A useful way to think about sepsis is the sepsis 6, give 3 take 3. The sepsis six are:
    - Cultures (take)
    - Urine output (take)
    - Lactate (take)
    - Antibiotics (give)
    - Oxygen (give)
    - Fluids (give)
Also, your standard DRABCD as mentioned in the video

Thankyou for another excellent video Armando. Recognising and treating sepsis is extremely important due to the high morbidity and mortality aspect. Get senior help involved early, and get the speciality teams of outreach and anaesthetics/ intensivists involved as they may need HDU or ITU.

A 68-year-old woman suffering from sepsis is hospitalized in the internal ward and is being treated
in antibiotics and antipyretics. During her examination this morning, she was sleepy, vital signs
Within the scope of the norm, breathing on her own, with no unusual findings on physical examination.
Suddenly the monitor beeps due to a drop in storage. The patient is tachypneic and tachycardiac.
What is the first step to be taken?
    1) Connecting the patient to oxygen goggles
    2) Intubation
    3) Observing the oral cavity and suctioning secretions when necessary
    4) Gas test to diagnose the source of distortion

Body temperature that is unusually low needs to be considered too! It's also important to clarify AVPU is a scale from A to U, rather than multiple components.

    • @Harlem55

Nobody uses AVPU. GCS score is the accepted standard.

Thanks a alot for your videos - very good drawings with very good teaching!
I have although one question regarding this video - its about vital signs on sepsis patients.
Vital signs on the SIRS criteria we use Heartrate/pulse - not bloodpressure:
Temperature higher than 38°C (100.4°F) or lower than 36°C (96.8°F)
Heart rate (HR) higher than 90 beats/min
Respiratory rate (RR) higher than 20 breaths/min or arterial carbon dioxide tension (PaCO2) lower than 32 mm Hg
White blood cell (WBC) count higher than 12,000/µL or lower than 4000/µL or with 10% immature (band) forms

Actually the diagnostic criteria of sepsis have been significantly modified over the past few years and the former concept “sirs plus infections “ is out of time and NOT appropriate neither, therefore,it has been replaced by SOFA scores, these series of modifications could be summarized in a short sentence is: goodbye sirs and hello sofa

I would caution on the advice if giving bolus fluids without checking for fluid responsiveness. Which is defined as a 10% or more increase in stoke volume (SV). If they are not fluid responsive after giving dynamic test such as a passive leg raise or bolus the literature says they are unlikely to be responsive to additional fluids in the near term. Consider inotrope or vassopressor to support perfusion. They key is to test by measuring stroke volume. I did not see when this was published but more recent data warns of fluid overload when the heart can't push it forward.

racha racha, Very different for him and for you. Medical school and nursing school are different. FYI- No nursing school teaches sepsis in its entirety during the first several months AND formally tests your understanding of it. You might have known something about it, but you were not expected to, nor did you have mastery of it then. You cannot have full mastery of sepsis and its presentations until you also have the clinical experience. Be easier on Adnan. He deserves our respect.

racha racha And please don’t say, “just a nurse”. It works to discredit the exceptional nurses of the past, present and future.
