    Dr. Tala, thanks for your sharing. Recently, I’ve met a situation that a baby is born with donor egg and father’s sperm, When we wanted to simultaneously take mother’s blood and baby’s blood for type and screen. That mother asked if it is appropriate to take her blood for the baby’s screening because she is not the owner of donor egg

    While my senior nursing colleague said it is fine even though mother is not really the owner of donor egg, but she did give birth to this baby. I wondered why we should take this mother’s blood for type and screen instead of tracing the donor egg’s owner for type and screen

    Is it because we wanna check whether there is any antibodies produced during pregnancy? But what’s the role of taking maternal blood for type and screen? I can’t really find the answer after searching though online 🙈Thank you Dr. Tala

      Hi Jeanie- this is SUCH a good point- and something we're seeing more of (donor eggs etc). Remember that the gestational mother (the one carrying the baby- let's call her Julie) will produce antibodies based on what's happening in Julie's body. So if she is blood type O, then she will have anti-A and anti-B antibodies no matter what the growing fetus has (as all O motehrs do). If she has Rh negative (and this is her first pregnancy)- it is unlikely she will have antibodies against Rh antigens- but she may develop them this pregnancy if baby is Rh positive. Mother may also have been exposed to other antigens- eg a previous blood transfusion, so she may randomly have formed antibodies to other RBC antigens (kell, etc). So even though Julie's baby isn't biologically her own, it's Julie's antibodies that can cross placenta and break down the baby's RBCs. We ran a bilirubin series- and I wish we touched on points like this more- but maybe check out the hemolytic one. Was this answer helpful?!

      Hello Maja! This is an excellent idea- one we hadn't considered before! After we get through a few more requests, we plan on starting a symptom based series (e.g. bleeding of gut, or green emesis or increase in apneas)- and then run through differentials and what we should be doing next. Thanks so much for your suggestion. We promise we'll get there eventually!
