    In this session, Ana Quiñones, PhD and David Dorr, MD discussed findings from the PCCP4P project, including a high-level overview of models and approaches to providing person-centered care planning (PCCP) for people with multiple chronic conditions (MCCs). The webinar highlighted learnings, gaps, persistent barriers, and promising solutions to the adoption and promotion of high quality PCCP.

    The HCSRN-OAICs AGING Initiative is an NIA-funded joint endeavor of the Health Care Systems Research Network (HCSRN) and the Claude D. Pepper Older Americans Independence Centers (OAICs) to create a national resource to advance interdisciplinary research focused on older adults with multiple chronic conditions (MCCs).
