She reaches for a box of laundry soap and then remembers the blob on her new dress after she washed it. She grabs the different brand next to it. There was that funny glob of powder when she poured it in. She'll never buy that brand again (until she forgets). The assembly line worker at the factory saw a glob of grease fall into the powder. He would get in trouble if he said something about it. Besides, he has the standby excuses. "Thats what QA is for.", "Thats what a money back guarantee is for". So, he lets it go and promptly forgets about it. It takes every worker doing their best all the time to succeed. In sales you are either on the top of the podium or you are not. Just like in F1.
