- Yesterday, I was working one of my shifts and I came across a patient who had back pain. This is a very common complaint in medicine regardless of the specialty.
85% of back is musculoskeletal. But, it's the other 15% we have to focus our attention on. The red flags will help guide our management, which is what I want to focus on today!

As of the 16 of August 2002 my neck got worse with pain. I am 65 old man and had injuries caused by drivers running into me while cycling in 2015 and 2019.
On the 27th of the same month the pain got even worse, unbearble. I can't sleep, can only sit down for short periods, my symptoms lessen a bit when standing. When I try to lie down I feel a massive pain (like vice-grips around C7), getting really tight in the area. It feels like pulling very hard down my spine into my groin, left leg and foot. I also fell numbness (specially the groin and the left foot) in those areas and get pins and needles as I move around. It has affected my walking a bit. I haven't been sleeping for 4 weeks properly unless I take meds such as Lyrica, but it has side effects. I had MRIs of my cervical and thoracic spine recently and they show:
Ongoing neck pain
Cervical
The cranio-cervical junction is unremarkable. Cerebellar tonsils are normally positioned. No abnormal focal or diffuse signal identified in the cervical spinal cord.
There is normal cervical lordosis. No spondylolisthesis identified. No destructive bony lesions. No acute or remote fractures identified.
C2/C3: No protuberant osteophyte, disc bulging, spinal canal stenosis or neural foramina stenosis.]
C3/C4: Minimal broad base central and bilateral lateral disc bulging with associated minimal right intervertebral foramina stenosis
C4/C5: Broad-based central and right lateral disc bulging with associated moderate right intervertebral foraminal stenosis
C5/C6: Disc bulging resulting in moderate bilateral intervertebral foraminal stenosis
C6/C7: Right lateral/foraminal disc bulging resulting in moderate right intervertebral foramina stenosis
C7/T1:[No protuberant osteophyte, disc bulging, spinal canal stenosis or neural foramina stenosis.
Conclusion: Multilevel disc bulging and multilevel intervertebral foraminal stenosis
Thoracic
Ongoing neck and upper back pain
There is a 2.6 x 2.7 x 1.7 cm rounded soft tissue paravertebral lesion on the left side contiguous to the left T1/T2 intervertebral foramina. The lesion is T1 hypointense and demonstrate predominant T2 hyperintensity. There is a focus of T2 hyporintensity centrally. The lesion demonstrated patchy moderate enhancement post contrast administration. No oedema or scalloping of the adjacent vertebrae. No other similar lesions identified in the field of view.
A 7 x 8 mm skin lesion in the upper back in the midline at the level of T5 vertebra.
Multilevel spondylosis. There is a T2 and T1 hyperintense focus in the T3 vertebral body on the right side and this demonstrate hypointensity in fat saturation sequence. Otherwise no abnormal bone marrow signal. No disc bulging in the thoracic spine levels. No focal lesion identified in the thoracic spinal cord.
Conclusion:
1. Focal paravertebral lesion on the left side at T1/T2 spinal level. Feature suggestive of neurogenic tumour
2. Feature suggestive of T3 vertebral body intraosseous haemangioma
3. A skin lesion in the midline of the upper back at T5 spinal level.
I have seen an orthopedic surgeon but he thinks that is related to age and that it is of a mechanical nature with no radicular or neuropathic pain. He has recommended exercises and cortisone injections at L5/S1. I don't but the pain is so severe that it incapacitates me, it is debilitating. I am arranging to see a neurosurgeon as well but there are waiting times. I feel that mine is an emergency if I go by the symptoms.

I’ve suffered for the last 20 years after an accident at work. Yesterday my back went, last night I couldn’t stand up straight and today I’m struggling to stand up from sitting. I’m also struggling to go to the toilet.

A normal sore throat came and a day after that the backpains got so bad it felt like ı got hit by a sledgehammer in the back

This probably stupid question but can colon cancer make your lower back hurt .I can manipulate mine by moving around just started 3 days ago had a cologuard a year ago and it was negative Any advice would be helpful I am worry wart thanks
So lemme ask you this.. 1)do you have colon cancer ? Or a history of colon cancer ?
2) is it only the lower pain or do you have any other symptoms ?

I think you mean spondylolisthesis not spondylolithiasis
There are many reasons for back pain. A surprising one is lung cancer. It might start with neck shoulder and back pain. Some cancers too like ovarian, lung just to name a few.
No such thing as spondylolithiasis. Maybe you meant spondylolisthesis?
