im not clear still my concern is if we block factor 10 meaning we will block thrombin and its ability to enhance the intrinsic pathway so then both drugs should require monitoring OR are we sayin that the extrinsic pathway generates thrombin which then enhances intrinsic pathway so the anticoagulant effect is mild then unfractionated

It is because warfarin inhibits protein C (a thrombolytic factor), and so can cause a paradoxical hypercoaguable state, until the clotting factors can be adequately inhibited, therefore heparin is used intermittently to "bridge" this problem.

Sir so we cant monitor lmwh as both pt aptt will be prolonged but if we use uf heparin pt will also be prolonged as commen pathway trombin is inhibited so aptt will be more prolonged that pt is this is what you are saying
