SodaBicarb is not an effective buffer at physiological Ph , what does it mean ? And sir my 2nd question can we give Sodabicarb in sever respiratory distress bcoz CO2 produced by HCO3 has to be removed by a healthy Lungs
concise and useful can you please make video on how to calculate fluid requirement in which fluid is mainstay treatment like D KA, s epsis, acute pancreatitis
Sir I am a pediatrician and follow your channel for updates . thank you.just for inquiry as there is increased capillary leak in pancreatitis and in denge,is urine output not a reliable marker for hydration in denge also?bcoz we depend heavily on it.
Yes it is , but if ATN sets in , we may fluid overload the patient with pulmonary odema and abdominal compartment syndrome . That’s why keep an eye on urine output but also monitor urea, creatinine , serum albumin and other clinical parameters. Hope this clears the doubt
Sir I have a doubt . I had a patient in ICU with CCF and ef 15% . Patient also had acute pancreatitis. His cvp was 23 cm of water. Now what to do in such cases. Kindly suggest sir
Yes this is a tough condition , but CVP can help but the only indicator for deciding fluids … you have to give challenges with fluid and see how the patient responds like chest crepts , IVC status and others … it’s basically a bedside assessment, and challenging.
Sir,one reminder as you promised that you will be uploading more and more content but there is no any long videos uploaded recently....hope everything is going good ...Thanks🙏🙏
