    First half the problems are defined. From 6:00 onwards solutions
    At 6:00 using a good basic program so that actives can be safely introduced without irritation.
    7:00 Gentle cleanser that won’t deplete your skin barrier, doesn’t leave tight dry skin and doesn’t have irritants like fragrance.
    7:25 Stimulate collagen production. Retinoids. Rx & over the counter. Don’t rush. Start slow. Apply at night with a good moisturizer.
    8:45 a more emollient moisturizer than you may have used in the past.
    9:00 Morning routine. Physical sunscreen every day.
    10:05 Anti oxidants and other actives like glycolic acid which goes well with Vit C.
    10:35 Azelaic acid if you’re prone to redness.

    Harsh info, but the reality. No one talks about skin/skin care for menopausal gals. I've been menopausal since age 35 (full hysterectomy) and unluckily dealt with thin, dry, sensitive skin prior so at age 50 now, I look and feel 60. Hormones are a blessing and a curse. Thanks for this video Dr. Sam.

      Andrea D Same here, except I went through premature menopause at 40. About to turn 50 and am really beginning to notice a dramatic change in my skin. Especially my neck!

      Andrea D same here , I starts menopausal at the age of 35 and its been 3 years now(2019) I started losing hairs , hard to sleep at night ,my body and skins hots all the time ( i.e hot flashes) kind a frustrated cos your body age faster . I also notice my skins dry and rehydrated

    I’ve always taken good care of my skin. I never really liked the sun, no smoking or drinking, might sound like a boring life. Now at almost 60 I look great. I’ve also been using prescription retin a for almost 40 years.

    Menopause is rough- it's even more difficult when you got through it early due to hysterectomy & have it complicated by hypothyroidism & autoimmune issues, & have more going on than just dry skin which all the moisturizer in the world won't fix. I completely ignore most skincare advice & products not promoted by medical professionals. Love seeing actual dermatologists & chemists here on RUclips giving out accurate information about skincare. Thank you!

    Thanks Dr Sam for some great information. As an Australian in her mid 60s, my face isn't too bad but my hands have lots of sun spots and the skin is thinning. I hope in another video you will include the best ways to treat these problems.

    My interest in skincare started when I was a young teenager so I feel lucky that my skin looks good at 60. Unfortunately I haven’t paid enough attention to my neck and I can see the difference. Looking forward to the upcoming video about the neck!

    What a wonderful comment - driving a bus into a wall. Surgical menopause from cervical cancer. As well as being scared for the future because of possible reoccurrence, it's nice to know I'll lose my youthful looks too.

      Many post menopausal women have lovely skin. It is not all doom and gloom. Protection from the sun can make a big difference. What makes most of us look great is a lovely smile. We tell the young not to stress over their acne, few notice it, the same applies to wrinkles, when we are older. Wishing you health and happiness, and I think that your beauty will always be there!

    I had acne as a teenager and young adult. I finally resorted to Accurane in my early 30’s. A true blessing! After menopause I have had a terrible time with blocked pores, milia or whiteheads on my cheeks. I use tretinoin regularly and use a 5% lactic acid/ hylauronic acid every morning after my moisturizer. Thank you for mentioning glycolic and azelaic acid. I will find a glycolic cleanser to use in the morning. I did find an azelaic acid booster that contains salicylic acid (amount undisclosed) from Paula’s Choice so I’ll incorporate that as well.

    Hi Dr Sam. I have been using Retin A (ReTrieve) 0.05 for a couple of years and my skin is completely adjusted to it. Must you always follow up with a moisturiser and if so how long should you wait before applying the moisturiser.

    Thank you so much for sharing your thoughts with us. It would be really helpful if you could share a list of products that would benefit a variety of skin types. I am 60. My skin is clear but dry, sensitive, acne-prone, fussy. Believe it or not, the hardest thing for me to find is a mild cleanser that is gentle but has the tenacity to remove the day. I am an enormous fan of Flawless (what a gorgeous formulation), but I need something even gentler. I welcome your suggestions. I bet others struggle with all kinds of skin issues at this age.       Have you tried using a microfiber cloth like the makeup eraser? I use a cheap dupe with La Roche Posay Toleraine Dermo-cleanser followed by Cera Ve cream (the one for face and body in the big tub) and then Retin a every third night (I have built up to using Retin a over the last two years). I am 52 with sensitive skin but I have beaten any skin problems, including very dry skin, with this routine. I have used a physical sunscreen everyday since turning 50. I now use Elta MD Daily tinted which is the most expensive part of my skincare but totally worth it as I live in New Zealand where the sun is extremely harsh. Hope this helps somewhat.

      @@fiona4731 That was really kind of you to respond. Yup, I use a microfiber cloth (which the dermo cleanser needs as it leaves a film and doesn't remove makeup. I'm not sure my face likes it or the Avene Gentle Cleanser. It banned the Avene Tolerance Extreme Cleanser from my bathroom). I hate having to do a second cleanse but most cleansers force me to. It dries out my already dry, sensitive skin. Sadly, my face hates Cerave (my body likes the tub). I can't find good cleansers for dry, sensitive skin that take off makeup and cleanse - without a cloth. In fact, I don't think they exist. Dr. Sam had to create one! If it had gentler surfactants, it would be my got to. Retin-A and I tried to like each other. My skin said, "Get away from me." I am on a Retinol, which likely does little but makes my skin look like it's doing a lot. Baby steps. Luckily, I have clear skin. But my days are numbered. I use a combo sunscreen (from my derm. He used to have me on the Oil of Olay for Sensitive Skin, 30 - only the yellow cap; the Vanicream for Sensitive Sunscreen, 35; and, Blue Lizard Sensitive, all mineral, for body). Dying to try Elta for my face. Can't decide which one. I have heard such mixed reviews and so afraid of breaking out. You have done such a great job nailing a routine that works for you. I am working on it. So proud of you for sticking with the Retin-A!

      @@seechangenyc280 See Change NYC I've always double cleansed because I don't think one cleanser can remove it all. You need something to remove make-up and sunscreen first, and then follow with your "regular cleanser" to remove any last traces/residue. Have you tried the Clinique Cleansing Balm? It works well. Micellar waters also work well, but not all are created equal, so, you have to try until you find one you like, and if you do use a Micellar water, be sure to rinse it off. Now, my main reason for commenting back to you is to tell you about the Paula's Choice Moisture Boost Cleanser. I've been using it for 20 years and haven't found anything better. It is extremely gentle, but thoroughly cleanses your skin without making it feel tight, dry, irritated. I have extremely sensitive skin (normal to dry, slightly oily t-zone) which is even more sensitive since starting Retin-A, and this cleanser NEVER causes irritation, even if I cleanse twice with it. Even on days when my skin is angry, it doesn't irritate. I also love the fact that it rinses clean and doesn't leave a film on my face. It's a creamy type cleanser that suds up just a little when you work it into your wet face. It's a beautiful cleanser. I can't say enough good things about it. I feel confident you will like it!! ALSO, if you're looking for a good sunscreen, my favourite sunscreen is also from Paula's Choice....The HydraLight Shine-Free Mineral Sunscreen SPF 30. I've been using it for years, and it has never caused any kind of breakout. I like that it's not too matte on the skin and doesn't cause foundation to pill. It's a good one to consider. I've tried many other sunscreens, and this continues to be the one I re-order most often. If you've never tried Paula's Choice, you could call their customer service number and ask them to send you a few samples. Their customer service is great! Good Luck!       @@michellekelly2937You should work for Paula! I'm sold! Will definitely call customer service and have them send some samples. I'm a longtime Paula follower, yet I never tried her products. She taught me so much about skincare. What you're using sounds great!. I have a good SPF (knock on wood) but that could change at any moment, and when it does, I'll be sure to give the one you rec a whirl. I tried the Clinique TTDO balm. Works so well! I heard the TTDO oil is good too. I thought the TTDO made my face feel oily the next day but it could have been from a million things. I've been trying so many products, I don't know which to blame. Will give it another shot (love Sephora and their samples!). Recently, I've been trying out double cleaning with mineral oil (forget the bad noise. It's inert. Gentle. Cheap. It's only flaw: it doesn't emulsify. But once you add the second cleanser, it does the job. Maybe my cleanser woes will be solved by Paula). On the run, I love Bioderma H20 Micellar Water: fragrance-free, doesn't irritate....I used it the other night because I was trying out the Avene Gentle Cleanser and it felt like something was left behind. It was: my entire face of sunscreen! And you're right, I can't get seem to get away from the double cleanse method or I might as well not bother cleansing. Flawless is the closest I've come to one cleanser that does it all. Maybe Dr. Sam will make a Flawless 2.0 with milder surfactants. I'm crossing my fingers (hint, hint). Many thanks for writing!

      @@seechangenyc280 I'm always afraid people will think I work for Paula, but I just can't help myself when I see someone struggling to find a good product. Paula's Choice has so many good products!! I'm just a happy customer! Like you, I've followed her for years and decided to try a few products, and I'm so glad I did! The Moisture Boost Cleanser was one of the first products I tried. I've been hooked ever since! Just so you know, PC has another cleanser that is similar, called the RESIST Optimal Results Hydrating Cleanser, and a lot of people (my sister, included) really like that one. It comes down to personal preference. Both are great. I just happen to prefer the Moisture Boost Cleanser, but the only way to know which one will work best for you is to try. I did try the Avene creamy cleanser, and I did not like it. For some reason, a lot of the Avene products irritate my skin. If you do try the P.C. cleanser, I would love to know what you think.     I have had a lot of difficulty with small whiteheads (closed comedones) during menopause. It’s quite disturbing to try to treat acne and wrinkles at the same time.

    I have worn gold and silver all of my life. And now after menopause I can't wear any jewelry. Because immediately it irritates and swells and gives infection even with sterling silver and gold.

    Thank you for this video. We older women tend to be forgotten or written off as lost causes when it comes to skin care. However, we shouldn't be.
    I'm 55 years old and in my sixth week of using tretinoin 025. While I don't see any changes yet because it's too soon, my skin already feels softer and smoother. I'm sure it's due to the changes in caring for my skin in order to accommodate the tretinoin, rather than the tretinoin because the tretinoin hasn't had time to work yet. Whatever the reason, I'm calling it a win and I look forward to the three month mark, when I should be able to see some visible improvement.
    I'm also looking forward to your video on neck and hand care for post menopause. Thank you for thinking of us.

    As I got older, after having babies, I am a huge fan of Noxzema. Not as a teenager, but for my older years. It does a great job of exfoliating, but still keeps things balanced and moisturized afterwards. My face never feels weird or dry after washing it, it always seems happy after washing with it. Now, don't wash it off right away. Let it sit first for a few minutes on the face. It's my go to.

Question: I was considering adding an led light mask to my 45+ skin routine to boost collagen, but I have thyroid issues. As a dermatologist, would you have any concerns? I already use a prescription retinoid, actives, etc.

      I am 52 and going through menopause and have tried many skincare products over the last two years for my sensitive and drying skin. Paula's Choice has good products as do La Roche Posay and Cerave. I use LRP Toleraine Dermo-cleanser at night with a moist microfiber makeup cloth followed by Cerave face and body cream (in the big tub). I now use a prescription Retin a but started with P'sC Resist night cream with retinol then her 1%retinol serum then one tube of Differen. Dr Sam Dr Dray and Dr Davin Lim all have great advice and skincare recommendations on their you tube and instagram channels. I use Elta MD sunscreens with zinc oxide. Hope this helps

    I have always had cystic acne but this summer is particularly humid and for the first time ever, at 52, I'm having breakouts all over my forehead, nose and chin. I'm not sure of the best treatment.

    I went into menopause in my mid-40's and medications depleted pretty much all my estrogen. But through a combination of pretty good genes, skinwise, sun avoidance and good skincare, my skin is moist and minimally wrinkled as I approach my 70's. So my holy grail in order of importance is a great sunscreen always, a tretinoin, vitamin C, chemical exfoliation (shout out to Biologique Recherche P50), other topical anti-oxidants, hyaluronic acid, lots of fruits and veggies and regular exercise. I use gentle cleansers, but which one and which moisturizer is not critical, for me. And whatever is left over from my facial skin care, goes on my hands. Of course, I never smoked. But, no, I'm not a health "nut". I'm a foodie and drink my fair share of mostly wine, some beer and occasional hard liquor.

    Excellent info, and you’re right. My mom went through the change at 47-57 but her skin didn’t change, much to her shock! She attributes this to her mom making her use moisturizer and spf from a very young age. She in turn slapped a moisturizer in my hands at age 13 (I’m now 20) and told me to use it am and pm, and I do, along with spf. I have added Vitamin B serum just in case I don’t have my moms genetics - and some of it for sure 100% is genetic. Her hair is easily as thick and lustrous as yours with minimal greys and is so long she can sit on it, but she wears it in a beautiful updo. I know she uses AHA's as I’ve seen her do it in wipes and 20% cleansers. Other than that she’s never used soap on her face. I do wonder what is due to her skin care and what is genetic. Her dad was the same and seemed to be forever young. But I hope some is genetic 😂.

    I am 54, live in Australia and I don't have much wrinkles but I have issues with pigmentation. I have 4 Picosure laser, on a daily basis used vitamin C in the morning, niacinamide at night and alternately retinoid cream. I use sunscreen every day. Why is so to get rid of pigmentation ?

If that is the case, good for her! Plus regarding questions on her products I am sure her customer service is more suited to answer that than herself...

    Dr Sam, do you wear hair extensions for thickness or your hair is naturally so full? I’d love some tips on what hair products you use and if you do any scalp treatments or massages etc. People often forget that good hair starts at the root and our skin under there needs just as much attention as the rest of our bodies!

    hello, and I want you to know I just love your channel. I would love your advice. I'm a 56-year-old, active woman. my skin never breaks out and I have been on hormone replacement therapy in hopes it will help with my collagen. This is where I'm confused. I suffer from very red skin, it's smooth but I often feel like my barrier is not where it should be. In other words, my skin often feels very sensitive. I wash with a very gentle cleanser. Moisturize before I use my 0.01% tretinoin cream. In the morning I just rinse my face, splash it with some moisturizing essence, put a heavy cream on while my skin is still damp, and follow it with SPF. I do use red light therapy at night and every other day I use vitamin C. Where am I going wrong and why does my skin feel so sensitive all the time?

    Well I see what you mean about the behavior in younger years....I used to take a lot of sun and now my face is full of brown stains. Many years trying to eradicate them. Now solar protector every single day

    "'driving a bus into a wall"' ... every woman ages differently. PLus I have seen that it is up to 30% of collagen depletion, hence it could be less depending on women. I would like to see tangible studies showing the iofference betwenn someone who aslways looked after their skin before menopause ie avoiding sun etc vs someone who hasnt as I can see around me that this makes even nore of a huge difference than just menopause alone !

      C. D. I used to use the Paula’s Choice niacinamide, but now now mix The Ordinary’s version through my body lotion as it’s so cheap (along with AHA and vit C serums from TO).

      Galato G I mix TO with body lotion; for face I layer serums then use moisturiser etc last. As TO is so inexpensive I think: why not?
      This has actually come about since I started to work in Aged-care, and have been mortified by clients’ skin integrity - that paper-thin skin, so prone to tears, is SCARY, so I’m now getting serious about body skin care.

    Dr. Sam - in your opinion, is retinyl retinoate more efficient than retinol for slowing down skin aging? I recently switched from Dermalogica's Overnight retinol repair to Medik8's r-retinoate, since I had read that r-retinoate is supposed to be more effective and less irritating. In Sweden it's impossible to get a prescription for retinoic acid unless you are severely afflicted by acne, and I think doctors only prescribe a kind that also contains antibiotics.       Yes, niacinamide can lighten skin & fade dark spots. I recommend CeraVe products if that is available in your country.

    I know you are not a fan of facial oils, but as I have been teetering on the edge of menopause my skin has come to need them. I can absolutely baste myself and my skin will gleefully slurp it all up. It has made an enormous, positive difference. Missing substantial application of oil, even for a day, causes a complete facial hissy fit. It is not as bad as the cheese hissy fit, but it is not nice either.

      I love a facial oil and have used one since I was 15 and at 61 I'm still a fan. Most skincare doctors seem to hate facial oils for some reason, but I get great results and will keep using them.

      Facial oils can cause pimples but that is only if you are acne prone, maybe that’s why doctors don’t like them 🤷‍♀️

    Depressing as hell. I had my ovaries removed 5 years ago bc of ongoing problems with cysts. I wish I would have never done this bc of all aging side effects. But, no turning back now 😢

      Stacy B I had a radical hysterectomy for the same reasons, plus fibroids. I hear you! I just wanted to share the one thing that has made such a difference to my facial skin, and therefore my self-esteem. About six months after the surgery and noticing radical changes in my face (as in I looked like I’d aged 10 or more years seemingly overnight), I did some reading, then went to my doctor and asked to be put on Retin-A. After a month of adjusting and peeling like an onion, I have never looked back. It is honestly the very best thing I have ever used. Hopefully, you live somewhere where Retin-A is available to you. Also, supplements are your best friend. Best of luck!

    Thank you! I’m about to order your products ... I am nearing menopause and have used cerave pm and hydrating cleanser for at least the last 10 years. And decent sunscreen. I have similar skin tone to you. I am lucky so far and have great skin so I get nervous about trying retin A . Because I know there can be a bad stage before (or if )your skin gets used to it. Do you still recommend ? (if it’s not broke don’t try to fix) ( “saying”)

      jodi Aryal I can honestly say that Retin-A has been a skinsaver. The peeling/burning stage is a pain, but it doesn’t last long. The amazing benefits you’ll see afterwards are well worth it! 💕

      jodi Aryal been on it for about 6 weeks. .025% for 30 days(I used it every day). I’m now on 0.05 every night, still no bad reaction. After another 60 days I will probably go up to 0.1% I am using sunscreen and wearing a hat outside. It’s all good so far.

      It doesn't have to just be a sign of hormonal changes. It could be excessive sun exposure, stress, poor diet (eatng grains, sugar, alcohol or having intolerances to things like dairy),deficiencies such as vitamin B complex, vitamin c, d, zinc and trace minerals

    Boys have the same issues too, due to the decline of hormone levels. Well i do when the mirror reflects back reality of my skin. Never smoked, but the sun had free access and wine has been active too. Nowadays there is so much more info, but hey I’m before the Internet 😎
