That was all very interesting, thank you.
One thing I dislike a lot is when professional people call the stopping of potentially harmful drugs a "holiday". It is nothing like a holiday, waiting to see how much your bones deteriorate before being put back on the drug. It's condescending to have people call this time a "holiday".

Thank you, Margaret, for all your fantastic work on osteoporosis. As a "young, old" woman who had been on bisphosphonates for 9 years, then a holiday for 3 years with regression, and reluctant to go back on the drugs, I find this presentation invaluable. With a good education on all the details and nuances, we are able to come at this condition from a spirit of collaboration instead of fear! Thanks again.

I was put on fosamax when I was in my 50s because of osteopenia and was on it for five years but then when had oral surgery and needed bone graft because I was told I had lost jaw bone and jaw bone was infected from having been on that medicine. So I have been afraid to go on any meds since but now have moderate osteoporosis. But I’ve very scared now to go on Prolia or Evenity or Forteo though my endocrinologist wants me to take them. But I don’t think she is appreciating my concerns. Doesn’t seem like there are safe drug options yet for those of us with osteoporosis

Just came across this. I was diagnosed with osteoporosis but I've been doing lifestyle change and weight lifting and actually bone density showed improvement between first scan and second. Since then (2019) my gp has denied allowing me to get another dexa. Plus I was denied a referral to an endocrinologist. The latest thing is to tell women "thyroid issues are gp issues and no testing is necessary other than TSH". Regarding the "dreaded osteonecrosis of the jaw" - both my periodontist and dentist strongly (STRONGLY) recommend Not taking bisphosphenates. I have two implants, about to get a 3rd. My gp said, seriously, "would you rather have your teeth or spend the rest of your life in a rehab facility because you broke your hip." With the way women are treated it's no wonder we don't trust the medical profession, and the side of these meds are awful. Plus they're also pushing statins which are also Not recommended for people with osteoporosis. It's depressing how one medical specialist doesn't talk to another, and the whole situation starts feeling like a medical con. It's time for functional medical doctors to be covered by insurance. My mother was on bisphospenates for years. She suffered terribly from Gerd and died from a sarcoma at the base of her esophagus. Yet no doctor connected the dots. Docs are not looking at people as whole bodies. That's a crime.

I feel like she glosses over the GI side effects of some of these drugs. I took Fosomax for several years but the heartburn that it caused made me absolutely miserable on a daily basis. That's the reason I haven't started up on another drug treatment despite my DEXA numbers getting progressively worse.

Fosamax is a nasty drug with equally nasty side effects. There are better, safer and more natural treatment options now available.
“Fosamax creates thick, but brittle bones. In October 2010, the FDA ordered Merck to change its drug label to reflect the bone-fracture connection. Fosamax use may also make fractures more difficult to heal.”

These drugs actually stop normal bone breakdown and rebuilding. That's why the jawbone and femur - the most heavily used - fail to build new bone and disintegrate. Side effect? WTF? All that happens is a crust forms on the outside of bones to make some of then appear "thicker." This whole thing is a pharmaceutical myth that it helps most people (women). It might be advised for someone who has an imbalance of bone formation, but that really really rare. The rest of us are being told we should be on these actually bone-destroying drugs. We get taken off for "vacations" before it's evident how much bone destruction they do.

Why no mention of the newer osteoporosis injections Evenity or Romosozumab. This is s very expensive medicine to treat osteoporosis, and I understand that even with this expensive treatment from Amgen, after the treatment is stopped with the 12th monthly injection routine, the new bones that were built up during the treatment starts to decline quickly. SO what is the use of spending a fortune on this kind of treatment when it also does not last in terms of it's effects on building up newer bones and avoiding fractures in the future?

My first Envinity shot at the Dr. office was a Monday.
That evening my left jaw hurt so bad. And I I hurt all over. So bad that I called them because I have RA rheumatoid arthritis. I said I will never take that again no more shots. Because I felt like I was In total flare. On Wednesday I went to my Dentist appt. I told him I got some kind of a shot on Monday and my jaw was killing me. He canceled my tooth extraction that day and said we don’t know if you’ll heal because you did that shot. By Friday I was back at the Dr. office at his request.
He told me my jaw could not possibly hurt. There was a 0.1% chance this could happen and that’s after many shots. And all this pain was from an RA flare. I told him. Well my RA flare started last Monday evening.
I said, OK, let’s go with what you said. I’m not doing the shot again. RA is bad enough I do not need anymore pain. And 3 disks were messed up and I am waiting on back surgery. He talked me into doing the shot one more time but not for a couple months so I said OK.
it took about a month and a half for all those symptoms I had to go away. It was awful stupid me did another shot.
My jaw still hurt, but not as bad my left thigh was killing me. With the second shot. He reassured me that this was not from the shot and I reassured him that it was my left thigh, muscle bone, whatever still hurts and top it off my neck that I had gotten surgery on last year. It no longer hurt it was killing me. Like it never hurt before. Mostly at night. It’s been hurting so bad I made an appt with my neck dr. In two weeks I’ll get a neck x-ray. Before I see him. This Dr had fixed my neck and I had no more pain. He put screws and rods in my neck and in two weeks they fell out due to brittle bones, so I have no more screws or metal in my neck .
I am afraid to lay down at night. I told him I’m not doing those shots again and he suggested proleia . . I’m not doing any shots ever again for my bones. My sister did Rosina’s for years. She’s paid thousands of dollars for her teeth because she has no bones left in her mouth.

When I woke in the morning I had extreme stiffness and difficulty walking and moving throughout the day. I am 64 and felt like I was “old old”. I don’t know if this is common or not, but it was uncomfortable enough for me to chose not to continue with further injections. I am now on HRT in hopes that this proves to be a more effective treatment plan. I am told I should wait 2 yrs for my next bone density test to see if HRT helps. Very discouraging to have to wait and see, but I am hoping that my results are better in time. Maybe in 2 years there will be better alternatives.

Diet and expertise will not help much if you are in estrogen deficiency also known as menopause. We need to make estrogen replacement standard at menopause like it used to be before the flawed 2002 Women’s Health Initiative study. It was bullshit. They blamed blood clots and breast cancer on estrogen when it was actually do to starting women over 65 on HRT. HRT is healthy when started within 10 years of menopause. The sooner the better. After that the arteries have hardened to much an their can be an increase I. Blood clots or other heart problems. There is no evidence that HRT causes breast cancer. More breast cancer happens in older women weather on HRT or not. It is a condition of getting old.

Next morning hip replacement. I was buzzing along great went to post op found another fracture lower so put on no weight bearing until Aug 22nd. Says I’ll be good to go??? What does that mean? I take a multivitamin/ mineral daily. Boron but no added Ca just dietary . D3 + K2 any suggestions. I do all exercises not weight bearing plus arm shoulder weights . The doctor claims that the side effects are rare . Well, not according to the people that share their experience on several osteoporosis groups on Facebook. Many many had to quit the drugs because of the side effects. • @margaretmartinpt
      @margaretmartinpt  3 года назад +12

Dr. Rubin quotes the incidence of side effects in aggregate against the total population of people taking the medication. She is careful to quote the facts. The reason you might see a higher incidence reported among Facebook groups is because the people who do have a side effect spend time on these Facebook groups while those who do not (the majority), do things other than spend time on Facebook (in those groups). If they are doing well with the medication, why would they frequent these groups?
Another point to consider is that people often experience side effects but they might be caused by sources other than the osteoporosis pharmaceutical. The "side effect" might be from another pharmaceutical. a supplement, inactivity or general decline.
I have treat many patients on Prolia and rarely see the side effects.Thank you.

    • @CloverHikes
      @CloverHikes 3 года назад +7

The incidence of side effects is studied and reported. Of course, anecdotal data from a Facebook group is going to make side effects seem much more prevalent. People with side effects are often looking for other methods to treat their osteoporosis or looking for others with whom they can commiserate. Frankly, I think FB has been a detriment to treatment. I see many who refuse prescriptions because they have been scared out of proportion to actual risk. I’d bet most of them are taking a more common medication for something else, and aren’t even aware of common side effects.

I respect having Dr. Rubin on but to ask why doctors would prescribe something that will cause fractures, well, that has an answer that I should think most doctors can guess. Most patients today feel doctors' hands are tied by drug companies and that they get told in administrative meetings that this is what you give patients with certain problems. And that there's only so many treatments you can give and that's that; no room for their own thoughts on the matter.
I don't believe in conspiracy theories, but you tell me. How much wiggle room to doctors have today to speak and think for themselves?
Also in one FB group I have only been in for a few weeks, I have already heard of one person getting osteonecrosis and dying from it, due to these drugs. How "rare" can osteonecrosis be if I have already found out about someone who died of it. after just being in a group for three weeks?
The medical definition of "rare" seems to be wildly inconsistent.
On top of that, if you don't give people in their 50s these drugs because they don't fracture, but they do improve bone density for people in their 50s, doesn't it make sense to think that the reason fracturing happens to people in their 60s is because bone density was decreasing progressively during the 50s? And wouldn't drugs help bone density so as to keep fractures from starting in the 50s to begin with?

    • @attop21

In US, our healthcare industry is for-profit, meaning their No1 goal isn't healthy patients, but $$$$$$.
      No conspiracy needed. It's fact.
Big drug companies send out their sales people to push their drugs on the doctors, wining & dining them, & in turn, they push it on their patients.
Many patients say that their doc never discussed possible side effects with them when they handed out these prescriptions. To me, personally, with such SERIOUS side effects that these drugs have, that's downright immoral!! What ever happened to "Do no harm"???!!
It's not illegal to have doctors own stock in these drug companies, & knowing that these drugs are serious poison to their patients.
This is what for-profit means. Dollars over life, over health.

Yep, there is no evidence for bone quality. The only evidence is for bone density and density does not equal strength. We can easily buy Strontium at the health food store. Strontium increases bone density and is somehow known not to build bone quality. DEXa scans only measure bone density so there is no way that the Pharma companies could have proved that their drugs increase bone quality=strength with flexibility.

The interviews with Dr. Rubin are so helpful. I'm in the class of women who are early 50's and diagnosed with osteoporosis. I've been active my whole life with both weight bearing and weight resistance exercising. Though I'm on HRT now, I am not confident this is enough, and I feel like I should seriously consider taking a pharmaceutical. From the video, it sounded like Evinity is not a 'first line' drug for someone in their early 50's. Did I misunderstand? Evinity sounds like a good option for me.
One other comment, would you consider an interview with someone who can discuss the Echolight REMS testing vs the DEXA? I had the REMS done recently in NC. My lumbar spine T was -2.5 (vs 3.0 DEXA) and femur neck was -2.5/-2.9 (vs -1.5/-1/8 DEXA). DEXA was done 9 months earlier. Thanks for all your information!

Genetics, what kind of life style did you have growing up and into adulthood? Did you drink alcohol, or lots of soda? Are you big into keeping active and into weight bearing exercise, even walking up a hill is great for bones.
Your bones start to decrease in density in your 40s.
Fractures of the spine can happen to even normal bone!
Two things to know are not just bone density which is measured with DEXA but also the quality of bone--how well mineralized are your bones and there's tests for that that rarely are done automatically by your doctor & often you need to request them.
      There's so much to know!

Took D3, k2/m7, magnesia. Stood ok two years. But now I’m 83 and going bad again. I don’t know what to do. Plus I’m on S/s and the drug is my cost 1200. Every six months. I can’t afford it. No help. There are different kinds of Cslcium supplements. Eggshells are the worst. Not easily digested and absorbed.
If interested in Calcium supplements, take Calcium citrate. It's digested betterand more absorbable. The best is hydroxyapatite. It's a form of Calcium that also has other essential minerals attached in a crystal form (from what I read) and it's all good for the bones.
Stay away from too much salt and soda pop, if you liked those things. Calcium doesn't do well with those things.
