Reference ranges are based on the sample population averages so they will vary with said sample size. That’s why when you reach the high end or low end of a reference range, even if within the range, it’s good to look at it more carefully.

Hi Amina, it is hard to know what you mean by "all my blood work" different doctors test for different things. Insulin resistance and high insulin is a very common cause for high SHBG. You need to find a doctor that can look a little deeper than the surface level, like a functional medicine doctor.
I found out I have very high SHBG I also have PMDD so I think that maybe why. I also have low progesterone and estrogen. What do u suggest would be a good way to fix this?
My shbg runs high, free testosterone was low. I recently switched from testosterone cream to injections, due to fear of the cream getting on my children. My estradiol runs on the low side, since I am 45 and perimenopausal. I am on a low dose estradiol patch and 50 mg compounded estrogen, due to very low estradiol levels ( night sweats, fatigue, achy legs, etc). Testing still indicates lower in the range, but within the range estradiol. I am also on a combination of levothyroxine and sublingual Np thyroid. Two questions. 1- could the thyroid medication or transdermal estrogen be driving shbg up, despite my levels being in normal ranges? ( I don’t have a thyroid). 2- it could be a coincidence, but I am noticing that the two days after I inject 15 mg of testosterone cyprionate, I’m irritable and a little sad. Could it be the testosterone? Could taking testosterone cause a temporary increase in shbg, causing these moods, or is it just estradiol that does it? I’m perimenopausal, do I get moody, but it is just weird that it’s the couple days after I inject the testosterone.
Hi Katie Bugs Room. Here are a couple things to keep in mind, estradiol is the main estrogen that should be tested. The ranges for this vary widely through your monthly cycle. If you are not checking at the same time of the your cycle each month, the results don't mean much. The issues mostly like is coming from too much estradiol not too little. Estrogen increase SHBG. Testosterone increases estradiol. Typically women in perimenopause have estrogen dominance already. It is possible that you don't but you might be getting too much now. Hope that makes sense.

He thinks it’s just psychological and gave me a Cialis Rx. Do you think my slightly higher than normal SHBG and estradiol could be causing it? I was thinking of taking a mild aromatase inhibitor to see if it will lower my estradiol and SHBG. He thinks I’m fine because my testosterone is high
Hello Logan! A free T of 16.3 pg/ml is decent. High or high normal estradiol can have a negative effect on erectile function. It won't do this to everyone though. It may be worth while to have a closer look look at the estradiol and check the MS/LC estradiol. This is a more accurate test.
Hi, I'm confused as to why my SHBG is 173, my total testosterone is 53. I'm a 30 year old female. I have read that it usually isnt like this. I've never had birth control. Any insight on what might be going on? My progesterone to estrogen ratio is 45.16.
Hi Samantha, high estrogen is often a cause but bc estrogen is changing all the time (with your cycles), you have to check at different times of the month. Also you have to check when you are non-fasting as the level will be lower.
Hi Tina, It sounds more like your estrogen is high. Androstenedione is converted into either testosterone or estrogen. Since you have high SHBG I would predict higher estrogen levels. As for the question why is harder to answer but the production of androstenedione comes from adrenal glands and the ovarian tissue. Do you have a direct measurement for estradiol or testosterone?

Hi Beatriz, get a comprehensive evaluation of digestive tract (through a naturopathic or functional medicine doctor). You may not have any overt digestive symptoms but it is a common source for chronic health issues of unknown etiology. Also is that total testosterone or free? what are the units?
Yes it can because if something is causing high SHBG there is less bioavailable testosterone (low free T). Low Free T can cause low libido and ED. The part that does not fit is the sperm count. To my knowledge SHBG does not relate to sperm count.
What would it mean if both my shbg and my testosterone is high? I did start birth control for regulation of hormones/period but I always see that the relationship for the two is that one is high and one is low
Hi Yolinna, high estrogen/ estradiol is a common cause of high SHBG in females. I'm not sure how your BCT will affect affect SHBG. It will reduce your bodies estradiol and therefore reduce SHBG. Because some BCT contain estrogen, it will likely increase SHBG more. There are too many variables to know for sure without and in depth evaluation.
Hi!. I'm 27 male without libido problems, with quite good body composition. So basically my total testosterone and SHBG are above range, free testosterone is below. What can be the cause of this result?
I’m a 28 year old male. My total T is 741 ng/dl, free T is 18.09 ng/dl, and my SHBG is 54 nmol/L which all seem to be on the higher end of normal. I don’t understand why my SHBG is this high given the amount of free testosterone I have. My estradiol was measured at 34 pg/ml which is also on the high side of normal. Could that be the reason why my SHBG is on the higher end?
You are underestimating this issue for high shbg low free test one and normal test one. Fix all of these and nothing changes it's like a curse. All of ot these guys with the symptos of mentioned in the beginning are aslo physically active. So its not like their a guy that jacks off all day and eat what ever they want.

My A1C was high-normal (just before “pre-diabetic”. I have read insulin resistance can lower SHBG, but I’m not even pre-diabetic, yet.

Yes it can. There is a video on fatty liver and liver enzymes coming up.

Hello Doctor I am from India.
My name is Nikunj and I am 14 year old boy. I am suffering, (*sorry) having T1D (Type 1 Diabeties) but actually having a good control on sugar levels as I am taking Insulin and monitering blood sugar levels. But I have other problem in my body, I am growing hair all over my body, like arm hair, chest hair, hairy legs, and unwanted hair growth of forehead and surface of nose. None of my friends have this issue and I think I am having high testesterone. I went to dermatologist and she prescribed me two tests. One is testesterone serum and other one is the thing you are talking (SBHG). The only doubt now I have is, Is it anything connected to T1D? If I take medications for controlling Sbhg or testesterone levels, will it affect my diabeties? Or even any other hormones in my body? If yes, please inform doctor 😊

Hi. I have very high SHBG and very low progesterone. Does it mean I can have PCOS? Also suffer from hairloss. Might caused by this hormonal imbalance? Thank you
Hi Zuzana usually with PCOS there is low SHBG. Hair loss can be from high androgens but not low progesterone or high SHBG.

Is it normal to have a high shgb if you have type 1 diabetes?
Not if you are taking insulin. Insulin usually lowers SHBG.

Thanks for the video! I just got an at home Testosterone test done and my test levels where normal, but my SHBG was a bit high. I've been in a modest caloric deficit for roughly 14 months losing about 1.5 lbs per month and I've done so via intermittent fasting and tracking my calories. Sounds like there's a decent chance this is the reason for my slightly higher SHBG. Going to go off intermittent fasting and see how I feel and go get retested.
Best of luck! Thanks for sharing and I think that sounds correct.
Watched a lot of these videos and no one as of yet tells you the range. I got three blood test, dates 4/6/23 SHBG 68.9 - Range 19.3-76.4 One lab. Second lab: 4/20/23 SHBG 57.2- *Range 22-77; Third lab: 5/15/23 69.5- *Range 21.4-67.8. So the third lab shows out of range according to the range all three were out of range if I used the thrird lab range. Anyone know what the true range is?
Reference ranges are based on the sample population averages so they will vary with said sample size. That’s why when you reach the high end or low end of a reference range, even if within the range, it’s good to look at it more carefully.
More importantly, why would it be LOW? And how does one fix it?
What are the symptoms?
Hi,
Would you be able to help me. I am 26’year old female. All my blood work came back as normal apart from
SHBG. What could this mean? I’m not on any medication or have any health issues. I was also told this is the reason as to why I have irregular period too
Hi Amina, it is hard to know what you mean by "all my blood work" different doctors test for different things.
Could be usually estrogen and SHBG run opposite so u might double check that. It’s different at different times of cycle
Hard to take anyone serious with a name like that.

hi! i’m a 20 year old female and just had a fasting blood test done. my shbg was 190.30 nmol/L and my t3 was slightly high at 4.1 pg/mL. i was wondering if these had any correlation? thanks!
Hi. My total test is 854ng/dl. Free T is 16.3pg/ml , SHBG 54.3 Nmol/l and estradiol 43 pg/ml(slightly higher than normal range). I went to the doctor for mild to moderate. ED.

Hi,
I'm confused as to why my SHBG is 173, my total testosterone is 53.
Also 53 total testosterone is not that high.
What is normal? What is high?
I am a 42yo female and my SHBG is high. So is androstenedione. Can you help me understand why? Does it mean my testosterone is high or low?
SHBG is 138 and the range is 30-135. Androstenedione is .915 and the range is .19-.82
Hi Tina,
It sounds more like your estrogen is high.
Hi. I'm a 64-year-old male that works out 3-4 times a week. I do HIIT and lift weights. I also do intermittent fasting 16/8. . My total T is 1086 (high). I did have prostate cancer 3 years ago and did 2 rounds of brachytherapy radiation I am cancer free now. It runs on both sides of my family. I knew I was going to get it, just didn't know when. I didn't know if that could have something to do with this?
Hi
My testosterone was high at 42nmol/l and SHBG high at 79nmol/l so it worked out my free testosterone was in normal range. Should I worry or is there anything I can do 🤷♂️?
Hi John C, total testosterone of 756 ng/dl seems normal to me. I would not worry but if you have low T symptoms, you might want to look into it more. You do have to check the SHBG in fed and fasting states bc it will change with this.
hey im 34/male, gym 3-4 times a week, higher protein diet, moderate fats and carbs and eat at maintenance calories and im not to sure why my shbg is so high love to hear your thoughts thanks in advance , my blood results are

Yeah that SHBG seems really high but I am use to different units. Have you had an estradiol done with MS/liquid chromatography? This estrogen seems normal but would be curious what the more accurate test shows. Was the test done fasting?

Hi Beatriz, get a comprehensive evaluation of digestive tract (through a naturopathic or functional medicine doctor). You may not have any overt digestive symptoms but it is a common source for chronic health issues of unknown etiology. Also is that total testosterone or free? what are the units?

Hi, could the cause of low sedrive, bad erections and low sperm volume be the cause of high shbg? My total t was fine. Shbg not checkdd yet
@@swintegrative thanks it was worth a try lol I know our bodies are complicated

FSH 5.06 mlU/ml (1.50 - 12,40)
LH 4,64 mlU/ml (1.70 - 8.60)
Estradiol 18,10 pg/ml (11.30 - 43.20)

Total Testosterone 864 ng/dl (249 - 836)
SHBG 65.76 nmol/l (18.30 - 54.10)
I've calculated free testosterone, it is 12.2 ng/dL. So basically my total testosterone and SHBG are above range, free testosterone is below. What can be the cause of this result?
check SHBG in fed state.
