    I underwent a hair transplant of 2500 grafts on the crown. Its been 17 months now and there is no growth of the new hair which are visible to the naked eye. The doctor suggested that there is no need to take minoxidil or finesteride post treatment, so I did not take it until to date.
    Did the transplanted hair fall off due to not taking finesteride? The transplanted hair was taken from the lower back region of my head.

    Mind blowing explanation doctor.....I am 36 year old and just did my first hair transplant with 2500 grafts with DHI method......can I take finesteroid for a year and side effects is permanent or only at the period we take the tablet's?.....is it required to have more than one year?....or we continue only with minoxidil without finesteroid post 1 year of course?

      It all depends upon how your balding is, the state of your scalp donor and what your objectives are. In my experience side effects occur rarely and they are reversible provided the patient does not have underlying risk factors I have talked about.     Sir what are your thoughts on fluridil?, it is a topical androgen receptor antagonist used against mpb in czech republic and slovakia, could it partially substitute finasteride until clascoterone is available? Thank you.

      Only available and approved for use in 2 countries- Solak and Czech Republics, Eucapil is being used by many people in Europe alongwith oral Finasteride; but I do not see it as an alternative to Finasteride. I an eagerly waiting for Breezula istead.

    Hey Dr. Bhatti, at 13:00 you mention the tests you have patients in that symptom/age bracket to determine their likelihood of eligibility for getting on Finasteride. I am in that age/symptom bracket and have been on Fin for a few weeks now - is it too late to attempt to get such tests done since I’m technically not at baseline anymore? And if I were to get tests, what should I be looking at?

      Please get your levels tested. If they are normal, you will be reassured that you are doing well. If not, you will need to stop the drug- but that is very rarely the case if you are asymptomatic.

      @@hairtransplantinindia Will do, but I guess my main question is WHAT I should be getting tested. I know I should be looking for X amount of Y, I just don't know what those numbers of units of measurement are. Testosterone levels? Other blood factors?

      Topical Finasteride does not reach the follicles. It is a commercial product with no use.

    I believe I in one of your videos you mention to not take finasteride before surgery is that right? I’m having surgery in a couple of months and haven’t started taking it yet. Should I wait?

      Since after taking Finasteride , the real status of your donor hair gets masked. But do take it before the procedure if you have no qualms in taking it in the long term.

    Hello sir, my question is that 2 doctors from australia , from the hair loss show, says that finasteride 3 times a week gives almost same effects as of one mg everyday. Do you consider 3 times a week is an effective dosage?

      That is effective dose for maintenance only after 6 months of hair transplant.

    Could you elaborate on why you don't recommend finasteride under the age of 21? I'm 20 and considering finasteride because of thinning hair and receding hairline and i have balding in my family.

      This is since the secondary sexual characteristics develop by 21 years in some. DHT is the hormone which causes this. So I feel uneasy blocking the medication before 21.

    if somebody don't take finasteride or dustasride or minoxidil after hair transplant and he lives his life the natural way without medication, would in that patient hair transplant last or become successful.
    Only by following proper diet is it possible to make the hair transplant successful?

      If you wish to live life the natural way, let baldness take it's course!
      If however you wish a hair transplant you need to follow guidelines or the procedure will be unsuccessful and you will be unhappy with the outcome. his has been answered in the video I will not be comfortable doing a hair transplant on you. Bad result of hair transplant if due to non-compliance, cannot be reversed; and grafts lost in regrowth phase never come back they are lost forever.
      It is a battle your hair are fighting against DHT and if they are not well equipped, it is like fighting with sticks, when the enemy has AK-47s!

    If you stop Finasteride will you get gynecomastia... When I STOPPED taking it, 2 weeks later i had large gynecomastia tissues developing, they werent there whilst taking the finasteride?

      @@hairtransplantinindia Ok will do, my bmi is on the high side because i have alot of muscle, bf% is low, gyno is not visible, but large under the skin an tissue is growing toward armpit i can feel. Seems to lessen by the day. Also ED was subtly there. Also has lessened. Probably estradiol. thx doc!

    Greetings Dr. Question - Regarding your suggestion, of taking a dosage break for 15 days after a month of Finasteride to flush it out of the system, can this stop start Pattern be done regularly for years or is it just a one time test you recommend in the intial phase to test the effects on the patient ?     Hello Doctor, very helpful videos, impressive work.
    I am 41 years old from Greece and just had a FUE transplant 3670 grafts temporal and top area. Currently on Minoxidil/Finasteride lotion, Biotin 5mg and Saw Palmetto.
    Do you think taking oral finasteride is better and for how long? I am mostly concerned about the side effects since sometimes they can be irreversible.

      You should take Oral Finasteride. Topical Finasteride is a commercial product without scientific basis. It does not penetrate the skin barrier. Side effects are rare. I will be posting a video on how to avoid side effects of Finasteride tomorrow.

      @@hairtransplantinindia So Topic Finasteride is not very useful Doctor. I cannot use the normal Oral Finasteride at 50 years old cause side effects for me were also blurring eyes that could not be reversible. As you know at 50 y. old to go forward, testosterone will be every day less and less so, I don't know how to do now after the recent transplant. Now I use Minoxidil but not Finasteride. What do you suggest Doctor? Thanks

      @@patrickpat5277 Thanks for the query. Since you cannot use a DHT blocker, you can use 5%Minoxidil twice a day and hope your baldness is stable and that the grafts all grow fully. There is no alternaive yet to oral Finasteride.

    hello sir, I am 27 with a Norwood 6 scale baldness, and had a hair transplant procedure a month ago. Although my surgeon did not advise it, I have been taking finasteride right after my hair for self relief . Could you please comment on the dosage and duration of finasteride intake? I have seen some surgeons advise a moderate of 3-4 mg per week, usually taken on alternative days. Is it as effective as a 1mg/day dose. Thank you!

      Thanks. Micro-dosing is for maintenance and not for after a hair transplant. In the latter instance, Tab Finasteride 1 mg a day is mandatory for 6 months.

      Male pattern baldness is very easy to detect. You can tell a bald person from another who has hair loss from reasons other than genetic.

    Hi doctor I’m 17 year old and have male pattern baldness for about 2 year [ I did test and those were the results] I was wondering why you wouldn’t recommend a hair transplant other than the fact that it will get worse. I take finasteride and decided if I did one now I would do another one in 5 to 6 years.

      I would also like to stated that at the age of 15 I had a crazy high amount of DHT probably leading to the early baldness and it has now calmed down and haven’t seen a differences in my hair for about a year. And am willing to take all the precautions to make sure my hair stay healthy.

      I would do a hair transplant only if I am sure the donor area has very little or no miniaturisation, and that can be determined only after 25.

I have been taking Dutasteride every other day for a week, but I had to discontinue the treatment because it gave me low blood pressure, vertigo, and tinnitus. In your opinion, would these side effects occur with Finasteride, too? Thank you in advance! Filip

    Thanks for the video, doc. I've noticed that almost every single doctor talks only about ED when it pertains to the side-effects of finasteride. How about the other side effects, especially breast/nipple tenderness or enlargement in men? Is the probability of this side-effect higher or lower than that of ED? And if it does occur, is it reversible like ED or not reversible at all?

      That is since these relatively more commonly encountered side effects of Finasteride are not centerpoint in the patient's mind. However I agree the other sides need more clarification.

      You can but it is not an alternative to Finasteride which is a DHT blocker. Most people waste precious time experimenting with various things while their hair loss is happening at a fast pace. Taking good and timely advice of a competent doctor will save your hair which otherwise will be lost very fast as you dilly-dally. Finasteride though a magic drug, does not bring back lost hair.

      No. Since these DHT blockers are not meant to block 100% DHT. And baldness is progressive.

      @@hairtransplantinindia sir meri age 24 hai aur mujhe hairfall last 1 and half year se ho raha hai lakin abb hairline recede hui aur male pattern baldness area main density kam ho gyi hai, aur meri density sayad 60% hi bachi hai, tho maine finasteride use karna shuru kiya hai, tho kya mere next 10 year tak bhi save nhi rhenge sir😢?
      Agar 10 se 20% baal kam bhi ho jaenge tho bhi chalega.

      That is natural for the hairline to set back around adolescence. There is nothing you can do. It is natural.

    hello sir,thanks for such good information.I want to ask sir iam 27 years old and after transplantation my clinic advise me to use 10% minoxidil and 1mg finasteride for next 1 year but in my case 5%minoxidil is effective as i was using it before hair surgery,so can i use minoxidil 5%.And also can i stop finasteride after 1 year.Please help sir🙏

      10% is not required after a hair transplant. Even for hair loss 10% is not needed. Your doctor who knows your balding better will be able to advise you on stopping Finasteride. However if your baldness is extensive to my mind you will need to continue.

    Hi Sir,
    Thanks for the post. I am curious that all medicine have side effects including paracetamol but the frequency of intake is less for other medicines. In case of finasteride it is like taking medicine for life long. So does it affects the health over the time. Please advise.
    Also if we do not take finasteride after hair transplant. due to hair loss after the transplant will it look too odd? Does the placement of grafts in such a situation will overcome any situation in the future of hair loss due to not taking finasteride. Need your suggestion please.

      Especially for young people less than 30-35 years of age who have signs of ongoing extensive baldness you cannot wish away Finasteride. Technique has no bearing on this. It is the hormonal milieu in which the grafts have been placed. And no donor area is 100% permanent!

    Hi doc, Minoxidil stops my hairfall and makes my hair thicker but I get severe chest pain. I have seen online that doctors prescribe beta blockers with minoxdil. Can I get it prescribed by you?

      Medications like these should be prescribed after in person examination. There are so many indications where beta blockers should not be given. Please see your doctor immediately.

      Dude if you are getting chest pain go checkout an cardiologist. Minoxidil increases inflammation and mass in LV. Dont ignore it there's a research paper proving it.

      @@abhinaykumar39 I had same heart palpitations after using minoxidil 5 for 4 long years. Went to cardiologist and took ECG,echo. It came out normal. But stil i feel palpitations even though I'm taking beta blockers. I can see diuretics can also be taken. But which tab to take?

    Dr Bhatti, it would be good for you to refer to this article ‘Health Risks Associated with Long-Term Finasteride and Dutasteride Use: It’s Time to Sound Alarm” published by Boston University School of Medicine. It is a free access article which you can view from PubMed. There are several other side effects of 5 alpha DHT blockers which are not commonly mentioned by many hair transplant specialists. I would be very interested in hearing your thoughts about it.

      Never do I prescribe Finasteride in all patients. Everyone is not fit to take this medication. But then I do not recommend a hair transplant for this sub group.

      @@krishsarkar5034 Iagree Fiansteride is not for everyone. The other options are lifestyle changes that will remove effect of epigenetic factors on the hair. Other medications like Minoxidil do help but they do not alter progression of baldness.

    Sir I am using topical finasteride which is combination of minoxidil and in one of your videos you said there is one topical duastreide which might be more effective than the topical finasteride is it available in our country sir...is there any topical finasteride or duastreide in your experience which might effective to arrest the hairfall

    Sir from when i can start finastrated currently only taking minoxdil ... i had a HT 2 level bladness ... but little bit tension as i want survive my existing hair more 25 years..current age 28 ... i was suffering my hair issue from 18 years if age... when i turn 25 years old ... hair fall slow down to like 4 to 5 hair during shampoo ... but during this i have lost 25 of hair ... so i did a HT .. please tell can i take finastrated

      At your age if you have aggressively progressive baldness, you need to start yourself on 1 mg a day oral Finasteride. But check hormonal levels before doing so.

    I am already 36.5 years old and I have just had my second hair transplant done 6 days ago, do you think it would be justified for me Not to start finasteride. I have also never tried it before. Personally I think that when I first started to notice hair loss at the age of 24 I should have tried it then but not anymore at this age.

      Just to be safe there is no element of baldness co-existing alongside Sih pattern traction alopecia, you should consider Finasteride for a limited period of just 6 months after counseling with your treating doctor.

    -it is used a lot in Germany
    -In combination with minoxidil.
    Pantostin is medication which is used topical in the treatment of pattern hair loss androgenic alopecia or pattern baldness in men and women.

    Well, I’m 63 and I sure never needed finasteride before or after my hair transplant at age 54, but then I had no hair loss until age 50. I never lost a single hair in 10 years since my hair transplant.

      @@hairtransplantinindia Thanks, but it really wasn’t much of a storm at all. My dad had a full head of hair his whole life, but my maternal grandfather was totally bald, like a Norwood 9 or something. I was only slightly affected. Very minimally. It’s all genetic.

      @@hairtransplantinindia I have already started taking it. Took 3 doses every other day and not too long after taking the medication I would feel dizzy and lightheaded. I have since stopped the medication. Is this a side effect of Finasteride to happen so quickly? Thanks

    Hi Doctor, I saw your videos, the advantages overweigh disadvantages in using finasteride for people over 35.
    But here is another may be silly question.
    Does the increase in the estrogen levels due to taking this DHT blocker makes us a cowardish(darpok) like behavior? The reason I ask is I have gone through some extremely tough times in the past (for many years) and coming out strong just recently So this question means a lot to me?
    BTW, I just had HT few days back and the doctor did prescribe finasteride to which I am both skeptical and nervous.
    Please let me know?

      To prevent side effects of excessive estrogen, do not block most of DHT by taking strong blockers like Dutasteride. Also do not get into sedentary habits. What is your weight?
      Estrogen does not make you a coward. Look at Indira Gandhi, Margaret Thatcher, Sirimavo Bandaranaike, Golda Meir, Khaleda Zia, Benazir Bhutto, Chandrika Kumaratunga, Megawati Sukarnoputri, Angela Merkel, etc.

      I am sorry for asking the question in that way, I totally agree to what great leaders and people have accomplished.
      My weight is 70kgs and I exercise 2 to 3 times a week (resistance training in gym) and no sedentary life, but I also saw your whey/casein protein suggestions and to rely on natural proteins. This was an eye opener for me, and wanted to ask you what if I move onto organic plant protein supplements (soy free) because I am a vegetarian (no eggs as well).

      Hair fall ill continue. Taking Finasteride has done good for you.But it will not completely stop hair fall hen you carry the baldness gene. Why? Because, Finasteride does not block 100% DHT!

    Hello Doctor, unfortunately I was not given much time and effort in helping me understand about finasteride and help me feel at ease, so I ended up getting sceptical about finasteride. Hence I did not take it for year and I couldn’t notice a great density with my hairline transplant. But after watching your videos and few more. I understood that it’s something I cannot ignore if I need a good hair density. I was just wondering, if I’ll be able to see some improvements to my hairline transplant density now at least (of course I’m aware of the variables). I just wanted to understand how much of an impact it will have on transplanted hair and will it improve them in some way atleast after 11months of not taking it

      Finasteride will at this stage maintain your existing transplanted hairs (if your scalp donor was not stable) and ofcourse the native thinning hairs (if your baldness is still progressing). It will not grow hair that were lost (if they were) during the growth process after the hair transplant.

      Breezula will soon be in the market- expected next year. I am also eagerly awaiting it for my patients. Till then there is no workable alternative. If you have side effects, just stop Finasteride, apply Minoxidil 5% twice a day, and hope for the best!

      Blocking DHT upto 70% in most people on Finasteride is tolerated well by the body. provided the testosterone, DHT estrogen levels are no upset too much or the body will respond by Reflex Hyperandrogenecity. I have a video on this- last month's upload. You can watch it.

    Well side effects are real and scary for some people and usually last about 6 months. But it's a good idea to take it and test it because if the sides show up it will show up early in the course. Insomnia followed by Depression are the earliest symptoms, and if that experienced never continue the pill.another option is taking saw palmetto and try it. If you have sides to saw palmetto you would probably get sides from fin. And vise versa. Sides are similar.My experience.

      If a person has not been counseled well and he has a degree of erectile dysfunction present, like in the case of 40% of normal population in the 25-35 years age group, they are likely to e 'permanent'. Because they were present in the first place.

    You have talked about the alternatives to finasteride coming in the next 2-3 years, can you elaborate on them, and also there have been talks of trying to harvest patient hair grafts in a laboratory and grow them to a certain stage and then implanting them back into the patients scalp, is it a research that is going on or is it just a myth floating on the internet ?

    The Australian doctors from The Hair Loss Show YT channel recommend dosing finasteride 1 mg 3x week (Mon/Wed/Fri) versus the standard US dosage of 1 mg/day. What are your thoughts on that?

      Yes this dose may work for maintenance.
      I would not risk the wisdom of age old knowledge of Finasteride dosing, if I were young or if I had a hair transplant and I am in the first 6 months.

    Sir i have buzzed my hair like 1cm length for purpose of applying Minoxidil but due to Minoxidil I sometimes get itchy scalp so when I scratch my head I get tiny hairs which are like sprouts in my nails which are smaller than surrounding native hair what are these are these new hair and will it grow back they are so tiny and Shorter than other hairs usually all hair that are stck in my nails are tiny needle like hairs with black buld ends

Also these claims of persistent side effects have been well documented for many years and Merck was shown to even hide certain information from the public.

    I sure never needed finasteride and I had zero hair loss until 50. But, after that I had some hair loss and had a hair transplant at age 54. now, at 63 I never lost a single hair, and that hair transplant was back in 2012. no one needs that junk drug, finasteride. Just get a hair transplant. That’s all you need.
