Prince of Persia: Sands of Time is a masterpiece. When the Prince dies he goes "No, no, no... that's not how it happened" and I love those details so much.
My favorite from Max Payne is when you're in the elevator and if you shoot the speaker to stop the music he thanks you for doing so. Amazing game series.
And Number One, WhatCulture. In every one of their videos WhatCulture has consistently spoke directly to the viewer, not only breaking the fourth wall by being a narrator but even giving stories behind the scenes. One notable point is Jules, who not only gives out sincere advice directly to the viewer but also acknowledges said viewer’s mother’s sexual past by making one joke on each video.

I was astonished by Spec Ops: The Line and the way while game is actually a 4th wall breaker: starting from mentioning your nickname in the credits as a guest star, following with jokes after battles like "where ia all this violence coming from? Ah, it's probably from videogames" and with an amazing finish (no spoilers here!) when basically whole ending scene dialog is not for your character but for you personally. Breathtaking
I'd say about half of the characters in the "Shantae" series are aware they are in a game, particularly the Squid Baron, though Shantae herself never seems to know what these characters are talking about.
Rare love their fourth wall breaks. I remember in Battletoads on NES there are flying enemies that steal bits of your life bar from the top of the screen.
The Bard from The Bard's Tale. Where else do you have a main character arguing with the narrator over the most meta things? Seriously, The Bard should be #1 on a list like this. Almost the entire game is chock full of 4th wall breaking stuff!

They should have added the protagonist for saints row 5. Cause if you idle for a certain amount of time your character will say "hello, is anyone there?" Referring to you, the player. Or sometimes they'll say something along the lines of "pick up the controller, PICK UP THE CONTROLLER."
The philosopher in "Secret of Evermore" talk to him long enough will end up with him stating "This is a video game!" Not that your character believes him, and you can turn him into a goat, chicken or basket (or be nice and leave him as is so you get a reward) but he knows. One of the Two Guys From Andromeda in Space Quest 3, he even asks the player if the game was worth $59.99.
Neptune from the Neptunia series. 4th-wall breaking is constant to an annoying degree where she has to drill the fact that she's the series protagonist on every single game, even if it meant destroying the mood or tension the story actually makes. Also, reminder that she also helped in committing basically genocide just because she is fed up of being treat as a gag character getting hurt (because it is a spin-off where she isn't the protag, so she gets treated as a side character).
Was also another PS2 title called "Killswitch" where the MC comments "I feel like my every move is being controlled by someone else" (I think that was the statement he made) thus joking on the fact he somehow knew he too, was in a video game.
You missed the best line in Deadpool, when he wonders who will play him in a movie then mentions Ryan Reynolds, the game came out 3 years before he officially played Deadpool 😁
What about Vampires the masquerade? One big conversation is with a Malkavian thin blood and you ask her if you win the game in the end? Her reply is that it doesn't matter if you win or lose the game, all that matters is that you bought the game. And then a few other winks through out the game, including telling you about the trap and hinting about a choice you should make

Bug! on the Saturn. The whole thing was about actors and film crews creating a video game. The death screen literally had the protagonist say "Ok Mr hotshot game player, now what?".

The Simpsons Game pretty much starts with Bart finding a copy of the game you were playing. But not only that they later had been the in others that were forgotten about and abandoned with sequels and spinoffs being churned out and more so in later console generations (what about the XBOX 720 or PlayStation 4?). It even sees you jump into levels of non existent Simpson's parody games, like what they were doing before that up until Hit and Run. It even sees you fight the creator of The Simpsons, demanding for an explanation.
The Chaos Theory example...debatable. It was the writers poking fun at the gamey bullshit in the first two games through the characters leaning on the fourth wall, they also did NOT know they were in video games.
How about an honorable mention for Greg the garlic farmer from the Epic NPC Man series by Viva la Dirt League. While not actually a game itself it's a spoof of many different games.
No mention of the fourth-wall break in _Batman: Arkham Asylum,_ where the Scarecrow's fear toxin causes the game to 'crash' then reboot to a nightmare version of the game's intro where Batman and the Joker's roles are reversed...?
In the NDS game Valkyrie profile: covenant of the plume, when you complete the game you will have access to the seraphic gate you will battle varrious bosses of the franchise but the seraphic gate of this game is a parody, there is a line from Valkyrie Hrist "the game title is Valkyrie profile not mortal profile" alluding to the fact that of the 3 Valkyries she has the only one that doesn't have a game with her as the main and there is this game that a mortal is the main character.
The dialogue exchange between Sam and Lambert is more of a jab at the rule of three in video games. Neither make a comment acknowledging that they’re indeed in a video game, nor do they speak to the player
