THANK YOU FOR THIS! Add bad advice from store employees to that too! Just went into a Sephora over the weekend where a very pushy sales clerk tried to tell me that I had to stop tretinoin because it’s only supposed to be for temporary use! I told them that is not what my DERM said and they had the audacity to keep pushing me. They told me to go back to my Derm and ask to make sure! I recently quit at Sephora and I agree with u. I was in Cosmetology school and want to go to an esthetician school that just opened here (never wanted to do hair in the first place lol!) I worked under people who had zero skincare, chemistry education at all! My major beef with the Sephora at kohl's was the fact that I asked if I could be the one responsible for the cleanliness of our store while the other workers focused on register since it was clear they hated cleaning and I love cleaning. I got my Barbacide certification so I know how important sanitization is and I was embarrassed to have friends and family coming to the store and seeing how gross it looked. The Sephora field trainer's response was, "though I appreciate your attention to detail, cleanliness is our last priority." I wanted to say, "yeah clearly!" The clean at Sephora is also appalling to me. Working there was the worst job I've ever had, ever!

As an older woman now, it seems to me teenagers are still using the same pointless products that I did in the 1970s but even more importantly adults, like parents and doctors, are still failing to step up and take skin issues and proper treatments seriously. When I was in my late teens I was able to get my own GP and she sent me to a good dermatologist who gave me an antibiotic lotion that knocked out my active acne in a few months. Later when I was about 30 I still had oily skin, my GP sent me off to another dermatologist who put me on Neostrata Glycolic Acid which was a brand new product and independent start up company at the time. I used that for a couple of decades. Now that I'm 60 I finally have combination skin so I've got a good routine for that. Anyhow my point was that effective treatment for acne has been around for at least 45 years but each generation of teens seems to be on their own with the folk remedies and drugstore stuff and harsh cleansers. Parents in my generation didn't know about modern treatments and tended to think you just grew out of it but why are parents still like this? Nobody should be struggling with active acne and long term scarring these days.

I am your age and I also struggled with acne in my teens and in adulthood. Many doctors were just dismissive and one told me to use methylated spirits, which we now know is incredibly drying and would make it worse. One was on the right track and prescribed antibiotics and a sulphur solution from the compounding chemist, which many people still find successful today. But I had angry acne in my mid 20s and saw a dermatologist who was a proponent of non shampoo use and oatmeal baths, which helped. Even he tried to blame me for using hair products and excess cosmetics but I never used leave in hair products and did not wear much makeup including foundation. But my experience is that acne runs in families and relates to allergies and a tendency to inflammation and hormones are a big factor. Mine only ceased in my 50s. My skin improved overall when I stopped using shampoos with harsh sulphates which must have been drying my skin every time I showered, and I used to wash my hair every day.

Around age 18-20 (about 7-8 years ago) I was one those people searching for how to fix my breakouts, even tried the apple cider vinegar toner trick. I used to think it was what I ate that made me break out more too, so I reduced sugary things a lot. Seemed to help a bit but what really helped - before aging came and stopped making me oily - was actually just washing and moisturizing, instead of slathering my face in too many acne products. Literally just face moisturizer lol. If I'd known that and that getting better sleep, reducing stress, etc, helps, my last years of high school would've been clearer LOL.

I had such a similar experience! My acne seems to be majorly triggered by dehydration, and the drying acne treatments alongside my refusal to use a richer moisturiser just made it so much worse. Once I cut my routine down to a good moisturiser, ceramide serum, and gentle cleanser, my skin started to heal from the over exfoliation and constant dehydration.
I still deal with acne, but it's gone from several new painful, inflamed pimples every day as well as white heads to just some whiteheads every few days and the occasional painful pimple. I've built up a routine and now use Differin to manage the acne that I still get, but my skin is better than since before I hit puberty, and all I had to do was chill the fuck out with drying products lmao.

I felt that way too - I thought just lotion would break me out more or make me feel more oily/dirty. The oiliness did contribute, very greasy T-zone by end of the day back then, but all I thought and was taught was "dry it out", so I'd use Neutrogena acne cleanser with salicylic acid, then ALSO slather my face with a Neutrogena spot treatment or some other spot treatment that also had salicylic acid (even stronger %), and go to bed with that on... LOL
My pimples weren't TOO bad, a few whiteheads or bumps, but my chin felt so rough from all that, right under my lip... Once I learned a bit more and only used SA stuff when I felt pimples coming in, it got better. Finally just having a "plain" moisturizer and changing my pillow cases more often made the difference, any pimple went away much faster and was way smaller. I love that you are plant based yourself and speak out about the veganism and skin health lies. I have been a vegetarian since childhood. I along with many other vegetarian and vegan teens struggled with acne and now I even have adult acne. so we already feel self conscious socially, but the diet we follow isn't curing our acne as promised by celebrities and vegans trying to recruit people by appealing to this concern. that is why the "veganism to cure acne" movement really rubbed me the wrong way ever since I first heard of people saying that stuff about diary, and dramatic before and after success stories on the internet by people with severe acne saying that nothing else cleared it up like becoming vegan, when no actual dermatologist will tell you that food will affect your acne positively or negatively. I feel the same with promoting veganism for any vanity reason, because as you said, when it doesn't work people will run further away from the vegan movement and remember it for disappointment and beliefs that aren't backed by science. why would they listen to the message if we spread BS about how it will make your skin glow and cure cancer? or if you are vegan/veg but dont look like a model with perfect skin, people will assume youre not "sticking to the diet" or that it simply doesnt "work" when it isn't about looks! most vegans aren't in it for the superficial reasons so I hate that it is marketed as this superficial diet solution. thank you for speaking sense to your audience and being the real deal.

When you’ve got people who are depending on YOU to make the best, and right choices for them! I believe that’s how Dr Dray approaches her philosophies. It’s refreshing! My oldest is 30 years old and took it about 10 years ago. She hats acne once in a while but no cystic acne. The dermatologist told us it is often hereditary. My husband and I had it growing up. I appreciate what you have said, but I do think there is a relation between acne and poor/no skin care. When I was growing up, if I washed my face daily and used an oil free moisturizer, my acne reduced. If I got lazy about that skin care, went to bed with makeup on my face or didn’t keep up with washing my face regularly, my acne increased. So, there is correlation, if not causation. So… that’s hygiene related.

when i was into all this kind of stuff (2016-17) i was so desperate that i tried it all. baking soda, lemon juice, all kinds of home made scrubs, aspirin masks (???), avocado masks, oatmeal masks, apple cider vinegar toner... thinking back on it really makes me wanna through up. ofc the culprit, the thing that made me break out horribly in the first place, was coconut oil, another one of those secret super weapons that were supposed to make your skin glowy and amazing lol

    • @hannal5512

I had forgotten about the aspirin mask😆 Lemon and soda, yes.
What breaks me out is oils, particularly oil cleansers. It took decades for the penny to drop. I suppose it’s because my skin was never so very bad so I could just live with it. Also having always had dry skin, I didn’t think oils could have adverse effects on my skin.

People who say that they were on Accutane but failed to improve should be informed of the following:
    1. Accutane success is dependent on the Total Cumulative Dose taken. That means most people need to take 120-150 mg for every kg body weight. If you fail to take that much then you are unlikely to achieve clear skin and are very likely to relapse. Dermatologists with a lot of experience treating acne patients will continue dosing until their patient is clinically clear, and sometimes that requires more than 150mg per kg body weight.
    2. Accutane (isotretinoin) is optimally absorbed by the body when taken with a high fat meal because the molecule needs to attach to a lipid (fat) in order to be absorbed. If you don’t take it with a high fat meal, it’s like you are only taking half a pill! Experienced dermatologists often recommend taking it with a bowl of full fat ice cream.
    3. If you take a full course of Accutane based on your body weight, plus you always take your dose with a high fat meal, then 80% of these compliant acne patients will achieve clear skin. These people will not require further courses of Accutane, BUT they will very likely have to use topical anti acne products to control minor occurrences of pimples in future. The remaining 20% unfortunately will experience relapses of moderate-severe acne, and will need to take another full course of Accutane. It is not known why a small number of people need additional courses of Accutane to treat their acne.
    4. At the present time Accutane (isotretinoin) is the only acne treatment shown to provide a “cure” (long term remission) for acne.
    5. Expected side effects during treatment include very dry/flaking skin, very chapped lips, nose bleeds, aches and pains in the muscles/ joints. Possible side effect of depression is debatable, patients should be aware and report any such feelings to their doctor. More patients actually feel a sense of relief and happiness as they see their skin clear up.
    6. One serious side effect to keep in mind is that Accutane (isotretinoin) can cause very bad birth defects in a developing fetus, so females absolutely must not become pregnant while taking this drug. Effective contraception is a MUST.

I determined that cow's milk was the cause of my severe cystic acne through trial and error. I also developed a lactose intolerance around that time. As a dessert person I noticed more acne occurring after a high sugar meal too. So changing my diet worked for my face!
Eventually used 0.5% retinol serum from the Ordinary that seriously reduced cystic acne. Not sure if added probiotics helped clear my skin but it was a habit I developed overtime. Didn’t use accutane.
Great video, cleared up some myths I have read online on this journey but it doesn't hurt to change up some habits to see what triggers your acne.

I know it's just anecdotal, but my acne does seem to be caused by diet. When I go on a 100% WFBP diet, I don't get acne. It's too difficult to stick to, though, so I inevitably start to eat some processed food again and start to get acne again. I also find that I'm really sensitive to coconut products.
I'm 41 now and still dealing with acne. I try to control my acne with prescription and non-prescription meds now that I'm eating processed foods again.

Love the videos and this one helped as always!!! Do you have some recommendations for permanent facial psoriasis, especially around the eyelids and inner corners 😢There is so much confusing information regarding psoriasis and mostly for the body not for the eyes... At this point, I'm just desperate.. My derm told me for example to not use mineral sunscreen (Right now I'm using make p:rem Daily Sun Essence) and not wash my face in the morning.

Hello Dr Dray. Do you have any clips on Perricone MD products? Do you you recommend their products on men with heavy facial wrinkles like me? Or do you have any better and less expensive alternatives? I love your clips and have been following your advice. TY

Please let me know your opinion on people using glycolic acid as a scalp treatment for cleaning the scalp. I see it all over social media.

The inkey list has a glycolic acid hair product. She's reviewed the other hair products in that line. I believe manes by mell has reviewed it.
