Having been a nurse almost 40 years in many critical care areas (and now an educator) and having read a lot about SBAR and been a nurse when SBAR was first introduced, I have some insight. A nurse's physician assessment findings are certainly valid, but A-assessment was originally intended for the nurse to assert what they think is going on (a medical diagnosis of sorts) and then the R-recommendation of what they recommend happen. Providers like smart, assertive (while being respectful) nurses who can be their eyes and ears. One's A-assessment (e.g., I worried he's septic") might be off or even wrong, but go ahead and ASSERT yourself.
