    I've got few questions, I'll be thankful if you could find some time to address these!
    1- I've done my two years residency in anesthesiology from College of physicians and surgeons, CPSP Pakistan! Can I be the part of this alternate entry pathway program?
    2- If yes, kindly please guide me how can I proceed ahead!
    3- Secondly do I need to appear in licencing exam and have to score extraordinary well to get matched for anesthesiology?
    4- If I've to appear in USMLE should I be preparing for the exam or can I just come to US via alternate pathway and clear my exams while proceeding ahead as faculty or educator.
    Plus I've 1 year experience as an educator/demonstrator as I've been teaching medical students Basic sciences plus I've been teaching pain management to the physiotherapy DPT students!

      I am not aware of a pathway to become a Radiologist without medical school/college degree!!! Such a thing does not exist in the USA, Canada or India.

    Hello, Dr Iyer. I want to thank you for your endeavor of reaching out to people at large to enlighten them with your knowledge.
    I have a query regarding waiver job. I am an IMG, MD anesthesiology from Delhi, India. I am currently doing a year long transplant anesthesia fellowship from Miami. I have got acceptance for another year of neuroanesthesia fellowship at mcgovern medical school. As i understand, after two years of US clinical experience(two fellowships) i can apply for selection as an attending under AEP program in US. Currently, I am on J1 visa. Would i need to do a waiver job as a non board certified attending outside AEP to avert the two year home requirement and then apply to AEP. Is it easy to get waiver jobs like that without ABA certification? Or is it possible to get AEP in a waiver area to eventually be ABA certified? What are my options?

      Great to hear your progress. First, you do not need two years of US clinical experience prior to joining the alternate entry path program. Where did you come across this? There are two parts in the alternate entry path program - one is you get selected for the position and second is you get an independent state license. The latter is state dependent as some states requires 2 years of work experience and some 3 years of USA or Canadian experience. So this would ultimately depend on where you end up getting the position. These years can also be completed as an alternate entry path candidate.
      Regarding your J-1 question, in your case, the best case scenario will be to get a AEP position in a hospital that offers J1 waiver.

      @@AnesthesiaGuru Thank you for your insightful reply. If I join the AEP before two (or three) years of US/canada clinical experience would I be working as a fellow or attending till I complete the 2(or 3) years to get a permanent license?
      Secondly, do you know of any anesthesiologists from country like India ( not UK, australia, canada, Singapore, EU) who got a admission into AEP without having to prove their mettle in US/Canada by working there as a fellow and getting decent LOR first?
      Thirdly, do you know of any top programs that allow J1 waiver in their AEP?
      I have an interview for a fellowship from Cleveland clinic. But I doubt places like those would have J1 waiver positions for AEP.

      1. Rishabh - Your first question yes without USA or Canada experience you begin as a fellow. The reason for this is that the the state medical board requires training to give you an independent license. Honestly, I wouldn't personally worry about this. Getting into alternate entry path program is big and either one (fellow and/or faculty) should be fine.
      2. Yes I know anesthesiologists who have come directly form India to USA and those who have stopped in other countries and then come to USA. You cannot compare yourself to them as there may be differences in the experiences and how strong the CV is. Everyone has unique strengths and focus on your own.
      3. Yes good programs offer a J-1 waiver. Good luck with your interview and do check with Cleveland clinic if they offer a J-1 waiver.

    Thanks for your informative video. I have a very specific question. I'm a radiation oncologist from India with 10 years of experience. I have finished USMLE Steps 1, 2, and 3. I'm currently doing a non-ACGME accredited fellowship in radiation oncology at a reputed university in the USA. I recently was interviewed at another university for the ABR alternate pathway where they are willing to offer me a one-year fellowship followed by a faculty position. The chair of the program has asked me a specific question as to whether I will be eligible to practice in an independent capacity as a faculty and whether the insurance companies accept my services under an independent capacity for compensation? Can you answer this question? Thanks in advance.

    Thank you for making this video, it's very informative. I was just wondering if research/publications are needed for the Clinical Educator pathway? I'm an anaesthetic registrar in the UK and want to apply via the Clinical educator pathway down the line. Even though we take part in audits and quality improvment projects in UK anaesthesia training we don't all automatically do much primary research or get publications. Can I focus more on clinical anaesthesia and quality improvement projects, presentations and teaching opportunities instead of trying to get into primary research as that isn't my main interest?

      Thank you. UK graduates are eligible for clinician educator pathway which is not a research based pathway. Focusing on the things you mentioned should make you competitive. Publishing your quality improvement projects will be beneficial for your application.

Another question if it’s okay…is it possible to approach an institution to do a fellowship first then ask to join as faculty onto the clinician educator pathway? How would this work for places that only take people who’ve done US residency for their fellowship? Is it worth still contacting them or would they just not accept international anaesthetists?

      @@nelliedivani9063 As i mentioned in the video, it is difficult to predict which hospital is willing to hire an alternate entry path program candidate at this particular point of time. A good approach in my opinion will be to directly ask for alternate entry path program rather than a two step approach of fellowship and then clinician educator pathway. Definitely worth contacting as you never know who is hiring. Be mentally prepared and don't lose hope if the first or second or more hospitals say no..

    Thank you for your helpful video! I am a US citizen and a first year med student in Australia and my school is found on the wfme website. I am currently in a five year direct entry medicine and was wondering when is the right time to take the USMLEs. Also, following the alternate entry program, would this mean that I will be a fellow/faculty in the US after residency in Australia? Finally, would it be a stronger case if I were to do a residency in Canada instead of Australia under the LCME scheme?

      Good for you that you are doing a clinical fellowship. If you get a full license in Canada it would be relatively easy to get a full license at least in a few states in USA.
      ECFMG comes only after USMLE steps 1 and 2 and has nothing to do with mccqe1!

      Alternatively, you might explore the possibility of pursuing a clinical fellowship, which can provide specialized training and expertise in your desired field.
      Additionally, after obtaining your CCT, you could also explore the path of directly entering the workforce as a consultant, leveraging your qualifications and experience.
      The choice ultimately depends on how strong a background you are coming with.

      In short yes. However, there are a few considerations here. After fellowship, you will have to secure a job or repeat residency. This will depend on how strong your accomplishments are in Urology and your CV in general. There are IMGs who have been able to secure a job as a Urologist without repeating residency. This is rare and the license may be restricted to that hospital alone. On the other hand, if you get into an alternate entry path then you will get an independent license with no regional restrictions.

    Is it possible to do a critical care job via this pathway? I completed an MD anesthesiology residency in 2013 in India.. After 3 years in 2016, I joined a critical care program(FNB) in India, thereafter I have been doing a job in the critical care department since 2016. I received an international award in critical care medicine in 2019 in Berlin, Germany, as I cleared the EDIC(European Diploma Intensive Care Medicine) exam with the first rank. Then I moved to the UK and now working with NHS. Is it possible to go to the USA with this pathway, as I have an international award in critical care medicine? I prefer to be working as a critical care specialist.

Is it possible to be faculty for the entire 4 years of AEP without prior ACGME fellowships experience or only with non accredited fellowships, how about the license do we need an independent one( which needs ACGME training) to become faculty during the 4 years.       Sir, will we be paid during the tenure of AEP as a fellow or faculty before getting the ABA license?
      And the data you have mentioned, doesn't tell if it applies to those opting for AEP sponsored by the program. Though one can assume that is the amount one would get if taken the primary USMLE/Residency pathway.
      Can you please clarify on that or forward any source of anaesthesiologists currently in the system through AEP and what their pay looks like?

    Hello Sir,
    Thank you for your informative videos, more importantly for giving people like me hope to pursue this path.I am an IMG with anesthesia residency from India I have completed my USMLE step 1/2/CS, yet to give my step 3. I have a question, If I secure an interview for an ACGME accredited Anesthesia fellowship will the program be able to give me position after I complete my Step 3. As most of the states have criteria to give a License with at least 1 year of residency training in USA, or PD can waive that off. Also can i ask for AEP during the interview. Sorry for such a long question.

      I missed your comment. As an IMG from India your fellowship will not be ACGME accredited unless you get into Alternate Entry Path. You should rather ask for AEP rather from the get go.

    what are the jobs available and are the salaries same as compared to that of a person who took the residency pathway?

    2:41 is it tough to match in fellowship program in usa after you complete residency in home country and how competitive is it?

      If you have completed residency in your home country then you send an application for fellowship (there wont be a match). There are places that take fellows out of USA and this can take two options 1. Non accredited fellowship or 2. Alternate Entry path. I have explained the latter in this video using anesthesiology as example but the general principles apply to other specialties.       It is competitive. The programs generally prefer those who are in the system (this could be a US graduate or an IMG) but sometimes there may be open slots or the program just would like to hire an IMG directly as a fellow.

    1) Am I eligible to apply for faculty positions Radiation Oncology?

      The good news is that American Board of Radiology offers a pathway for radiation oncologists who are IMGs trained outside USA or Canada. You can find the link for this in the description of this video under American Board of Radiology. However, in order to apply for a faculty position, you will need ECFMG certification which comes after completing step 1 and step 2 CK. Faculty position also typically requires USMLE Step 3 for independent license.

I also did my clinical research Programe for 6 months & also preparing for USMLE .       Although there is no official age limit, programs typically focus on recent graduates for clinical or research fellowship.
      On the other hand, with your vast clinical experience, you could try applying directly for a consultant position in places where they have a position open. I don't mean to say you would easily get it but what i am saying is it never hurts to try.

      Higher scores are generally preferable, but don't let low scores deter you from pursuing your application. It's worth noting that IMGs with lower scores have still achieved success in securing spots. However, in such instances, the other factors must significantly strengthen your application to remain competitive

If I apply now for the fellowship programme of 1-2 years later from my home country, Nepal, I can be enrolled as a fellow there almost coinciding with the time I will get the PR. I won't have any financial worry if my plan goes this way but I won't be having a PR at the time of applying for the fellowship. What is your advice over this plan of mine?

      Thanks. Alternate Entry Path is not easy to get into - CV must be outstanding with research and must have many accomplishments. If you are very certain that this is the case and your CV is very competitive then the scores does not matter. For residency it does. If you are not sure then scores are important.
