Suffered from hyperpigmentation my entire life. Between 2006-2012, I did every medical grade chemical peels at a plastic surgeon’s office because I was told laser was a no no for my brown skin. It left me with hypersensitive, flaky skin for years but didn’t budge the hyperpigmentation. Fast forward to 2020. I knew lasers had become very sophisticated and it was a matter of finding the right provider. I was lucky to find an excellent dermatologist here in Boston who specializes in my skin type. 1 pico toning, several targeted sessions on my sunspots and a final overall fraxel laser later, I have NO sunspots, NO moles. I am so fucking happy, I am commenting under RUclips videos and basically screaming to the world that a cure exists!! No amount of peels, skincare serums will ever get rid of hyperpigmentation in the deeper layers of the skin. Serums and peels just cannot penetrate that deep. Bottom line, please see a good dermatologist and identify the right laser for your skin. It will be an upfront cost but it WILL get rid of your hyperpigmentation. 2 years on, my skin is still glowing. It is not sensitive or compromised as it was with the chemical peels. Best money I ever spent
Yay! I’m SO happy for you and really happy that you mentioned the added value that in-office procedures can bring to the table. You are absolutely right that some types of pigment are too stubborn to be corrected with skincare alone! Seeing the right dermatologist really can make all the difference.
I see dr. Jennifer Lin. She is the director of harvards colored skin program. She also co leads their melanoma prevention program. I was fortunately recommended to her by Brigham and women hospitals doctor recommendation hotline. Recently identified as having BRCA1 mutation so now she also does my skin cancer screenings. She sees cancer/clinical patients 4 days a week and on Thursday’s she does aesthetic stuff out of brighams chestnut hill campus. Her current next appointment is June so I would call right away if you are interested in being seen by her
@@shaziaq8097 if you can’t get in with her, ask the office for recommendations. It always helps if you can be a little specific about your concern and skin type( for me it was hyperpigmentation in skin type 3) and they will match you with an appropriate specialist

A lot of the comments are on melasma. For me it’s old age brown spots. Even as an older person, I’d like my skin to look as good as possible. My skin is more sensitive than it used to be. Please don’t forget us older women.
The one product that has really helped my PIH has been the Selfless by Hyram Retinol and Rainbow Algae serum. What drew me to this product was that it has retinol and tranexamic acid formulated together, and I was very curious in using both to help fade my dark spots. My skin also loves niacinamide so I always love to have that in whatever moisturizer I'm using, I really do love to have an minimal routine and not a fan of layering multiple serums. Multipurpose products are a win for me!
The discussion on various skin conditions with ingredient and product recommendations is very helpful. When showing example clinical photos it would be nice to show a variety of skin tones so we can see how hyperpigmentation presents on deeper skins. Thanks
Please make a video about melasma & how to treat it. My bf is 46 y.o. & has it mainly on his forehead. He didn’t wear sunscreen on his face daily until he met me. Trying to reverse the damage that was done over the last 4 decades lol.
Yes, video on melasma, please!!! Especially which peels and treatments are best. Like I don't know if a TCA is best, or glycolic, or Cosmelan, or what! Too many options make it confusing.
Thanks for such a comprehensive video! Would love a video on melasma because although most of the tips on hyperpigmentation removal overlap with melasma, melasma is definitely a little bugger that requires you to take it up a notch to remove as much as you can AND maintain (the latter of which I failed at previously because I did not know I had to continuously suppress tryrosinase even after clearing the melasma patches). It would help others and I’m always about learning more!
I deal with really bad PIH on my cheeks. I was looking for an effective, but pregnancy safe product (not pregnant, but thinking about trying for one more baby.) I've been using the Naturium Azelaic Acid 10% & really been liking it. I haven't even finished the first bottle & can notice a difference; albeit small, but I see it & I love that it's a slightly thicker texture so I can really focus it on my problem areas. (Just putting this out there for anyone in a similar situation.)
I hope that this helps someone. I have had pregnancy related melasma for years now. It didn’t completely go away, even tho chemical peels (I love the Perfect Peel highly recommend a series of those) and PRP/microneedling (2 sessions) and Morpheus have all really helped. It’s mostly gone except for splotches on the right and left side of my upper neck, little bits on the jawline and a tiny bit on the forehead. I have started using Photozyme MD’s C-E-ferulic serum and it’s really awesome. It’s only of the only vit C serums I can tolerate- even tho I have really tough oily skin vit C tends to bother me for some reason. It’s 15% vit C, but the unique thing about this serum is it also has 3 DNA repairing enzymes. I’d go as far as to say there isn’t a more scientifically advanced C serum out there. They also have DNA repair enzyme serums with just the repair enzymes, but I love the vit C serum. It’s very much like water and you only need a small amount. I’m glad I found it. You cannot use retinoids with it, so I alternate between this and Altreno (rx .05% tretinoin with collagen, hyaluronic acid and glycerin- also would recommend this one).
I absolutley love your videos, Dr. Ellis! I'm very surprised you don't have 1 M subscribers by now! Usually really good content spreads. You deserve that many subscribers any way. :)
Thanks for the awesome content! I wanted to add that the timeless vit c you mentioned also comes in a lower concentrate of 10% vitamin c if someone can’t handle the 20% or just prefers a lower strength.
I have dealt with PIH since my early teenage years and azalaic acid has worked the best for me in regards to the appearance of my skin. Tretinoin has been an absolute game changer. Within the first week of using my partner asked what I had changed in my skincare routine. I’m only 2 months into using however this is the best my skin has looked since childhood 🙌🏼
FYI: I have melasma on my arms, & neck & a little on my cheeks. 1. Sunblock SPF 50 (I reapy) doesn't cut it. I found wearing protective clothing does. 2. Because tinted SPF is better, but I don't want to get it on my clothing from my forearms, I apply tinted 1st, then colorless sunscreen.
I adore the Phloretin CF serum! My pharmacist recommended it and I fell in love with it. I use that in the morning and a prescription retinoid along with the Ordinary niacinamide 10% serum. My skin has never looked better, and the glow is real!
@@bsimple with pleasure! YES! Please do a dedicated video on melasma. Nothing has helped with my melasma of 5 years now 😫 I've used all these ingredients. I wear mineral SPF daily. I noticed that every time I reapply it makes melasma less visible but as soon as you wash it off you see it all. Heat makes mine worse. In the summer it is impossible to keep it from getting darker. But I'm still not giving up on it. I'm always learning new things.
Yes a video on melasma please! I have a melasma spot on my nose, tried hydroquinone, glycolic peel, and I see no difference so far. Right now I am using the Skinceuticals serum, wish me luck!
Absolutely LOVE your videos Dr. Ellis!! Could you make one on fading scars, particularly ones from things like burns or cuts? I got a pretty bad burn from a cast iron pan that I'd love to try and fade 😅
You've often been with my recommendations. I chose this topic,to watch a full video of yours. You seem to really know your stuff. Hopefully something you mentioned can help, with my hyperpigmentation. It has slightly darkened, around my mouth;as I get older. My confidence is not what it use to be, because of it. That Kojie san bar really does pack a punch! I found I saw results within a few weeks, and seemed to tolerate it okay in sensitive areas. I stopped using it after a while because it's soap-soap, and since I have eczema-prone skin I usually stick to soap-free cleansers… but I am kinda tempted to give it another go 🤔. I remember when I was using tretinoin and azelaic acid on my face, my skin looked really clear and even toned. I've been trying a more gentle routine for the last year to see what affects my redness, but at this point I think that's more vascular than skin barrier, so I'll probably start integrating some stronger actives back into my routine soon.
So good to hear your experience! It definitely can be irritating so I like to have people use it for just a couple weeks at a time (and moisturize really well while using)!
Problem with all vitamin C products especially from Skinceuticals is the oxidation that occurs and one ends up with an orange halo around eyes and everywhere that it was used!---Please recommend ones that are cosmetically elegant and don't turn orange!
Your video was really informative but by the end, I had no idea which product or combination of products to try, and in which order. At least 10 diff things were listed and I feel confused. Can you please do a video that has advice on which products of use and how to layer them. Also would be helpful to understand variations for skin of color as I think many of the offered solutions are not necessarily well tested for efficacy on skin of color.
So informative, there are just so many products out there to try (not sure my bank would be happy), I’ve tried a few things that haven’t worked for me, although I do get that patience is the key! At the moment I am using Azelaic Acid and Vit C moisturiser. My skin is now very soft but the brown hyper is still there…
Thanks for covering hyperpigmentation on the body. I'm trying to fade my spots on my shoulders and back of my thighs. I'll be checking out the soap and lotion!
I have very sensitive rosacea skin that definitely is dry and I’ve found that the Timeless 10% vitamin C has worked wonders on my skin (in combination with my other skincare). I have noticed a pretty significant difference with my redness and it has given my skin a nice glow. Very excited to see the long term results. Was too nervous to try the Stratia Night Shift retinol since the return policy with them isn’t the best but eagerly awaiting my Medik8 Crystal retinal level 1 since they have a 60 day return policy that makes me feel safer about my investment.
Kojie san soap is pretty big in Southeast Asia, they use this soap to lighten their skin due to certain beauty standards, and effective, the only disadvantage with this soap is It can burn or tan the skin easily if you are under the sun even with decent sun protection. I tend to avoid this during the summer season since this can burn my skin fast.
Do these work for darker skin? There are ingredients/products that are not good for darker skin and onky work on Caucasians. Some of these do more damage on deeper skin tones. I have had my melasma worsen with some products designed for hp.
Thank you for this info. I started peels last Dec & different brightening serums. What worked immediately for my sun spots (or within a week) was a laser called the Picosure. It's amazing. I'm still doing peels, Vit C & brightening serum s but this laser sped things up enormously
@@DrSamEllis I started using Platinum skin care last Dec. I have been very careful doing home peels & follow or watch all of their videos before doing the peel. I have been doing Mandelic 40 (probably have done about 10-12 so far) & TCA 13 with Jessner's--have done 2. I need to do more of the latter but I have noticed a big difference in the brightness & smoothness. I am also going to do 1 more Pico laser. Love your content & explanations
So happy it’s working for you. It’s great that you recognize the importance of consistency and repetition when it comes to peels. They can be so helpful when done regularly.
No antioxidants in my skincare routines. Since I am acne prone, antioxidants can be somewhat aggravating my acne. Currently I am using 0.1 % Tretinoin and 20 % Azelaic acid to control my acne and also treat my hyperpigmentation. I also love using Glycolic acid to really boost up the efficacy of my tret and azelaic. Alongside with sun protection and good moisturizers, I achieve improvement of my hyperpigmentation and and also anti aging.
I heard from another dermatologist on RUclips that azelaic acid is an antioxidant too but also helps with acne by preventing bacteria and lightens hyperpigmentation so it’s pretty awesome
I have melasma and use a self-tanner to try to hide it. Will these products, like a retinoid or antioxidant, strip the tan? Or should I stop using my self-tanner until after I correct my hyperpigmentation? I would also love to know your self-tanner recommendations! I personally have olive skin, green eyes, and get freckles VERY easily during the summer. I don't know if I am dealing with hyperpigmentation, age spots, or more active freckles. I am 28 and am just now starting to use sunscreen sadly. Would love your feedback and LOVE your videos!
Love all your information. I have some hyperpigmentation on my legs from sun damage from years ago. I am noticing white spots along with the brown spots. Is there any treatment for these white spots?
I love your videos. Your delivery is awesome. I am overwhelmed, though. I want to take better care of my skin but it needs to be simple. I’m 66, blonde, blue eyes and a former lifeguard. I need just the basic products for daily use . Do you have a plan or video for me?
In reference to Tranexamic Acid taken orally: What is your dosage recommended to patients?---Seems only the 650mg tablets are available in USA??? Are lower dosages available in USA?
my skin scars so easily and takes years to start fading, i don’t pick my pimples and i’ve never done it and my skin still gets scarred, i also try to wear sunscreen in the mornings. i currently use niacinamide and adapalene as my main actives and the rest of my skin care contains simple ingredients
I have been battling a combination of sun damage and hormonal melasma for over a decade (maybe even close to 15 years). It did not start with pregnancy but was exacerbated by it as well as just getting hot (I don't even have to be in the sun). My melasma covers my cheeks, upper lip forehead and brow bone, basically my entire face. After years of trying fade creams, prescription topicals, peels and lasers as well as every color corrector and high coverage foundation I am beyond frustrated and many thousands of dollars poorer. Since most of my face is covered I feel like it ages me. What is the best treatment course for very stubborn melasma?
Would love love that video about melasma.I suffer from it and my derm gave my Tretinoin 0.1% with the Blender(hydroquinone) 4% from Obagi. That stuff is great. The combination has worked for me.Also he gave me Azelaic Acid 15% for use morning and night.Of course I use a lot of hydration in between with the Obagi Luxe cream and the Cethaphil redness relief cream.I always star my routines with Hylauronic Acid.

I literally laughed when u mentioned the kojie san soap. Every Filipina probably uses that!! Buuut mainly for the toxic asian skin whitening trend, not eally for fading hyperpigmentations. That soap is omnipresent in the Philippines.
Vitamin c makes my hyperpigmentation worse. My skin looks tanned and leathery, and this is with every single pure vitamin c product ive tried, including skinceuticals. Even after properly storing them in a cold dark closet. Why is ascorbic acid darkening my pigmentation? It baffles me how people can use vitc and claim brightening effects when it's the opposite for me.
Your skin might not be able to handle ascorbic acid which is okay! Try a vitamin c derivative or licorice root is a fabulous skin brightener (not for melasma) and most skin types can handle it!
I have this same issue! My hands would turn brown like I had done self tanner! I like the medik8 C tetra luxe. It’s a 14% Vit C derivative. I always store my vitamin c and tretinoin etc in the refrigerator I notice I get several more months of use without the bottles discoloring and the smells going rancid
@@emiliemartin813 thank you. I'm currently trying an ethylated ascorbic acid serum from garnier and loving it so far, it has actually brightened my face. It looks like i might have to stick with vitamin c derivatives instead of the pure thing. But hey, if it works then I'm all for it.

So good to hear your experience! It definitely can be irritating so I like to have people use it for just a couple weeks at a time (and moisturize really well while using)!
@@DrSamEllis oooh, good to know. Thank you!
Problem with all vitamin C products especially from Skinceuticals is the oxidation that occurs and one ends up with an orange halo around eyes and everywhere that it was used!---Please recommend ones that are cosmetically elegant and don't turn orange!
Great information! Thank you for this video and yes please do a video on melisma.
Yes to a video on melasma!
Would love melasma video. I have an area that has been impossible to get rid of!!
So informative, there are just so many products out there to try (not sure my bank would be happy), I’ve tried a few things that haven’t worked for me, although I do get that patience is the key! At the moment I am using Azelaic Acid and Vit C moisturiser. My skin is now very soft but the brown hyper is still there…
Topicals has a good cocktails of ingredients for hyperpigmentation.
LOVE a good return policy. Excited to hear your thoughts!
@@DrSamEllis thank you for all the great recommendations, everything I’ve tried has worked great for me! 😊
Loved this video! I am going to try the Timeless and Naturium serums next.
So much excellent information…thank you!
I thought glycolic acid was too abrasive for women of color - any distinction there based on complexion?
Do these work for darker skin? There are ingredients/products that are not good for darker skin and onky work on Caucasians. Some of these do more damage on deeper skin tones. I have had my melasma worsen with some products designed for hp.
Thank you so much for your knowledge on skincare. Can you do a video on hypopigmentation?
Can you do a video of skin regiment women should have in their early 30s? Thank you!
Dear Doctor: can you do a video on melasma and laser hair removal? ( upper lips)It’s safe or avoid completely? Thank you
Thank you for this info. I started peels last Dec & different brightening serums. What worked immediately for my sun spots (or within a week) was a laser called the Picosure. It's amazing. I'm still doing peels, Vit C & brightening serum s but this laser sped things up enormously
Love! Do you know which peel you did?
@@DrSamEllis I'm trying. I have sun spots and asides the Pico, I want to see if the peels will help too so I need to be consistent. Thx :)
Azelaic acid really was the game changer, but I'm not so sure if I should be moving to 20% from 15% ?
A lot of products thrown into this video. Which one is the bare minimum? Or should be used to start with.
Great regimen!
Love this! Would love to compare notes! Would you be kind enough to share your regimen? How do you use/layer this products? AM/PM
It’s coming in a video soon 😉
I heard from another dermatologist on RUclips that azelaic acid is an antioxidant too but also helps with acne by preventing bacteria and lightens hyperpigmentation so it’s pretty awesome
@@DrSamEllis can not wait for your next video, Dr Ellis
Helpful ! Thank you very much
I always liked night time. I stay clear of the sun mostly.
omgggg I would love a melasma video !!!
I need just the basic
products for daily use . Do you have a plan or video for me?
Timeless has had a 10% Vit.C for a while! 🥳☀️🌞
Alpha H Liquid Gold worked wonders for my PIH!
Black raised spots and Milia remedies please. Thank you
Thank you so much for this! I love how informative and succinct your videos are. Could you please make one on microneedling? 💗

How do you feel about vitamin F and what products would you recommend please .
Can you make a video talking about PRF for under eyes?! Would love to know your thoughts! :)
Hi Doctor I have been using Rogaine minoxidil for women for a few years now . I want to switch to the virtue minoxidil. What is your opinion ?
Ambi cream has 2% hydroquinone and is OTC.
What is the best treatment course for very stubborn melasma?
try supplements
Have u try prescribed 4% hydroquinone?
Would love love that video about melasma.I suffer from it and my derm gave my Tretinoin 0.1% with the Blender(hydroquinone) 4% from Obagi. That stuff is great. The combination has worked for me.Also he gave me Azelaic Acid 15% for use morning and night.Of course I use a lot of hydration in between with the Obagi Luxe cream and the Cethaphil redness relief cream.I always star my routines with Hylauronic Acid.
Hey! Hope you are doing well dear can u tell me plz how many times in a week u r using tratinion i m also using but once in a week with hydroquinone and rest of the days azailic acid 20%
@@judicialninja778 I use it every night,a pea size amount as well as the hydroquinone .Go slow.Go up to 2 times a week, then 3 and so on to see how you tolerate it
@@judicialninja778 Azelaic acid I use it day and noght every day I rest on Sundays when I use hylauronic Acid and a lot of hydration.

You have something of Gothamista's voice and vibes.
I'd love your review of The Ordinary, if you recommend their products and if so, which ones? Thanks for all this great info.
Very informative. Thanks
Thank you for this incredibly helpful video! Question: can exfoliation help with stubborn tinea versicolor at all?
Do you think the Skinceuticals sunscreen that you mention in this video offers enough protection even though has 4% zinc and 5% titanium? thank you for your response.

Niacinimide in other words vitamin B3. My skin cancer specialist and Cancer Council of Australia recommends taking this supplement 🌷
Yes, for those who have had multiple non-melanoma skin cancers it has proven beneficial!

Pl have a video for sun spots and melisma for brown skin thanks
I will be making formulation for hyperpigmentation where i will be using, rice, tomato 🍅 and potato as active ingredient.
Using microspheres technology.
The dosage form will be in aerosol, stick. Mainly waterless stick.
How does it sound. Please tell. 🙏
How about a video about a skincare routine for PIE.
Thank you dr Ellis, I use lanyvof those products and I am very happy , my skin is nice with that routine!!
Could you tell me please if elta is better than colorescience ? Thank you, have a lovely day🌞🌻
I literally laughed when u mentioned the kojie san soap. Every Filipina probably uses that!! Buuut mainly for the toxic asian skin whitening trend, not eally for fading hyperpigmentations. That soap is omnipresent in the Philippines.
Interesting! Definitely not trying to encourage diffuse skin lightening, just treatment of areas of stubborn hyperpigmentation 🙁
I laughed too but only because i was surprised. Lol.
Who else has used this kojic acid body soap on the underarms and did it work?
Dr. Steven has also recommended Cyspera.
@@angelinebiswas7611 thank you for responding. I will look into the product you mentioned. It looks like my skin prefers vitamin c derivatives as opposed to the pure ascorbic acid.
