Thank you Daniel! Thank you Vincent! While a lot of your info goes over my head you always include guidance for laypeople like me. It is so appreciated!
Hello and once again layperson here but your updates are still excellent of course! I have been listening to Office Hours and these clinical updates for maybe 3 years or at least 2.5 years. My family and close friends now ask me for advice!
I need to get my 2nd shingrex soon soon and then will follow up shortly with flu/COVID. Thanks for your comment about the new booster: my husband and I got COVID in early August our doc said 4 months but I think before Thansgiving is wise. My husband sings in a church choir! We are both 65 with mild? comorbudities. Oh yes both times w/COVID I got Paxlovid immediately and my husband did this past time.
UK doesn't test for Covid/monitor waste water and Boris Johnson persuaded news outlets to stop reporting on Covid, because it was too depressing. Those of us who wear masks in crowded places are still seen as weirdos, because people don't know about the surge. Thanks for spreading the word.
@@suzannelooms7658 I’m so sorry, that’s super tough. My friend lived in London until about a year ago and he had Covid at least 5-6 times. It’s awful there’s so much stigma to trying to protect your health by masking.
How fabulous that your first correspondent had managed to go nearly five years into this pandemic without contracting covid! Well done that person! I do wonder how much their life has been curtailed while avoiding it though.
Speaking as someone who has also not gotten Covid yet. It does definitely curtail it but mainly due to the attitudes of others towards masking. The social pressure to unmask is enormous.
I too have not had Covid to my knowledge and have been vaccinated and I am in my 60s. However, I rarely wear a mask and travel a lot by plane and go to parties and lectures and plays in a large theaters. My life has not really been limited frankly. For me I have to balance living my life to the fullest and been reasonably wise about Covid.
@@lismartinez5363 yeah so everyone is not you. There’s about 10% of the population that is genetically resistant. Sounds like maybe that’s you. However there are a lot of folks who are getting sick multiple times a year and that is affecting their health. So awesome you get to “live life” maskless. How about a little empathy for those of us at higher risk who are subjected to people who run around spreading their asymptomatic infections while enjoying being maskless.
My older son has never tested positive for Covid either. But he’s the only one in our family who has managed to stay negative, showing Covid 19 is just part of our lives now. Thanks again to Daniel for explaining that Paxlovid is a real benefit to reducing risks of severe symptoms and the chances of long Covid.
Thx Doc' for taking time to do these updates...so important for us clinicians and lay' alike. Re: you having 4 vacceines at one x, whoa! , is that recommended? by what organization?
There is so much reporting on increased sexual violence toward women and young people in DRC, I am so surprised that you did not mention this possible association with mpox, or at least acknowledge it. You changed the subject toward “pets” after the question of the age group was brought up. Could you address this issue?
I have been listening to these 'updates for several years...and these two docs can go into detail as time allows' they try and keep this offering to 30 mins and some commentary has to be culled...sadly this is prob the case.
It will express modified spike anchored to the outer surface of the membrane, which will attract cells of the immune system (eg: macrophages) and be taken up and degraded, resulting in peptides of the virus antigen being presented in an MAC to activate the adaptive arm of the immune system. Host cells expressing modified spike will also be attacked by the T cells (natural killer T cells and later cytotoxic T cells. In either case the fate of the cell sealed, a transfect host cell dies.
I have a friend who had severe long covid after getting sick in 2020. After many treatments she got her shots in early 2021 and most of her symptoms went away.
The facts are so difficult in social situations. I'm 83, I am somewhat overweight but not yet obese, I have asthma and diabetes. When I don't wear a mask people treat me normally and I run the risk of infection. When I wear a mask, people avoid me in public places and in personal interactions they accuse me of hysteria. I am currently in a diet group through my HMO and the room, of course is indoors, no windows and with normal air conditioning. I am required to show up weekly for 16 weeks. Failure to show up and complete the course or lose weight goes on your medical record. Want to bet that someone in the group eventually spreads something? Isolation is unhealthy and impossible. If that does not get me then the trips to the clinic where I am getting physical therapy will eventually get me this winter. Few people in the waiting room wear the masks that are on offer and are recommended by the HMO. And of course, there are the lines at the supermarket where I sometimes forget to mask. I have been thoroughly immunized for everything according to CDC guidelines, but I bet my HMO won't give me vaccines twice a year. In April I will ask again. I feel like the ax will fall this winter but also truth to tell, I only have had one infection thas was pretty bad way back when everyone in hospital was dying. So when I got sick two weeks after my first covid vaccination, and my home blood oxymeter was reading in the high 80s whenever I got up to eat or go to the bathroom, I stayed home and increased my asthma meds and stayed in bed for a month. It scared me! I sure don't want flu, or covid again either. Humans being human and acceptance of science being slow, I think that some rotten respiratory disease is bound to get me this winter. I've been remarkably free of flu for years, and only have gotten covid once before N95 masks were available so far. Can this record stand up to another winter?
I got the high dose flu and GSK RSV last Friday. Holy cow! Had I known, I would have scheduled them at least a week apart and would have gotten Pfizer’s instead. The RSV one caused extreme shoulder pain. I was close to going to ER for pain shot. I’ve concluded it was the adjuvant. I won’t be getting Shingrix now. Same adjuvant. It’s been a full week and still hurting.
It may not be the vaccine that has caused your pain but the location regarding where they injected. Consider speaking with your dr about this. There is something called SIRVA that can occur when injected too deeply or too high. A nurse friend of mine had this occur and she ended up going for physical therapy for a short while.
Thank you both for another excellent video/podcast. As someone who has been part of the ME community for decades, I would just like to say that we did not feel that any light had been taken from us. As one of you said, "what light?". We were heartsick that so many more people were joining our ranks. We also had mixed feelings and could not help but feel a little hope that research money would finally find us. We did not feel resentment, though I suppose it's possible that someone somewhere, at the end of their tether, expressed something that was probably closer to twisted envy as everything we have always been denied was on offer for another group of post-viral patients. I assure you, it is far from a widely held sentiment. We've been criticized so much over the years, perhaps I am a little sensitive on the matter. Anyway, what we mostly feel is heartbroken. Keep up the good work! Love and gratitude from Canada!
Thank you Daniel!

More wisdom on polls:
Polls are like balloon animals, they're full of hot air and twisted in any way the maker wishes.
NIOSH certified N95 respirators are most effective for use in poor or non ventilation locations.
Looks like Daniel has lost weight over time (based on face). Hope that's all it is and he's well.

When will a cynical attorney sue a clinician for not giving Paxlovid...it seems only a matter of time. I suspect that would guide treatment decisions.

That’s extremely sad that this could be a reason. They were saying it could be a contact spread from the pets as a fomite (furniture, clothes, etc).
Is it possible autoimmune mechanisms can be linked with ME/CFS at least in a subset of patients?
What happen with a lymphocyte cell if mRNA transfect it ?

I have heard of folks getting it and dying not folks I know but folks who close friends know and our son (34) a friend our age. (66) up in DC.
