I wish there was no portal spawning behind you, as you'd strictly have to beat the boss for a one-way portal to appear and brign you back right where you initially were. The fact that most horror mods focus on adding entities rather than dimensions makes it boring too quickly or makes too much noise to be fun. I think the idea of a new dimension with a grotesque and psychologically oppressive atmosphere that is difficult to survive with vanilla items from Endgame is a fantastically scary idea.

I performed a lobotomy on myself, so much so that I was sent into a dimension that most likely only existed in my own mind, only to fling a chunk of my brain into existence, putting its constantly lobotomized existence out of its misery. freaky title, very interesting looking mod... i actually really like this lol jokes aside, i think the final boss could benefit with more body horror and not just reskinned medium slime spawns. maybe make it spawn the cells of the immune system to defend it while also turning the landscape around you? for example, surrounding meat blocks are replaced with neurons that try to lash at you, inflicting a debuff that inverts your controls, making it harder to move around to dodge the immune system's onslaught
You should add a meat demon - a friendly guide that fuses with your body once you enter meat world. He will explain every item and what to do in this dimension, but there is the twist - he does it like a complete madman and only with his "meat" language - which means he will talk about every aspect of any object like he would talk about meat. He will be also pretty talky and react to events happening in the world around, but you can shut him up for 30 mins just by straight up screaming at mic. Also no, he will not deatach from you in any other dimension he will just say he hates the ugliness of other worlds and hides inside you without bothering till in the meat dummension again
As stated in a different comment I made, make a Deep Dark style cave biome called The Undermeat, where all the meat is rotten and rotted versions of meat mobs spawn. Killing enemies near a block called a Rot Catalyst will make the rot spread to other nearby meat blocks, just like the Sculk with Sculk Catalysts. Crazy work btw.
You should get exactly 13 meat pearls from killing the lobotomy, to find the meathold in the overworld and use them to light the portal, you can decide what to add afterwards
Make the brain only the right part and stop the player from ever using offhand when you spawn the final lobotomy boss, since each side of the brain controls the opposite side of the body.
