting each other's orders. They allegedly then went off to discuss a treatment plan together.

    Good one! I'm a veterinarian, and I attended a lecture about managing patients with both cardiac and renal diseases. We had a cardiologist and an internist with an interest in nephrology (we don't have veterinary nephrologists), and they engaged in some friendly banter. At the end the internist said "and remember, if the heart was really that important, there would be two of them!"

    Had a patient 2 weeks ago (I'm in family med) mention during our medication review that his cardiologist and nephrologist always fight over his diuretics. I mentioned it to my supervising physician and he just said, "yeah, we're staying out of that."

    Jesus! I have been waiting for these skits my whole professional life as a floor nurse! The amount of times I've had a nephrologist storm up to me, and asked, "Who ordered these fluids?!" And I have to tell them, "the hospitalist did", or "the cardiologist did", and watch their lips curl inward . . . cue eye roll . . .

    I had to go to a nephrologist once for BP issues caused by my kidneys. As a biochemist, I know how important the kidneys are to proper fluid balance in the body. When I mentioned this to the nephrologist, he was like "Exactly! Cardiology just doesn't get it." 😂

      I work in the cath lab with cardiologists, among others, it is spot on. I showed one of the electrophysiologists the cardiologist goes to therapy and he about had a stroke from laughing so hard.

    I cannot help but laugh at all of this , for it reminds me of something that happened many years ago.
    Something happened and I was rushed in for emergency medical care.
    No cap ...I was literally dying. Specialists from all departments were brought in to consult.
    Even dentistry got an "invite" to the party.
    Was in shock and vitals were dangerously low with a bp at around 53/16 (yes, it was THAT low).
    Surprisingly , I was quite coherent and conscious enough to understand everything going around me.
    Not a surprise, Cardiology wanted to take charge. I am not exactly sure what they wanted to do , but Nephrology found out and immediately stopped them.
    Cardiology was not pleased at all and wanted to know what their problem was. Said that even if something happened, there was at least a "back up". They said that doing what was necessary to save my life, was absolute paramount. Nephrology asked Cardiology if they even bothered to read my ENTIRE medical history and file...for he would have seen that I had a history of bilateral hydronephrosis of the kidneys.
    Cardiology was a bit stunned when they heard that...and slowly backed away.
    Nephrology was beaming with a "I told you so" look.
    Fortunately for me, they were able to work together and with all the other specialists to provide me with the care , I most desperately needed.
    Took some time, but I made a full physical recovery from all of that.
    Thankful and grateful to all that took care of me, at that time.

      This is so classic of a cardiology move. As a vascular surgeon working closely with cardiologists and nefeologists, I have a couple of good stories too. Jokes aside, cardiologists rarely take into account the other organs or systems, other than the heart.

      Working as an R.N. in Cardiology and Electrophysiology for 20+ years, and then as a Representative for a pacemaker company for 6 years, I realize that I see everything through Cardiology eyes!!! Unfortunately, our bodies are far more than one system, and if we don't recognize our own prejudices, the patient suffers from our blindness.

      @@platedlizard That was their reaction and their colleague from the Otorhinolaryngology Dept. were thinking, when they were first called in.
      They were quite surprised to receive "invitations", based on what I saw and heard.
      Kept on asking and wondering, till they saw the blood was coming out of my mouth.
      They were essentially called in to evaluate and treat the soft tissue damage in my mouth and throat.

    As a non-doctor/med-student, I can't tell you how much I've enjoyed your videos. I found you because of your commencement address at Yale, and since then I think I've actually learned a bit about medicine by watching your videos and then researching so that I get the jokes I missed (though obviously a lot of it still goes over my head).
    I particularly like this video, though, since I was able to bring it up to my cardiologist and ask her what cardiology's beef is with nephrology. Almost instantaneously, we were talking more like people and less like clinician/patient. Just bringing your name up to my cardiologist and my GP seemed to give me some bona fides.
    Anyway, thank you for the videos. They've helped me laugh through quite a bit of stress, as well as learn more about the medical field and a system with which we all eventually deal. At least when I'm arguing with my insurance company, I can think of what Jimothy would think of the whole thing.

    Dr. G possibly missed his calling...these representations of these specialties are so incredibly accurate as to be nearly supernatural. His acting is quite good, skit length, comedy and flow are all superb.This a true polymat, and hope he expands his considerable skills beyond medicine someday if he so chooses!

      Neurosurgeons don’t ‘argue’ with colleagues. They just book the theatre and do the surgery. The Neurology lane is disorders of the nervous system where surgery is not indicated. Pills vs drills

      @@Gnv-q2m yep. Depending on how it's at the clinic you go to they can be separate clinics or they can be one clinic for both but still don't interact until necessary I've never interacted with neurology and may never only with neurosurgery as the patient.

      And the radiologist definitely needs to mention that the patient just had a CT 3 days ago and the ER doc has to say well this pain complaint is different from the pain complaint of 3 days ago. It's a must.

    This is almost completely what I have to referee is a hospitalist when I come on service and a partner of mine has consulted the entire specialty team for a complex patient. Diurese this might be the best line ever. Great work Dr.G.

    Hah! Too funny. I'm glad that when I was in the hospital for 2 weeks in August of 2022 Nephrology and Cardiology got along. My heart was doing a congestive heart failure thing which caused my kidneys *and* liver to start shutting down. They both visited me often. Especially while I was in the ICU.

    My X was a dialysis patient and also had a triple bypass and other heart surgeries. Can't tell you the amount of times in the hospital where these two specialties were at odds with each other. As his advocate - I had to be really on guard and ready to be confrontational everytime they wanted to load up his IV with fluids, order a heart healthy menu that was in conflict with a dialysis compatible menu. The fact he would be admitted as a cardiac patient, was totally lost on the hosp personnel - they didn't hardly evene read the chart enough to even realize his dialysis needs.

    Internal medicine with either orthro or neuro (or really anyone else that isn’t in the realm of primary care). Internal medicine kinda has a thing of “must rule out everything” and “my patient always”.

    I had no idea this was a thing until my mum had a heart attack almost a year ago. She was in the hospital for 2 weeks, there was some sort of balancing act the team had to do between heart & kidneys. She was doing ok but then she died. It was during a lockdown & I didn’t get to be with her as I live in Australia, her in NZ.
    Your channel fascinates me. Thank you x

      This is so late, but as an Aussie - with an understanding of the severity of the lockdowns both here and in NZ (I'm in Victoria) - I can't begin to express my condolences.
      Anyone needing hospital care, or trying to arrange proper care and stay updated 😔, and denied their families physical presence?
      ... the added trauma. 💔

    I was transferring from a general pediatrics house officer position (I’m a PA) where I rotated through all the services like the interns did so that the intern call schedule would be one night in four instead of one in three. I was transferring to the pediatric cardiac ICU. On my last day on call for Nephrology, the nephrologist called me aside and asked “Can you PLEASE get the cardiac surgeons to write for the Nephrology consult BEFORE they write the DNR order?”
    I tried… but I was not entirely successful!

    I care! My mom's new cardiologist just prescribed blood thinners and we didn't let her take them until nephrology approved, but then she told her to get consent from opthalmology, opthalmology from nutrition and nutrition from intern med who told her to ask cardiology again in three months 🙄

      I can totally understand this, both from being a caregiver to someone on dialysis and has heart problems, but also my experience as a patient between the neurologist and the rheumatologist over which medication was damaging my liver.

      @@PhoenixRoseYT not really. medicine evolved so much over the last decades that it's necessary for doctors to choose a speciality and normaly even specialise in an even smaller field within to be able to know everything about at least some illnesses. I am a med student and even in university we know that there's too much to learn.. so we focus on the things that we will need later on in our respective fields. For example I don't want to go into surgical fields, so I focus more on other stuff.

    God I wish I had this when during my prelim year. I would've showed this to my cardiologist and nephrology attending after getting pimped non stop about urine osmoles and where the lesion is in the heart based on the ekg 😂

      Would you mind explaining to me why the nephrologist is always carrying salt in these videos? This recurrent joke keeps going over my head and google has been no help lol i would think nephrology would hate salt? 🤔🤣

      @@Sup-jz2ow because working kidneys should be able to flush out the excess salt no matter the quantity ingested, although it's widely accepted that a low sodium diet (specially in North America) helps to prevent or reduce the onset of Hypertension, so the kinda crazy nephrologist in Dr G's universe doesn't want salt to be demonized...

    He wasted hypertonic saline?! Given that it was a liter and not 500mL it's a compounded hypertonic saline at that. Nephrology, do not request compounded products just to spill them! That takes time, effort, and resources that can go on back order at a moments notice!

    Cards fellow here. This video perfectly depicts the eternal struggle between nephrology and cardiology! Hilarious! I would like to say 2 things if I may: 1) can’t trust a nephrologist with an EKG, everyone would end up in the cath lab and 2) they can always go on dialysis, we need to decongest and decrease afterload.

      It's literally just concentration of a solution expressed as total number of solute particles expressed by liters (osmolarity) versus by kilograms (osmolality).
      But I honestly have no idea how this causes cardiologists and nephrologists to argue lol.
