In Teruteru's case, he was trying to stop Nagito from killing someone and ended up killing Impostor-ya instead
In Mikan's case, she was under the influence of that disease Monokuma keeps talking about
In Chiaki's case, she didn't even know that the fire grenade was filled with poison.
But monokuma didn't kill her for that but because SPOILERS...

My ultimate favourite is the first execution in the first game. Set the bar pretty high! My second favourite is the fourth execution in the second game.

Same for me. I actually quite liked Teruteru. His - slight or not - explicit behaviour and acts didn’t change that for me, to be honest. His facade voice is really smooth too and, I like his country accent a lot, although it’s not very accurate. And getting a great deal upset over a family member that you haven’t seen or heard from in years - it’s understandable why he killed and why he wanted to go home so badly. So the fact that most _Danganronpa 2: Goodbye Despair/Super Danganronpa_ players and watchers didn’t care for his pass away was kind of heart-breaking on/in my end. :>

If it would've been even more brutal if it were real. The human body is about 65% water, so on contact with something as hot as lava, all of that would explosively evaporate.

My theory is they weren’t brainwashed rather Junko saved them at their lowest point like she did the WoH. His was watching his mother die of an illness he couldn’t get the money for so she offered a cure so no one would have to suffer like he did and the program would also help his siblings

I actually felt bad during this one. I myself, am I cook and I love this character so much, how he makes food for everyone, how he talks, and how he acts. This made me cry because the last word he said before he died was "MAMAAAAA", he loves his mom so much that he would want to be with her before he died. This was incredibly sad for myself.
I absolutely loved the "reach for the stars" addition to the execution themes in this game, particularly for the case 3 execution (how ironic). It was so perfect for the usual combination of ghoulish cheerfulness and bloody murder.
I'm sure that Mikan reached some of those stars...
Not only that, but it plays when the one being punished is at least semi innocent.
In Teru's case, he tried to kill Nagito in order to save everyone else. However, he accidentally killed Twogami
