Most of the time, the ump will point to the thrown equipment to indicate an equipment violation, for which the penalty is a fine, not disqualification from the game. After this warning, a second thrown piece of equipment is an automatic ejection. Occasionally umps will eject on the first throw if they deem it particularly egregious, but that's the exception, not the rule. Also, I think the body language (or verbal language) has to indicate disgust for the equipment throw to qualify for this rule.

Just a couple of days ago, I watched a video where Albert Pujols tagged up at first and took second on a fly out to center field because the centerfielder, Yasiel Puig, was so lackadaisical in making the catch. Pujols even made a point of mocking Puig for his lack of effort. Even though they played the game for opposite teams, Puig for the Dodgers and Pujols for the Angels, Pujols met with Puig briefly before the next game to kindly rebuke Puig for not respecting the game and his opponent. Vin Scully even had a field day roasting Puig for his lax play. "Pujols is saying, 'What are you doing? You're gathering wool. That's what you're doing.'" 🤣
He wasn't any better as a Cardinal. He has 50 more GIDP than the #2 all time guy. He would smoke a ground ball, and start jogging. I also saw Pujols, personally, get thrown out three times at second on a ball bombed off the wall on the fly. He. Just. Didn't. Hustle. I was thrilled when the Cardinals let him go.
He was slow his entire career but he had plantar fasciitis and a lot of foot problems later in his Cardinal days and into his Angel years. Remember he somehow scored over 100 runs every year for a longgg stretch until his run stopped at 99 one year lol.
