Hi, you mentioned that you have prescribed 2.5mg of dutasteride before, and that the standard dose is 0.5mg. Have you ever prescribed a dose in between 0.5-2.5mg? Thiking to take oral finasteride to maintain my existing hair but not a fan of minoxidil due to side effects in oral form and irritation and being dangerous to my pets in liquid form. You mentioned that topical finasteride can have similar systemic levels as oral form. Is this the same for topical/oral minoxidil? I've been hesitant to start oral minoxidil because of its effects on the heart. But if I'm systemically absorbing similar dosages topically, then i would rather just start the pill form of it. im way more consistent with the pills. Does it all come down to molecule size?

I've been on finasteride since 2017 or 18 I think, and just over a year ago I started to take dutasteride. It seems my hair is a bit thicker now than last year. I'm wondering since the half-life of dutasteride is so much longer than fin, can I get away with taking dutasteride only every other day or even every 3rd day for the same effect? Is it worth considering taking finasteride one day and dutasteride the other day or does that really not accomplish anything more than just dut? also just had a smaller hair transplant, 1400 grafts, and I've also started to take saw palmetto and pumpkin seed oil. Not sure if I should ask doctor for oral minoxodil. Isnt that a blood pressure medicine? Loniten?
Don't take my advice serious since I'm not a doc, but I've seen many people taking the dutasteride pill once a week, while taking .5mg or 1mg fin every day. It seemed to work, since the half life of dutasteride was over 15 days anyway
I’ve been taking oral Minoxidil 2.5 mg tabs and oral Finasteride 1 mg tabs for a couple of years now and have gotten decent results. However, a couple of friends have been using a spray combo version of Minoxidil 7% and Finasteride 0.3% and there is a pretty significant difference in only 6 months. I am going to stop taking the orals and try the spray. However, there are two more additional ingredients in this spray - Ketoconazole 2.2% and Biotin 0.2%. Do you use these two ingredients in your practice? And if so, have your patients seen good results with these two added ingredients?
Do you think the side effects are underreported? E.g. gyneocomastia, people probably wouldn’t know how to report this in the trails etc or may just attribute this to other stuff
Do you think the side effects of finasteride are actually from other aspects in their life habits? Smoking, drinking, poor diet, no exercise, existing mental health conditions, etc?
@@andrewfischer6533 if anything, Gyno would be the clearest side effect to report. All the other mental stuff are the ones that are difficult to be judged objectively. I haven't had mental issues, just libido that doesn't push me to jerk it everyday anymore which i don't personally find harmful.
@@andrewfischer6533 yes they are underreported.. nearly everybody i know who took finasteride experianced some sorts of side effects at some time.. mostly mild ones but that doesnt matter.. and people in general forget what it means for your Organs to be on a drug from young age on. every drug has side effects
MD here. Yes it does. I had to drop back to my previous dose of 1.25mg daily because I developed exercise intolerance. Within 48 hours, I was able to complete my usual run once more.
I believe Saw Palmetto kept me in the game. I like Finasteride but I just don’t know what’s going inside my cells. So I stop if I see loss maybe use topical. What I find amazing since 1989 when minoxidil came out you would think 35 years later we find the real root of this problem.
I'm actually curious about how the DHT blocking works in terms of genetic programming. If the hair is programmed to fall, usually around early 20 to 30, but it's being blocked by finasteride so it wouldn't fall. But what if say you're older than that now and you stop finasteride, chances are your hair is less sensitive to DHT at that point right? Meaning you defended against most of the danger in hair loss (depending on your gene programming) regardless of you using finasteride or not at that point. Those hairs that you blocked DHT from wouldn't all just fall out all at once?
@drgarylinkov Hmm I was under the impression if say you're 40, where your hair loss is a lot less aggressive or even stopped perhaps, the hair you kept with finasteride would also have stopped being affected by DHT due to your gene. Is there a definitive answer to that? I couldn't find any studies regarding this issue. Maybe you could add your experience In a video about someone who stopped using finasteride for whatever reason they may have.
I've been on minoxidil 5mg bid for 2 years but am on a diuretic for hypertension which has some protection from edema. If I go more than 10mg per day i get some tachycardia . I did not like the topical minoxidil yuk
Mine didn't start to shed alarmingly until I was 56, so I am one of the lucky ones. I caught it in time, so at age 60, I have a fuller head of hair than many younger men.
sat and listened to the whole video- great upload. I was on fin, but recently went to adovart- no significant sides yet. maybe some libido loss?..maybe?...but either way I'm bummed I cant take minox. Both the foam and the liquid made me get some weird swelling lizard skin around my forehead and ears, cheeks etc. Too scared to try the oral version. Have you had patients allergic to the topical but not the oral?
I wouldn't touch it. It definitely increases DHT and just Google creatine hair loss and it's loads of posts. Creatine is a fantastic supplement but also not needed. I would love to use it but for me it's just not worth the risk.
After more than 20 years on 2mg of finasteride daily my sexual health had no negative side effect whatsoever however my anxiety increased and cognitive abilities declined. I had trouble learning new languages or remembering what I talked to someone just a week or two earlier. My memory was not as good as it was prior to using it that’s for sure. It was that constant pit of nervousness in my stomach that developed after about 5 years that destroyed my confidence and led to bouts of depression. I now use saw palmetto and topical minoxidil to control my hair loss. Honestly I will never take any oral drugs again for alopecia. Good luck on your journey to overcoming this nasty autoimmune disease that impacts a lot of young people.
I am using Hairy Pill (Australian Product and with 10mg Minoxidil and .75mg Finasteride ) for almost 3 years but the truth is its not what it says, the growth is there but not as thick they say, so I am confused by the facts this guys says. Now I am going for a Hair transplant.
Very informative video as always! Im 28M, Iv been using Fin and Minoxidil for 5 months now (Plus Mesotherapy), thought for long time before getting into Fin. NO side effects so far, but also no improvements neither unfirtunately. In fact, Iv been loosing more hair since I started with the treatment. Doc said it was unusual as shedding occurs on the first month. If you are thinking on starting Fin, I´ll say to go for it, if any bad side effects presents, you can only quit

As I've learned so far by your videos (they're great, by the way), only fin + minoxidil make sense, the rest of possible treatments are pretty much 1%. Does it make sense to add aromatase inhibitor along with fin & minoxidil? My understanding is that T, being too high, is being "transformed" into estrogen from which the sexual side effects are coming from & aromatase inhibitor would minimize these - let me know your thoughts. My PCP recommended Minoxidil but I want Finasteride. Should I go through a different avenue to get a prescription?
I think any type of drug even if it’s 1 mg over years got to cause other side effects. No matter how we look at it we are finding medication will cause other problem. I was on fin for 2 years I seen hair growth but I stop and I haven’t lost what I gain and it over a year. I also used minoxidil for over 32 years and stopped about 6 months and still haven’t lost what I gain. I still have more hair today than I did at 26yrs old and today I’m 61.
And then some people get side effects only near the beginning they started using it. I always thought that if you're 2-3 years+ free of sides, your chance is even lower to develop any.
@ I used topical minoxidil started 1989 with 2% and than used 5%. I used both foam and liquid. I used it once a day. It really help me maintained what I had. I did take saw palmetto. My goal was not to go bald and it definitely did that. I started finasteride and saw more growth. I decided to stop just to see what happens. I did notice my man hood wasn’t getting hard as I used too. Now not sure if turning 61 has anything to do with that. That was reason to see if that comes back. I’m only using Copper Peptides just watching if I start losing my gains. Right now it’s prevent any hair loss. I do use derma roll once a week.
If you have really bad hair loss at a young age like I'm 24 and a Norwood 5. I did recently have a transplant but it's still very thin on the sides and back. Would dutasteride be better for me than finasteride?

@@Tar0245 yeah, I started losing at 13, I understand, you gotta throw the kitchen sink in there. I'm from Russia, the use of Finasteride to treat baldness is off label here, and it's usually not prescribed. You can get topical Finasteride and minoxidil, but that's it. Forget even thinking about Dutasteride or topical anti androgens. Needless to say I'm still losing hair, even while on treatment. Strict regulations on drugs do that. So watch out for Kennedy Jr, he advocates for this exact thing.

oral minoxidil sides are way more serious than finasteride. Also, its not dose dependent ( idiosyncratic), which means that if youre genetically unfortunate you could develop heart related issues regardless of the dose. I don't understand why dermatologists recommended oral minoxidil without consulting a cardiologist.
If someone had heart issues due to either lifestyle factors or genetics, I would say that those issues of concern would have manifested before someone started losing hair.
@erik370 ask your doctor/dermatologist. There's an increased risk to everything these days but what I'm saying is that the risk doesn't warrant cardiologist/specialists consultation. By all means, if it makes you feel safe, then spend the extra money. There's a reason they start you at low doses and monitor things. Ask chatgpt but start with "science backed," then ask your question.
@@jalcantar619 dermatologists aren't trained like cardiologist. Minoxidil affects the heart, so you need an expert opinion on it ( dermatologists aren't experts on that field). Research tells us about the idiosyncratic effects of minoxidil. Also I doubt dermatologists monitor heart health while on the drug, so a lot of cases aren't reported. Scientific literature tells us about the dangers of minoxidil.
He commented on it in his previous video on Finasteride. If I remember correctly the incidence of cancer was roughly the same in placebo group. Don't quote me on that go watch that video
Well luckily we DHT causes heart problems, so a 5-alpha reductase inhibitor like finasteride and or dutasteride will help your heart health by a lot (reference 15:20). I think people should start talking more about the positive effects that reducing DHT has on the body.
@@crapdevil1772 I never said finasteride itself helps with heart problems, I said lower DHT of the type 1,2,3 seems to lower the risk of having heart problems. Here is one study: Cardiovascular Safety and Possible Benefit of a 5-Alpha Reductase Inhibitor among Benign Prostatic Hyperplasia Patients, A Nationally Representative Cohort of Korean Men There are also other studies done on mice, showing higher DHT causes problems to the heart. Now its not very well documented in terms of studies on humans, but the few that do touch on it does seem to indicate that being lower on the reference range of DHT, helps lower the possibility of heart issues. I can attest to this I was on 2.5mg of oral minoxidil for 6.5 months and had heart issues most of the time. So then I swapped from finasteride 1mg to dutasteride 0.5mg everyday once I was almost done with OM, I then ended up going back on oral minoxidil 3 months~ after stopping it but at 5mg and I had no issues what so ever. (And heart disease runs in my family) (I know this is an anecdotal but I've heard stories saying similar things) (Also I was on topical minoxidil before any 5-AR medications, and had really bad shortness of breath and heart palpitations like really bad it was scary, I had to stop after 2 months~)

I've been taking finasteride (1mg) for over a decade..I still have my hair...and the only side effect is reduced semen volume. ...and that doens't bother me.
even when you are on fin/dut + min, over time as you age you will lose ground. Stem cells and dna die, can't avoid aging of your body parts, our bodies are decomposing every minute of every day as we march toward our inevitable fate - death.
Would be awesome if you could do a lengthy discussion with MPMD Derek, and also Haircafe (Kevin Mann). For the record, post-puberty, DHT is a trash hormone and to the extent that we do get side-effects from fin/dut, they're mild, they go away after continued and/or discontinued use, and the side-effects aren't actually caused by fin/dut, or by DHT suppression per se, but rather, by temporary imbalance between estrogen to testosterone ratio. After a while, this estrogen to androgen ratio imbalance normalizes and the side-effects disappear. Post-finasteride syndrome is just anti-male agenda propaganda, and basically all males should be taking 5ARIs, at least if they have self-respect and self-preservation instincts. Post puberty, DHT only makes you into an old man, because that's basically what DHT is: DHT contributes to making you an adult male, when you're a prepubescent boy, during puberty. After puberty, when DHT is no longer needed because DHT has no valid use after puberty, and continued DHT production will make you into an old man if you don't shut off this redundant trash hormone. All you get from DHT post-puberty: androgenic alopecia (hair loss), enlarged prostate (BPH), prostate cancer, acne, heart attacks and stroke, decreased skin quality (because of course DHT is detrimental to your skin as well), like wrinkles and such. DHT is basically the old man hormone. DHT, not testosterone, is why men don't live as long as women.

i can testify to the fact that as a mtf trans woman that i got crazy results from my regimen of estrogen, progesterone, oral minoxidil and dustasteride. i didn’t need a testosterone blocker like spirolactone. of course that all comes with breast and shrinking genitals. a plus for me but not for cis men- sorry!

It certainly indicates a need for further confirmation on your part. Luckily for you, there is overwhelming scientific evidence that these drugs work as discussed in this video. You can find many, many confirmatory studies on PubMed for example.

I wouldn't touch it. It definitely increases DHT and just Google creatine hair loss and it's loads of posts. Creatine is a fantastic supplement but also not needed. I would love to use it but for me it's just not worth the risk.

After more than 20 years on 2mg of finasteride daily my sexual health had no negative side effect whatsoever however my anxiety increased and cognitive abilities declined. Very informative video as always! Im 28M, Iv been using Fin and Minoxidil for 5 months now (Plus Mesotherapy), thought for long time before getting into Fin. NO side effects so far, but also no improvements neither unfirtunately. In fact, Iv been loosing more hair since I started with the treatment. Doc said it was unusual as shedding occurs on the first month. If you are thinking on starting Fin, I´ll say to go for it, if any bad side effects presents, you can only quit

3 weeks onto my hairloss journey. Finasteride 1mg ,minoxidil 2.5 mg, sallalmetto 200mg a p7 nutrient mix. All put into one capsule. Also using some hair shampoo conditioner and serum from Korea. So far so good. doctor hopeful I will gain alot of regrowth and only require minimum hairs to be transplanted at the front.

I think any type of drug even if it’s 1 mg over years got to cause other side effects. No matter how we look at it we are finding medication will cause other problem. I was on fin for 2 years I seen hair growth but I stop and I haven’t lost what I gain and it over a year. I also used minoxidil for over 32 years and stopped about 6 months and still haven’t lost what I gain. I still have more hair today than I did at 26yrs old and today I’m 61.
And then some people get side effects only near the beginning they started using it. I always thought that if you're 2-3 years+ free of sides, your chance is even lower to develop any.
I just started GHK-CU have you any data if it grows hair. Since I been using it my hair more healthy than using minoxidil.

@ I used topical minoxidil started 1989 with 2% and than used 5%. I used both foam and liquid. I used it once a day. It really help me maintained what I had. I did take saw palmetto. My goal was not to go bald and it definitely did that. I started finasteride and saw more growth. I decided to stop just to see what happens. I did notice my man hood wasn’t getting hard as I used too. Now not sure if turning 61 has anything to do with that. That was reason to see if that comes back. I’m only using Copper Peptides just watching if I start losing my gains. Right now it’s prevent any hair loss. I do use derma roll once a week.

Yes, obviously. But it depends on the country you are from, in my country it's hard to find dutasteride at all.

@@Tar0245 yeah, I started losing at 13, I understand, you gotta throw the kitchen sink in there. I'm from Russia, the use of Finasteride to treat baldness is off label here, and it's usually not prescribed. You can get topical Finasteride and minoxidil, but that's it. Forget even thinking about Dutasteride or topical anti androgens. Needless to say I'm still losing hair, even while on treatment. Strict regulations on drugs do that. So watch out for Kennedy Jr, he advocates for this exact thing.

oral minoxidil sides are way more serious than finasteride. Also, its not dose dependent ( idiosyncratic), which means that if youre genetically unfortunate you could develop heart related issues regardless of the dose. I don't understand why dermatologists recommended oral minoxidil without consulting a cardiologist.
If someone had heart issues due to either lifestyle factors or genetics, I would say that those issues of concern would have manifested before someone started losing hair.

@erik370 ask your doctor/dermatologist. There's an increased risk to everything these days but what I'm saying is that the risk doesn't warrant cardiologist/specialists consultation. By all means, if it makes you feel safe, then spend the extra money. There's a reason they start you at low doses and monitor things.

@@jalcantar619 dermatologists aren't trained like cardiologist. Minoxidil affects the heart, so you need an expert opinion on it ( dermatologists aren't experts on that field). Research tells us about the idiosyncratic effects of minoxidil. Also I doubt dermatologists monitor heart health while on the drug, so a lot of cases aren't reported. Scientific literature tells us about the dangers of minoxidil.

Well luckily we DHT causes heart problems, so a 5-alpha reductase inhibitor like finasteride and or dutasteride will help your heart health by a lot (reference 15:20). I think people should start talking more about the positive effects that reducing DHT has on the body.

There are also other studies done on mice, showing higher DHT causes problems to the heart.
Now its not very well documented in terms of studies on humans, but the few that do touch on it does seem to indicate that being lower on the reference range of DHT, helps lower the possibility of heart issues.
I can attest to this I was on 2.5mg of oral minoxidil for 6.5 months and had heart issues most of the time. So then I swapped from finasteride 1mg to dutasteride 0.5mg everyday once I was almost done with OM, I then ended up going back on oral minoxidil 3 months~ after stopping it but at 5mg and I had no issues what so ever.
(And heart disease runs in my family)
(I know this is an anecdotal but I've heard stories saying similar things)
(Also I was on topical minoxidil before any 5-AR medications, and had really bad shortness of breath and heart palpitations like really bad it was scary, I had to stop after 2 months~)

I've been taking finasteride (1mg) for over a decade..I still have my hair...and the only side effect is reduced semen volume. ...and that doens't bother me.

For the record, post-puberty, DHT is a trash hormone and to the extent that we do get side-effects from fin/dut, they're mild, they go away after continued and/or discontinued use, and the side-effects aren't actually caused by fin/dut, or by DHT suppression per se, but rather, by temporary imbalance between estrogen to testosterone ratio. After a while, this estrogen to androgen ratio imbalance normalizes and the side-effects disappear. Post-finasteride syndrome is just anti-male agenda propaganda, and basically all males should be taking 5ARIs, at least if they have self-respect and self-preservation instincts. Post puberty, DHT only makes you into an old man, because that's basically what DHT is: DHT contributes to making you an adult male, when you're a prepubescent boy, during puberty. After puberty, when DHT is no longer needed because DHT has no valid use after puberty, and continued DHT production will make you into an old man if you don't shut off this redundant trash hormone. All you get from DHT post-puberty: androgenic alopecia (hair loss), enlarged prostate (BPH), prostate cancer, acne, heart attacks and stroke, decreased skin quality (because of course DHT is detrimental to your skin as well), like wrinkles and such. DHT is basically the old man hormone. DHT, not testosterone, is why men don't live as long as women.

@@ivangechev4243 It doesn't fix hair, it defends against baldness progression and it does work. Losing erection seem very rare, you just get less libido. The degree of libido isn't the same for everyone either, I don't feel I want to keep jerking everyday anymore but that's been beneficial for me.

It certainly indicates a need for further confirmation on your part. Luckily for you, there is overwhelming scientific evidence that these drugs work as discussed in this video. You can find many, many confirmatory studies on PubMed for example.
