    Pudendal Neuralgia (PN) is a condition where there is pain or irritation along the distribution of the pudendal nerve. The pudendal nerve provides sensation and motor control to the muscles around the perineum.
    Pudendal Neuralgia can be caused by:
    *prolonged sitting activities (bike riding, horseback riding)
    *prolonged constipation
    *childbirth
    *squatting exercises
    *post surgical procedures (hysterectomy, pelvic reconstruction)
    *chronic infections (yeast infections, bacterial prostatitis, bladder infections).
    Pudendal Neuralgia Symtoms
    *Burning, sharp, shooting, stabbing type pain
    *Pain with sitting (which eases when sitting on a toilet)
    *Pain with prolonged standing
    *Vagina, labial, scrotal, penile, and/or anal itching, burning, or numbness
    *Clothing intolerance
    *Rectal/ vaginal Pressure (feeling of a pressure or an object in the rectum/vagina)
    *Urinary frequency/urgency
    *Urinary hesitation
    *Pain during and after a bowel movement
    *Constipation
    *Decreased sexual function
    What is Pudendal Nerve Entrapment?
    Pudendal Nerve Entrapment (PNE) differs from PN because the pudendal nerve is constricted or distorted within a fibrous band or tunnel. The most common areas of entrapment occur in structures around the pelvis or sacroiliac joint, a certain tunnel in the pelvis (Alcock’s canal) or around the base of the penis.
    With both PN and PNE, there is often a correlation with an overactive pelvic floor. The pudendal neuralgia or entrapment and pelvic floor dysfunction may co-exist. In other words, one may lead to the other; there may be an overlap of the characteristic pain. One type of pain may be more intense than the other. All of this adds to a complication with diagnosis and treatment.
    Treatment of PN and PNE can be complicated because of the slow healing nature of nerves. Treatments often include medications aimed at neuropathic pain and physical therapy to address pelvic floor dysfunction which can be a cause of PN and PNE. Physical therapy treatment will be aimed at releasing tightened tissues in and around the pelvic region to improve the nerves ability to glide and heal.

It is a good one of a kind product for Getting rid of Neuropathy minus the headache. Ive heard some awesome things about it and my friend after many years got great results with it.

    Thanks for sharing this info: a very informative talk. This condition is by no means limited to women. Many men, after prostate surgery complain of pelvic floor dysfunction and pain, usually unilateral. No one locally treats this. Well done.

    Best vid on this subject I’ve ever seen. My husband just injured the Pudendal Nerve on his mtn bike. He is in agony. Pain from genitals to rectum. Worse with sitting. Relieved when recumbent or standing.

      @@infiniteg7852 Hi. Just sitting on his bike seat (saddle) was all it took. It’s called Cyclist Syndrome. Usually happens to long distance riders, but apparently it can happen to anyone. 2.5 years later, he is still having trouble.with the pain.

    Wonderful explanation of the nerve pathway, nerve function and nerve issues when it is damaged.
    I had entrapment. Physiotherapy exacerbated the pain. I couldn’t lie down and sleep anymore. Conservative treatment was a bust. I had terrible side effects to Lyrica and Cymbalta. I finally had the rear branch of my right pudendal nerve released 3.5 years after it all started.
    I didn’t heal. I found out in 2016, that a metal titanium Filshie clip was on my rectum! It had been causing continued flare ups of my nerve pain for five years after my pudendal nerveless released. It was taken out in 2017. The other clip had gone up my intestines and was in ileum bowel under my bladder neck! I haven’t been able to wear pants with belts or tight body shaping panties in over 11 years!
    My rectum continues to spasm and particularly after prolonged sitting. I carried a toilet seat with me everywhere I went for years and I acquired tendonitis in my right arm. I’ve never found a sit pad or seat that helps. Do you have any Suggestions? I’m desperate.

      deana riggs Decompression surgery via trans gluteal approach as all my neuralgia had been rectal and some perineal.
      Dr. Stanley Antolak in St. Paul, Minnesota did the surgery. My inferior rectal nerve and a part of the distal end of the pudendal nerve on my right side we’re stuck onto a ligament and fascia.
      The Filshie clips were removed via laparoscopy and I’m sitting on a donut ring with 70% less pain.

      @@debba1976 Yes. The burning pain was finally gone. It was agony!
      I’ll always need medications and a donut seat. I use pain patches on my right butt cheek if I’m sitting and to calm the nerves down at bedtime.
      Insurance paid but only because I hired a registered nurse consultant to advocate for me. Doctors here committed fraud. They covered up the migrated clips and said I had vulvodynia! I had no pain in my vagina....it was all rectal.
      The surgery saved my life.

    I have had a weird discomfort in this area for almost a week and I was terrified it was testicular torsion (I learned what testicular torsion was a few months ago and I've been horrified and paranoid about having it ever since). I kept doing research and came across pudendal neuralgia, and everything clicked. It was actually a relief (since I'm not picturing my testicle being strangled anymore)! Glad to have videos like this with good information on the subject.

    The physiotherapist has created nice video because pelvis model along with white bone, yellow nerve and red muscles is good to convey pathology and where is nerve and why the pain in saddle area.

    thank you for explaining this. you describe my condition to a tee. I got my condition from stretching my right leg too low from doing the "happy baby pose" during yoga. Now I'm doing a lot of physical therapy exercises and every day is better.

    Thank you for this video. 6 months ago I had davinci hysterectomy and since then my life has been worse. So much pain, numbness, tingling.. all the way down my leg. I ran into an article on pudendal nerve entrapment and pudendal neuralgia. I now know what to tell the doctor, who has been of NO help for me.

    Thank you very much for the excellent presentation. I have had pain in the area for 30 years, I have traveled thousands of Kms seen 8 different Doctors and 7 different Neurosurgeons with this pain and none have mentioned the pudendal nerve.

    Can a cystoscopy hurt that nerve? I 've been having severe pain under my pubic bone that feels like a pinched nerve ever since I had the cystoscopy. That tingling / stubbing pain gets worse when I seat down or bend.

    Plastic model of pelvis in this video, delineates pudendal nerve very nicely. Victims can see pudendal nerve and ischial tuberosity better although ischial spine is not visible. Sacro-tuberous ligament and peri anal muscles are nicely visible for professional cyclists etc. who have saddle area pain due to pressure on pelvic soft tissue ( muscles and skin ) supplied by branches of pudendal nerve formed by multiple fibers ( thread like thin elongated nervous structures ) from lower part of spinal segments S2, S3, S4 of spinal cord.

      I've recovered from it before but I have it again 7 years later. I never got help with it the first time cause I was told it was interstitial cystitis so hopefully this time I'll get the help I need and recover faster.

    Thank you for the explanation. I am suffering with pudendal neuralgia and currently trying lifestyle modifications along with medication and about to go for physiotherapy. I really do not want surgery and hope to remain fit and active for the rest of my life.

      Perhaps it's the other way around- maybe you carry a lot of tension in your pelvis and that causes nerve pain, constipation and hemorroids due to straining on the toilet, thus making the pelvic floor even more tight- a vicious cycle. This is just a layman's hypothesis as I am not a doctor and I don't know you, you should however see a specialist in gyno/uro to rule out illness and then go see a physioterapist who works with pelvic floor dysfunction.

    I was a brick mason for 30 years doing a lot of heavy work on my hands and knees.
    i herniated L4 5 S1 2 After that I started having bladder issues And when I kegel really hard It sends a pain from my left pelvis down to my right foot I have none toes on my right side I think those nerves are damaged also. When I'd have a bowel movement it would hurt on the left side for hours.
    I'm retired now because of the pain it hurts me to drive it even hurts to plant flowers.
    I had four doctors diagnose me with prostate pain diverticulitis prostatitis and they've given me at least 12 different track infection test none of which came back positive.
    I was in physical therapy for 10 and 1/2 months they said it was a psoas problem, The doctor said BS gave me another track infection test and the doctor with two degrees could not tell me what it was, This is like the 8th doctor that I've had!
    What does it sound like to you Please help

    What do u mean by an orthopedic? I’ve never heard of a pelvic orthopedic, do they exist or go by a different name? I’ve had tons of pelvic pt & used to have a pelvic pain speacialist

    I got these symptoms after a cortisone injection in my symphysis joint, it just keeps getting worse. I was ok before, just sore in the joint in certain movements. Can you get this kind of neuralgia from an injection?

    I am 28 weeks pregnant and this has been an ongoing pain since my 16th week. It has been getting worse and I’m tired of my doctor continuing to tell me it’s normal. This pain is crippling each morning I wake up and nothing seems to relieve the pain.

      Avoid sitting on hard surfaces, use cut out pillow, take rest, don't get constipated, sleep or stand whenever you can. Please don't take turmeric pills during pregnancy as it causes uterus contraction.

    What are the differences between hypertonic pelvic floor and pudendal neuralgia cause I am confused in which i might have since i have felt alot of symptoms but they have similar symptoms

      they are very similar and treated the same way,pt,stretches,muscle relaxers etc....resistance excercises too help some people

      Basically if you can sit without pain it's not PN but just problems with pelvic floor muscles

      Leo If a nerve is being compressed or pinched, physiotherapist will aggravate the injury.

      Mackenzie Meador have you ever heard of a piece of self-myofacial equipment called the pso-rite? I have been using it for about three weeks now and have relieved a lot of the pain I have in my pelvic floor muscles. I feel as though it has reduced some of the urination due to the tightness in those muscles possibly compressing the pudendal nerve. However, because I tend to excersise those muscles a lot, its hard to tell if it would reduce it all together if I stopped working out. Its a catch 22. Here is a link, in any case, it's been worth it

      "spark note"??? As in light a match to the presentation or illuminate it for easy reference. I'm 70 years old, live in Alabama so now you know why I ask.
      I have had two lumbar fusion surgeries and now the surgeon says I need another. After those two surgeries I was informed I needed both hips replaced.....two more mistakes. So please pardon a dumb question, but I'd rather ask than have another surgery.

    Does this also cause numbness from the belly button down towards the vagina? I did have extreme pain in my pelvis two days ago but now it numb. also it feels like pressure every time I lay down.

    I’ve seen a PT before and I have gotten 85% better. I am currently working on strengthening my muscles again and I feel it is making a difference.
    My PT never touched on whether or not shoes/feet could have something to do with it. I have fallen arches, could this irritate my nerve ?

    I have a feeling of intense heat on the left side of the vulva and also the low buttock (sometimes spreads a bit down the leg). I also feel that the area is swollen or there's sth there.
    Problem is that the symptoms matched with a bacterial vaginal infection (that was healed) and doctors keep on goin around it and repeating gynecology exams over and over again.
    But the burning/heating happens only on the left side (gets worse after sitting for a long time or even lying down...just standing up straight seems to help but i Still feel the "fullness inflammation feeling".
    I had to give up of my job abroad causs i couldnt stand it and i started to get depressed.
    I feel like if i relay on doctors ill be in like this forever. help!

      That’s not prudential neuralgia. It’s classic genitofemoral pain/neuralgia. Google it. The genitofemoral nerve innervates your vulva, buttock and the inside of the top of your leg.

    I started out having extreme pain in my right testicle and then a few weeks later I felt an extreme pressure feeling on the right side of my pelvic floor. doctor said the muscles on the right side of pelvic floor were extremely tight. The nerves in the pelvic floor on the right side or compressed so bad that my entire right side of the body from my right foot all the way up to my right ear tingle with numbness and electricity feeling. is this normal to have happen?

      I suspect that my sacrum sticking more out might cause this but all the ortho that I have seen keeps saying I was born with a sticking out sacrum. I believe it is muscle tissue related

    I started having numbness around the vaginal & rectal area & thought it was sciatica at first, but after hearing about this, it makes A LOT of sense. I had exercised Friday, doing lower body strength exercises (IE side lunges, squats, etc) & that's when it started the next day. I'm hoping it's just irritation to the nerve (also treating UTI at the moment though as well with Cipro which scares me 'cause they say it can cause nerve damage, although I've taken it before & never had an issue like this). Gonna see a doctor soon or try to do some stretches/physical therapy at home & rest it to see if it gets any better. Thank you so much for the informative video!

      I wonder if your UTI is truly a UTI! Pudendal neuralgia symptoms can mimic a UTI with frequent urges to urinate and urethral sensitivity! That’s how mine started. I got a few UTI pee tests during obvious symptoms and they both came back clear... after troubleshooting that’s what brought me to a PT specialist who suggested pudendal neuralgia and all the areas of pain checked out!! This was a huge helpful distinction for me. Just make sure you get a proper test for UTI. Best of luck!

I did have a really bad UTI at the time when I posted 'cause they did a test & said it was very significant & with what was going on (IE blood in urine) it definitely was infection. But I was dealing with weird feelings after things cleared up like you said with feeling like I had to go still & I still, on & off, deal with lower body discomfort but it's not anywhere near as bad as it was. I don't know if I aggravated a muscle in my lower back/glute region & it was putting pressure on nerves around there, but I feel 100% back to normal, just occasionally get flare ups if I overuse certain muscles or do certain exercises. I'm glad you found out what it was causing your issues & I hope it's manageable for you! Does your pain come & go, or is it just constantly there?

    I’m 42 I had a total abdominal hysterectomy in June of last year. The only thing left behind were my ovaries. I have no pain whatsoever until I have an orgasm. The pain is generally deep and goes up the right side of my vagina. I’m wondering if this could fall into the category of Pudendal neuralgia?

      The surgery took me about a year to recover. Had I had it earlier, I believe I would have fully healed.

      @Expensive stuff I didn’t heal well because a metal clip was left on my rectum. It was removed 2017. I needed a third surgery and it was done to remove a second metal clip that had migrated in 2018. I’ve been able to wean off three meds.

    I had kidney stones removed and ended up with chronic pelvic pain syndrome. I've got pain in my scrotum my bladder my anus my perineum my lower back my tail bone. I'm trying to help people as I'm also going through this condition very badly.       @@jgwentworth2696 The most important thing was the release from internal massage. You can get a tool that may let you release yourself. It is hard plastic and in the shape of an 's'. I am a "clencher" which is the worst thing to be when your have PN so I have to conscientiously not clench. "Reverse" keguls help me too. Nerves love to move so she has me doing a lot of stretching daily (also very important). Lastly, NEVER sit on hard surfaces, take your pillow with you, don't be embarrassed. You are trying to heal the nerve, not torture it! That last one if from experience. Hope this helps.

    I have been dealing with horrendous pain for 3 yrs... I think you just diagnosed me. What kind of a Dr. Can I go see to get this treated? I have been to an orthopedist..my obgyn and a proctologist. I have an anal fissure and struggled with hemorrhoids since age 13. I always thought this was from hemorrhoids because it would go away but 3 yrs ago it came back and never left. Pain is awful around my period. Nobody has been able to help me and I am starting to lose my mind...I'm 38 and can't imagine having this pain for the rest of my life. The pain runs all the way down my left leg to my heel.

    It has ruined my life. Endometriosis surgery trigered this horrendous pain. I've had it since I was 24. I'm now 50 this yr. Don't see just any surgeon. Do your research. Unfortunately there are no surgeons who can j help me in my country. New Zealand is 20 yrs behind other countries tries when it comes to pudendal nerve entrapment. And the pain teams are even more backwards. God help me cause as I get older I'm loosing faith in this world.

      @Jim Puzycki I tried Clonazepam but it just made me depressed. I occasionally take 1/2 5 mg tablet when my pelvis is really tight. I've also tried Baclofen,for muscle relaxation but it gave me the most vivid nightmares. Everyone reacts differently. Thanks for replying. Hope your day is going OK.

      jo massey It might not be a surgeon you’ll see. Start with a urologist, gynaecologist and neurologist. There are some specialists in America, France, and Belgium that may know how to evaluate your case.

      I'm on Gabapentin (Neurontin). 3600 mg a day. It has been a god send. I would be unable to function without it.

      No mri does not usually show the pudendal nerve it’s a very hard condition to get a diagnosis which is another reason why it’s sucks so bad for many of us we don’t even get the closure we need to move on and improve

    2.mirtazapine 15mg
    3.duloxitine 30mg-30mg(60mg) gastroresitant sustain release
    4.stalopram(esitalopram10mg+clonazapine5mg)
    After pain go away reduce the dose.
    Meditation+excercise eliminated that problem(Mentally not physically remember)

      @@dawitshewangzaw2144 No, I've had it for years, but I'm now working on exercises that'll hopefully free up the pain. I have noticed changes since starting the exercises, but it's only a stronger tingling. They say the first few/weeks months it can get worse but then it's meant to get better. I'm only 6 weeks into the course so far and you're meant to do it for at least 6 months so we'll see how things go.

      Thank you so much for replying doc. I visited your website and honestly doc the fee's listed over there is my one month salary. I live in a third world country, Pakistan where we are going through massive crisis. I'm honestly telling you but I can't pay that much and I'm continuously in pain. Please if you could just give me 20 mins of your time so i can tell the doctor's back home that what's really wrong with me. I'm lossing the best days relationships of my life. I'm 27 and I'm loosing it. From the last 2 years I'm in the pain and right now I'm sitting in a small tub with hot water filled in it because there pain. A lot of pain. Just 20 mins so i can tell my Doc's that this is how we can treat this condition because they dont know what to do. I've lost around 16 kgs because I can't eat anything. No meat nothing. Please if you could help me. I promise I'll pay you the fees once I've that much money but it's almost more than my one month salary right now. I've to buy medicines and run a home in 350 dollars.       I have tried this and had mixed results. It flared up some pain but gave relief in some areas. perhaps it only targeted one branch. I'm not sure, i think i would like to maybe try again in combination with some of this physio. this condition is hell

    Hello i am suffering from penile numbness since more than a year, actually i was sitting on a PC at the corner of the wooden bed and one ay i felt numb penis so i got up and immediately the numbness returned back, but after. few days i felt electrical shock in my perineum and i lost my penile sensitivity , now my scrotum is numb and penis an poor erections , although i have errections but not like before and no taste in intercourse. kindly help me i need some exercises to get it back. i believe some nerves pinched actually i lso developed back issue so i developed paresthesias in the legs.

      After shingles the only thing that work for me was Tramadol now I have the pain the phn and that area and runs to my sciatic nerve in my foot
