I thought hematoma occurs if we deposit LA in artery thats why we aspirate.. But if hematoma Occuring even after withdrawing needle following positive aspiration what is the use of aspiration
Sir last week I extracted 36. While giving IANB i got positive aspiration. I withdrawn the needle and given the block again. It was fine. Patient was also fine. But after 1 week when she came for suture removal she was having hamatoma ( swelling with bluish discoloration ) at her angle region. She was having no pain intra orally but the swelling was tender on palpation. I prescribed analgesic and antiinflammatory. I was about to precribe for heat application. But my senior Dr told me not to. Bcz haematoma may get worse according to her experience. Sir my question is Why this haematoma happened after some days of extraction and what was i suppose to do in this case??? Plz rply sir
Hematoma occur due to extravasation of blood in peripheral cells, there is no need to worry. It will resolve after 7 days. Heat pack is good choice it will resolve the problem

In this case try articaine for buccal and lingual side infiltration.. patient feeling pain due to acidic nature of periapical area in that tooth due to abscess. But try to avoid injecting inside abscess.
