I think it's mostly because in season 2 he went back to Addison, but then continued to string Meredith along. Like Addison says at some point, Derek has this hero complex where he constantly wants to be seen as the good guy, even when he is actually in love with the other girl. At least that is what annoyed me the most about his character in the early seasons.
I think the “hero complex” comes from being forced to be in charge of the family at a young age, after his dad’s shooting. It really affected him, but it was never really unwrapped (which I think was a lost opportunity)!
