My guess would be that it's in practice tool and rammus just aggros the two nexus turrets with instant HP regen turned on. Once the turrets are at max heat, rammus just uses the instant teleport back to base and turns off instant HP regen. The turret shots will take a couple secs to travel across the rift before one-shotting rammus.
